PURPOSE: To review the advances in the surgical techniques of orbital decompression.
METHODS: A literature review of orbital decompression surgery and experience-based consideration of bony areas for decompression.
RESULTS: In the 100-year-plus span of orbital decompression literature, multiple incisions and multiple bone and fat removal strategies have been described. In general, bone removal has been conceptualized as consisting of 4 walls of the orbit. However, the orbital bony anatomy is more subtle than a simple box, and considering 6 areas of potential bony removal allows a more nuance…

Congenital orbital fibrosis (COF) is a rare disorder characterized by an infiltrating orbital mass with secondary involvement of the extraocular muscles that may present with extraocular muscle dysfunction, and globe and eyelid abnormalities in infancy. This condition is thought to be a nonprogressive process and literature on longitudinal assessment of COF is limited. The authors describe a case of COF which was followed for 15 years. The patient had stable symptoms of ocular dysmotility and ptosis but was noted to have spontaneous regression of the orbital mass on serial MRI.

A 10-month-old girl presented with eyelid edema and erythema that did not improve with systemic antibiotics. Due to a lack of improvement, MRI was performed to avoid ionizing radiation from CT. An orbital abscess was recognized and drained. However, the abscess recurred 2 times. CT scan was performed and a tract in the sphenoid bone helped to diagnose a congenital dural sinus tract with dermoid. Definitive surgery was performed with neurosurgery to remove the entire tract including cutaneous connection. CT scan proved critical to diagnosis and should be considered in infants in select cases de…

Our objective was to describe our experience with orbital amyloidosis, and illustrate the different forms of presentation. This was a retrospective case series of four patients with biopsy-proven orbital amyloi- dosis, over the period from 2014 to 2016. We describe its diagnostic and clinical characteristics, management and systemic study. The series comprised three women and one man of mean age 52 ± 9.4 years. Affected sites were the lacrimal gland, tarsal conjunctiva, lacrimal sac and orbit. In three of the four patients, calcifications were observed. Three patients had associated ptosis. Th…

We reported two patients of orbital myositis with abnormal accumulation of 6 Ga in the bilateral orbits. The first patient, a 34-year-old man, developed bilateral lid swelling and external ophthalmoplegia. T1-weighted MRI of the bilateral orbits showed enlargement of the bilateral extraocular muscles. 67Ga scintigraphy also showed abnormal accumulation in the orbits. The second patient, a 64-year-old woman, developed diplopia caused by restricted abduction of the bilateral eyes. 67Ga scintigraphy showed abnormal accumulation in the bilateral orbits, in spite of no finding in the MRI of the ext…

PURPOSE: To report a case of a respiratory epithelial-lined, air-filled orbital cyst as a late complication of orbital fracture repair.
METHOD: Case report.
RESULTS: Recurrent episodes of diplopia and hyperophthalmia developed secondary to pneumatic inflation of a respiratory epithelial-lined orbital cyst 6 months after orbital fracture repair. This cyst remained in communication with an ethmoidal air cell and became inflated during pressurization of the ethmoid sinus. Removal of the cyst and orbital implant was curative.
CONCLUSION: Air-filled, respiratory epithelial-lined orbital cyst is …

Before reconstruction of the orbital walls and other surgical procedures concerning the orbits leading to a modification of pathologically altered orbital volumes, it is useful to measure these volumes in order to allow preciser correction. Orbital volumetric studies on 22 patients and 6 dry skulls were performed using high resolution computer tomography. 14 patients presented enophthalmos of various origin, 3 patients fibrous dysplasia involving the orbits and 5 patients showed no orbital pathology. In 10 patients with unilateral posttraumatic enophthalmos an increase of the bony orbital volu…

A combined orbital decompression procedure has been found effective in treating proptosis. The procedure includes a lower eyelid incision by an ophthalmologist to expose the floor of the orbit, while a Caldwell-Luc antrotomy is performed simultaneously by an otolaryngologist. The ophthalmologist has a better view of the contents of the orbit and the anterior orbital floor. The otolaryngologist has better access to the posterior orbital floor and ethmoid sinus. Together, isolation and preservation of the second division of the fifth cranial nerve are facilitated. The headlight worn by each surg…

CT scans of 9 patients with orbital pseudotumor (bilateral in 6 and unilateral in 3) showed findings distinct from those observed in Graves' ophthalmopathy. In bilateral involvement, they ranged from localized mass lesions to complete obiliteration of normal orbital CT anatomical landmarks; diffuse or multifocal lesions involving the posterior globe and muscle insertions were most typical of the diagnosis. However, findings in unilateral psedotumor may be indistinguishable from orbital mass lesions other than Graves' ophthalmopathy. Serial CT scans were used to show progression of disease and …

Topological insulator (TI) surface states exert strong spin-orbit torques. When the magnetization is in the plane its interaction with the TI conduction electrons is non-trivial, and is influenced by extrinsic spin-orbit scattering. This is expected to be strong in TIs but is difficult to calculate and to measure unambiguously. Here we show that extrinsic spin-orbit scattering sizably renormalizes the surface state spin-orbit torque resulting in a strong density dependence. The magnitude of the renormalization of the spin torque and the effect of spin-orbit scattering on the relative sizes of …

Orbital emphysema refers to the presence of air within the orbital tissues and most commonly develops following trauma involving the paranasal sinuses or orbital walls. Although typically benign and self-limiting, it may rarely cause vision-threatening complications due to orbital compartment syndrome. The authors report a case of traumatic orbital emphysema presenting with clinical features suggestive of an orbital floor fracture. A 35-year-old man presented with left periorbital swelling and limitation of upgaze after blunt trauma. Orbital computed tomography demonstrated intraorbital air co…

OBJECTIVE: To analyze the visual outcomes and sequelae of endonasal intervention for orbital infections.
INTRODUCTION: Orbital infections pose a serious threat to vision in the pediatric population and can result in complications such as blindness, diplopia, intracranial involvement, and death. [1] Orbital decompression by endonasal intervention is a common treatment to address a variety of orbital infections including orbital cellulitis, orbital abscesses, and subperiosteal abscesses. [2] The outcomes of visual sequelae such as loss or limitation of visual acuity, extraocular movements, and …

Dermatofibrosarcoma protuberans (DFSP) is a rare sarcoma of the dermis. It is a malignant, locally aggressive, and infiltrative tumor with frequent recurrence. In this case, a 44-year-old woman presented with a 15-year history of a swelling in the medial canthus of the right eye that caused tearing. Imaging revealed a septated mass isodense to soft tissue that had eroded the medial wall of the orbit. Macroscopic examination showed an elastic, gray-brown, encapsulated, irregular mass measuring 45x35x22 mm. The surgical margins were positive, so adjuvant radiotherapy was started. The patient was…

Delayed orbital apex syndrome (OAS) is rare during orbital blowout fracture reconstruction. A 30-year-old woman fractured the right orbital floor in a fall and undergone repairing operation 2 weeks later. After severe sneezing on postoperative day 10, she gradually arose vision loss, ophthalmoplegia, ptosis with a dilated and fixed pupil within few hours, then consulted our department and was diagnosed as OAS. Computed tomography scan showed displaced implant and retrobulbar emphysema resulting in a constellation of compression to orbital apex. Therefore, an exploratory operation was engaged t…
