Facticial panniculitis and Löfgren's syndrome: a case.

We herein report a patient who came to the hospital because of a polyarticular joint pain, fever and cutaneous lesions. She had silicone implants in her buttocks, a surgery performed 3 years before. We made a biopsy of the skin of the buttocks (facticial panniculitis due to silicone) and of the pretibial surface of the inferior extremities (erythema nodosum). A chest X- ray and a CT scan revealed bilateral hiliar lymphadenopathy, and a transbronquial biopsy showed granulomatous inflammation. She had a good response to rest and anti-inflammatory drugs, so the removal of the silicone implants ha…

Lupus erythematosus panniculitis in children and adolescents.

INTRODUCTION: Lupus erythematosus panniculitis (LEP) or lupus erythematosus profundus is a rare form of chronic cutaneous manifestation affecting both adults and pediatric patients. The prevalence of this manifestation was seldom reported in juvenile systemic lupus erythematosus (JSLE). CASE REPORTS: From January 1983 to December 2010, 5,506 patients were followed at the Pediatric Rheumatology Unit of our University Hospital and 278 (5%) of them met the American College of Rheumatology classification criteria for JSLE. Two (0.7%) of them had LEP at JSLE onset. These two cases had tender deep …

[Lipoatrophic panniculitis in adults: treatment with hydroxychloroquine].

BACKGROUND: Lipoatrophic panniculitis is generally considered to be a rare disease affecting children. We report a case involving this condition in an adult patient presenting with striking clinical features and responding to hydroxychloroquine therapy. We discuss the nosological relationship between lipoatrophic panniculitis and connective tissue panniculitis. PATIENTS AND METHODS: A 62-year-old woman was referred to our institution with a six-month history of painful erythematous nodules and plaques on the calves, thighs, buttocks, breasts, abdomen and arms. With each outbreak of new lesion…

Panniculitis ossificans traumatica of the lower leg.

Heterotopic bone formation in the skin, osteoma cutis, occurs in a variety of clinical settings. Primary osteoma cutis arises in otherwise unaffected skin, often in patients with systemic conditions, whereas secondary osteoma cutis is associated with cutaneous neoplasms and inflammation (metaplastic ossification). Metaplastic ossification almost exclusively affects the dermis. Presented is a patient with posttraumatic bone formation in the subcutaneous fat, panniculitis ossificans traumatica, an entity that has only rarely been reported.

[Panniculitis in a 32-year-old man].

INTRODUCTION: The term panniculitis refers to a heterogeneous category of conditions characterized by inflammation of subcutaneous adipose tissue. We report on a 32-year-old man who developed severe panniculitis with subsequent lipoatrophy. The disease initially presented as moderately painful subcutaneous nodules associated with general symptoms (fever, malaise, musculoskeletal symptoms). As the symptoms exacerbated despite treatment with antibiotics and nonsteroidal anti-inflammatory drugs, the patient was referred to a dermatology unit. Erythema nodosum was suspected and some transient impr…

Panniculitis associated with pancreatitis in a cocker spaniel.

A 13-year-old, male cocker spaniel presented with a history of inappetence, depression and reluctance to stand. The dog had multiple, ulcerated skin lesions which were diagnosed as panniculitis by histopathology. A diagnosis of pancreatitis was made on the basis of markedly elevated serum lipase concentrations, abdominal ultrasonography which showed an abnormal lobulated area of hypoechoic tissue in the body and right lobe of the pancreas, and a fine needle biopsy from this area which revealed large numbers of degenerate neutrophils. After treatment with antibiotics and prednisolone, the dog m…

Panniculitis: definition of terms and diagnostic strategy.

Inflammation of the subcutaneous tissue represents a dynamic process that shows different histopathologic findings at different stages of development; therefore, the stage of evolution of a lesion at the time of biopsy influences the microscopic appearance significantly. Furthermore, location and type of inflammation may vary among different examples of the same panniculitis independent of the stage of evolution. For these reasons, the histopathologic diagnosis of panniculitides is often difficult. Currently, the most common approach to diagnosis is differentiation between predominantly septal…

[Bilateral scrotal panniculitis in a prepubescent child].

Scrotal panniculitis or scrotal fat necrosis is an uncommon acute scrotal pathology. We report a case of scrotal fat necrosis in a 9 1/2 year-old over-weight boy with bilateral tender scrotal masses, located beneath the tests. Pathologic findings were those of subcutaneous fatty indurated masses with lipogranulomatous foci. The etiology of scrotal fat necrosis is unknown. A similar condition has been described in young children exposed to cold. In obese prepubescent boys, a greater sensitivity to cold and a higher saturated fatty acid concentration of the scrotal adipose tissue would induce fa…

Neutrophilic tuberculous panniculitis in a patient with polymyositis.

The clinical spectrum of cutaneous M. tuberculosis infections is varied and atypical. Once a major cause of death, with the advent of antimicrobial drugs it had, until recently, been an uncommon consideration in the differential diagnosis of cutaneous disorders. Panniculitis can be caused by a variety of etiologic agents; however, only rarely has it been reported caused by tuberculosis. Herein, we describe a patient with an unusual case of neutrophilic panniculitis caused by M. tuberculosis that arose following corticosteroid therapy for presumptive polymyositis. This case illustrates the need…

[Nodular panniculitis: a manifestation of Lyme borreliosis?].

Infection with Borrelia burgdorferi can induce various skin manifestations. The type of skin manifestation and the histopathological picture depend on the stage of infection and vary from local inflammatory infiltrates to chronic atrophic skin disease. Involvement of subcutaneous tissue has been observed only very rarely. We report on two patients suffering from nodular panniculitis (Pfeifer-Weber-Christian) and present evidence that the disease was caused by Borrelia burgdorferi. In one of the cases Borrelia burgdorferi was repeatedly isolated from skin and subcutaneous tissue biopsies in spi…
