Orthoses in the Treatment of Non-Specific Back Pain - A Systematic Review.

Non-specific back pain is of great importance in health economics, due to its high impact on the ability to work and the need for rehabilitation. The effectiveness of orthoses in patients with low back pain is unclear, as these studies have heterogenous designs and methodological weaknesses. The aim of this work is to formulate evidence-based recommendations for the use of orthoses in patients with low back pain. The literature search was conducted according to the PRISMA protocol (Preferred Reporting Items for Systematic reviews and Meta-Analyses) at PubMed, ScienceDirect, Cochrane and Google S

Design of a Telerehabilitation Software Platform for a Compliant Upper-Limb Rehabilitation Orthosis.

Patients with spasticity require intensive rehabilitation to regain motor function, but access is often limited by travel or healthcare constraints. Telerehabilitation provides a structured approach for home-based therapy, allowing patients to perform more exercises with minimal external assistance. This work presents a telerehabilitation platform for a compliant upper-limb robotized orthosis to support rehabilitation in both home and institutional settings with minimal setup required. Built using the OpenTera framework for rapid prototyping and modular service integration, the platform includ

Assist-as-Needed Impedance and Admittance Switching Control for an Upper-Limb Compliant Rehabilitation Orthosis.

Individuals affected by stroke often experience spasticity, which limits movement and impact activities of daily living. Physical therapy is essential for improving voluntary movement; however, therapists often face time constraints that limit their ability to provide optimal care for patients with spasticity. To address this issue, an assist-as-needed admittance and impedance-switching controller has been developed for an upper-limb compliant rehabilitation orthosis currently in development. This controller allows patients with spasticity to perform exercises independently while complementing

Effect of equipping an unloader knee orthosis with vibrators on pain, function, stiffness, and knee adduction moment in people with knee osteoarthritis: A pilot randomized trial.

BACKGROUND: Wearing unloader knee orthoses for the long term may have a side effect on knee adduction moment (KAM). RESEARCH QUESTION: This study sought to determine whether equipping an unloader knee orthosis with vibrators improves its effectiveness in pain, stiffness, function, and reducing the KAM. METHODS: The authors performed a clinical evaluation with the Western Ontario and McMaster Universities (WOMAC) questionnaire and instrumented gait analyses on 14 participants with medial compartment knee osteoarthritis in two testing sessions: before wearing the orthosis and after 6 weeks of

The first case report of multiple thoracic vertebrae fractures caused by a low-voltage electric shock.

BACKGROUND: This paper describes a unique case-the first case of multiple fractures of the thoracic vertebrae caused by a low-voltage electric shock. CASE PRESENTATION: A 22-year-old male patient was diagnosed with compression fractures of Th2-Th6 caused by a muscle spasm resulting from an electric shock. The patient was treated conservatively using a cervico-thoracic support corset. After rehabilitation, the patient has regained his physiological movement of the spine without any back pain. CONCLUSIONS: Albeit vertebral fractures caused by electric shock injury are extremely rare, clinician

Reaching higher: External scapula assistance can improve upper limb function in humans with irreversible scapula alata.

BACKGROUND: Scapular dyskinesis, i.e., the deviant mobility or function of the scapula, hampers upper limb function in daily life. A typical sign of scapular dyskinesis is a scapula alata-a protrusion of the shoulder blade during arm elevation. While some reversible causes of scapula alata can be treated with therapy, other, irreversible causes require invasive surgical interventions. When surgery is not an option, however, severe limitations arise as standard approaches for assisting the scapula in daily life do not exist. The aim of this study was to quantify functional improvements when ext

Printing a static progressive orthosis for hand rehabilitation.

BACKGROUND: Static progressive orthosis is used for the treatment of severe joint contracture after trauma and/or surgery. However, a custom-fabricated static progressive splint would be expensive and labor intensive. Especially, owing to very limited payment under the current Taiwanese National Health Insurance, the incentives to fabricate a patient-specific splint are insufficient for a therapist. To ease splint construction, we introduced three-dimensional (3D)-printed "shark fin"-shaped device works as a static progressive orthosis for the hand rehabilitation. The aim of this study was to

The effect of extensible and non-extensible lumbosacral orthoses on anticipatory postural adjustments in participants with low back pain and healthy controls.

BACKGROUND: Wearing a lumbosacral orthosis (LSO) is known to influence spine mechanics, but less is known about how LSOs affect motor control. Whether the use of a LSO can negatively affect motor control of the lumbar spine is still under debate. OBJECTIVE: The current study examined the immediate effects of two flexible LSOs (extensible and non-extensible) on the anticipatory postural adjustments that prepare the spine for a predictable perturbation. DESIGN: A comparative study using a repeated measures design in a laboratory setting. METHODS: Healthy controls (n = 20) and participants wit

Minimally invasive endoscopically assisted remodelation (MEAR) of sagittal craniosynostosis: an alternative technique to open and endoscopic procedures with cranial orthosis time span reduction.

INTRODUCTION: Sagittal craniosynostosis represents the most frequent simplex skull suture pathology. There are currently several operative approaches to this defect. Minimally invasive techniques are preferred for young infants. Since July 2017, we have employed endoscopically assisted craniectomies followed by cranial orthosis. Gradually, we have developed our modified technique, the minimally invasive endoscopically assisted remodelation (MEAR). SURGICAL TECHNIQUE: MEAR is a combination of principles gained from classical cranial vault remodeling techniques and minimal invasive approaches.

Comparative efficacy of the dorsal application of Kinesio tape and splinting for carpal tunnel syndrome: A randomized controlled trial.

BACKGROUND: Carpal tunnel syndrome (CTS) symptoms are problematic especially when signs and symptoms are not substantial enough to require surgical intervention. Conservative treatments have mixed effectiveness, yet are one of the best options for mild to moderate CTS. Kinesio tape is an emerging modality, as it provides biomechanical support while allowing movement. PURPOSE: The purpose of this study was to determine the efficacy of dorsal application of Kinesio tape on occupational performance as measured by pain and function in individuals with mild to moderate CTS, as compared with the ac

Flatfeet: Biomechanical implications, assessment and management.

Several complications due to flatfeet have been reported in previous literature such as poor postural stability, injuries, pathologies, and discomfort. Early detection and appropriate management are mandatory to minimize these effects. There are different feet assessments established in the field with distinct advantages and disadvantages. Additionally, selection of management methods from various options should be done vigilantly as the application differs according to the individual. Therefore, the objective of this article is to review previous literature on structural anatomy, pathomechani

Ring orthosis design and production by rapid prototyping approach.

STUDY DESIGN: In this study, ring orthosis design and manufacturing was performed by using rapid prototyping technology. INTRODUCTION: Swan neck and boutonniere deformities may occur because of various reasons such as tendon injuries, sharp object injuries, or rheumatic diseases. Although the ring orthoses are the most commonly used conservative treatment method for swan neck and buttonhole deformities, they cannot be widely used because they are both difficult to produce and esthetically unsatisfied with the patients. PURPOSE OF THE STUDY: In this study, it was aimed to manufacture with an

Developments in the Orthopaedic Management of Children With Stüve-Wiedemann Syndrome: Use of the Fassier-Duval Telescopic Rod to Maintain Correction of Deformity.

BACKGROUND: Stüve-Wiedemann syndrome is a rare inherited condition, which is frequently fatal in infancy. Those patients who survive into childhood demonstrate a complex progressive deformity of the long bones, with high rates of recurrence after initial successful correction. Because of the rarity of the condition there is, at present, limited evidence on the most appropriate treatment. METHODS: We describe our experience in the management of 4 patients, who underwent correction of deformity and fixation with Fassier-Duval telescopic rods. RESULTS: In this series we have seen good correctio

Effect of Custom Orthosis and Rehabilitation Program on Outcomes Following Ankle and Subtalar Fusions.

BACKGROUND: Fractures of the distal tibia, ankle, and foot sustained through a high-energy mechanism can be extremely debilitating, and ankle and/or subtalar fusion may be indicated if the limb is deemed salvageable. Functional outcomes among this population are often poor. The purposes of this study were to evaluate the effect of an advanced rehabilitation program combined with the use of a custom ankle-foot orthosis for patients with ankle or subtalar fusion on selected physical performance measures and patient-derived outcome measures and to determine if the response to treatment was predic
