[Local anesthesia in ophthalmologic surgery].

Operations of the anterior segment of the eye are dominant in ophthalmic surgery. It is a lesser known fact that these are the most common elective surgical procedures of all. There has been a dramatic change of anaesthetic practice for ophthalmic surgery over the last twelve years. LA is the anaesthetic of choice nowadays unless contraindicated. These changes were brought about due to the fact that anaesthetists initially became involved and then took over the performance of LA in ophthalmology. Peribulbar technique was introduced in 1986 as a safer alternative to retrobulbar anaesthesia. The…

Ophthalmologic manifestations of rheumatic diseases.

OBJECTIVES: Detailed review of the manifestations of eye involvement in the context of rheumatic diseases. METHODS: An OVID Medline search of the rheumatology and ophthalmology English literature related to the eye manifestations of human rheumatic diseases from 1966 to the present was conducted by the authors. RESULTS: Analysis of 300 recent and consecutive rheumatology consultations from a large Veterans Administration Healthcare System shows that 4% are referred for eye manifestations of suspected rheumatic diseases, most commonly, anterior uveitis and keratoconjunctivitis sicca (KCS). Oc…

Ophthalmologic emergencies in the patient with diabetes.

Diabetes is associated with many emergent ophthalmologic conditions. The management of patients with diabetes requires careful monitoring for visual symptoms and frequent physical examination for signs of retinopathy. Randomized studies have documented a significant reduction in the development of new retinopathy and the progression of existing retinopathy with tight control of diabetes. Photocoagulation laser therapy is helpful in preserving vision in severe nonproliferative retinopathy, for proliferative retinopathy, and for clinically significant macular edema. Vascular events include arter

Neuro-ophthalmologic aspects of aneurysms.

The visual pathways and the ocular motor cranial nerves are frequently injured by expanding cerebral aneurysms. Neuro-ophthalmologic signs and symptoms may be the only indications of an aneurysm prior to rupture. Acute or chronic visual loss may herald an aneurysm prior to rupture. Acute or chronic visual loss may herald an aneurysm in the carotidophthalmic, supra clinoid carotid, internal carotid bifurcation, or anterior communicating artery distributions. Diplopia and retro-orbital pain may be warning signs that precede the discovery of a posterior communicating, basilar, or cavernous sinus

A ray tracer for ophthalmological applications.

Ray tracing with a personal computer allows realistic simulation of optical properties of the human eye. Patterns of point sources are used as objects. The path of light rays is calculated between the point source and the retina for a Gullstrand eye model with improved parameters; the normal eye model has a resolution limit close to the natural resolution limit of the human eye. The image formed on the retina is projected back to a screen at the distance of the object so as to simulate image interpretation by the brain. Refractive errors are modeled by a change in eye parameters and corrected

Ophthalmological signs of tuberous sclerosis.

A patient with tuberous sclerosis and retinal astrocytic hamartomas is described. The different types of these astrocytomas are illustrated as well as systemic signs of Bourneville disease. Finally the value of the ophthalmological examination in the diagnostic work-up prior to genetic counselling the patient and/or the family members is discussed.

The ophthalmologic manifestations of mitochondrial disease.

The cardinal eye manifestations of mtDNA diseases are ophthalmoplegia, optic neuropathy, and pigmentary retinopathy. A number of other eye structures may also be affected in these disorders and the ophthalmologist is in a unique position to detect and interpret these findings. The presence of these ophthalmologic manifestations may be the first clue that the patient has an underlying mitochondrial disease with the eye as the initial or most prominently affected organ. The phenotypic manifestations of mitochondrial disease are protean and variable, and there are no clear-cut, minimal features t

Ophthalmologic abnormalities in encephalocraniocutaneous lipomatosis.

Encephalocraniocutaneous lipomatosis (ECCL) is a sporadically occurring disorder that belongs to the group of neurocutaneous syndromes. Important characteristics of the case we present are: intracranial lipomas, a skull hamartoma, bilateral lipodermoids and jaw tumors (ossifying fibromas and compound odontomas). We propose four minimal criteria for the diagnosis of ECCL and review the ocular abnormalities reported to date.

Ophthalmological aspects of Alzheimer's disease.

Patients with Alzheimer's disease may present to the ophthalmologist with visual symptoms that precede or overshadow the features of dementia. This visual variant of Alzheimer's disease has been recognized only recently and typically appears in late middle age as progressive visuospatial and reading dysfunction, with normal visual acuity and fields. Common features include simultanagnosia and the other features of Balint's syndrome, and variable hemispatial neglect and hemianopia. The neuroimaging and neuropathological findings in this Alzheimer's variant are distinct and suggest disruption of

Ophthalmologic features of thallium poisoning.

Thallium intoxication is characterized by the development of painful peripheral neuropathy, alopecia, mental disorders, and in severe cases, respiratory failure and death. Toxic optic neuropathy is also a feature. Ophthalmologic features of thallium poisoning include optic neuropathy, blepharoptosis, lens opacities, and ophthalmoplegia. A 44-year-old man with criminal sublethal thallium poisoning was examined one month after he was seen in the neurology department with classic systemic features. He was found to have diminished contrast sensitivity, a tritan defect in color vision, and a relati

Ophthalmologic findings in biotinidase deficiency.

Biotinidase deficiency is an autosomal recessively inherited metabolic disorder characterized by neurological and cutaneous manifestations and metabolic abnormalities. We studied 78 symptomatic children and found that 51% had ophthalmologic abnormalities. These include infections (30%), optic neuropathies and visual disturbances (13%), motility disturbances (13%), retinal pigment changes (4%) and pupillary findings (1%). The most commonly reported findings are optic atrophy and keratoconjunctivities. Although the disorder can be effectively treated with biotin therapy, untreated children are a

Ophthalmologic manifestations in MELAS syndrome.

We describe a 15-year-old boy with full-blown mitochondrial myopathy, encephalopathy, lactic acidosis, and stroke-like episodes (MELAS) and chronic progressive external ophthalmoplegia (CPEO). He presented with visual disturbance, hearing impairment, continuous partial epilepsy on the right aspect of the face, and right hemiparesis since the age of 13. Four months later, he experienced another strokelike episode with continuous partial epilepsy on the left hand. Serial computed tomographic scans revealed bilateral parieto-occipital hypodense lesions with gyral enhancement and an additional low

[Ophthalmologic manifestations in patients with ichthyosis].

Different manifestations on some ocular and adnexal structures may accompany ichthyosis. Some ophthalmological manifestations in the clinical course of ichthyosis may endanger the eye and even lead to the sight loss. The paper reports a case of a patient with congenital ichthyosiform erythrodermia and ophthalmological manifestations on the lids and cornea. In addition to ichthyosis of the lid skin, squamous blepharitis and cicatricial ectropium of the lower lids were also present, while signs of exposure keratitis on the right eye and corneal leucostaphyloma on the left eye were also noted. Dyslexia; ophthalmological aspects 1991.

Dyslexia or specific reading disability is an important and frequent handicap afflicting 5-10% of the population. It is basically a disorder of acquisition of written language, probably due to a poorly developed phonological awareness, which in turn may be neurologically related. As anatomical correlates symmetry of the planum temporale and ectopies in the cerebral cortex have been suggested. Functional correlates are discovered with brain electrical mapping and stimulation of brain structures during neuro surgery. From an ophthalmological point of view there are no relations between dyslexia

[Ophthalmological aspects of Reiter's disease].

Clinical picture and course of eye involvement were studied in 254 patients with Reiter's disease. Altogether 130 patients suffered from conjunctivitis of varying severity, 23 from keratitis, 21 from episcleritis, 20 from uveitis, 2 from detachment of the retina, and 5 from secondary glaucoma. Chlamydia were detected in one third of conjunctivitis patients in scrapings off the conjunctiva. The eyes may be accidentally infected by agents from the urogenital organs if the patient does not observe the hygienic rules. Uveitis developed as a rule after a prolonged course of Reiter's disease. They w
