To investigate the pathogenic mechanisms of eosinophilic pleural effusion in patients with paragonimiasis, we measured the levels of IL-5, granulocyte-macrophage colony-stimulating factor (GM-CSF) and interferon-gamma (IFN-gamma) in pleural effusions. A 57-year-old man with a history of having consumed fresh-water crab 8 months previously presented with a solitary mass lesion in the left lung without any symptoms. Since neoplastic disease could not be ruled out, an open biopsy was performed, and histological examination confirmed a definite diagnosis of paragonimiasis. A high level of the anti-Paragonimus westermani antibody was detected in the sera of the patient, but this level decreased substantially 3 months after chemotherapy with bithionol.

Paragonimus westermani is a lung fluke of humans that is usually found in the lungs but may be found elsewhere in many unusual locations. A case of pelvic paragonimiasis was found incidentally by surgical intervention of inflammatory disease and myoma uteri. She was a 51-year-old Korean woman complaining of lower abdominal pain and intermittent vaginal spotting. Numerous Paragonimus ova were observed in the resected omentum in the pelvis after total abdominal hysterectomy. It is suggested that pelvic paragonimiasis may be one of causative agents of pelvic inflammatory disease.

We describe four cases of pulmonary paragonimiasis in Southeast Asians who emigrated to the central San Joaquin Valley of California. Physicians should be alerted to the possibility of this disease in Asian patients with hemoptysis and pulmonary infiltrates. Paragonimiasis can masquerade as pulmonary tuberculosis, especially in patients from areas that are endemic for both the parasite and the tubercle bacillus. The ease and safety with which this infection can be treated, in contrast to tuberculosis, reiterates the importance of diagnosing this lung fluke when it is present.

Enzyme immunoassays (ELISA) and indirect haemagglutination (IHA) were used to determine IgG and IgE serum antibody levels in 43 patients from Ecuador before and after therapy with praziquantel. ELISA indicated infection more sensitively than IHA. The tendency of increasing antibody levels between the first and second month after therapy was more distinct when measured by IHA, whereas the decreases of antibody levels between the third and sixth month after treatment was more distinct when measured by ELISA. The IgG ELISA for detecting long persisting antibody was more sensitive than IHA.

In the children from Liberia and one woman from Guinea an infection with the lung fluke Paragonimus uterobilateralis was diagnosed, being the first cases of human paragonimiasis recorded in these two West African countries. Species determination was based on measurements and morphological structure of the parasite ova isolated from sputum and stool of the patients, and supported by biological and epidemiological evidence of the occurrence of this parasite species in natural animal hosts and P. uterobilateralis--metacercariae in freshwater crab populations in various parts of Liberia.

An evaluation of intradermal test in screening paragonimiasis was attempted by comparing antigens, filtered through Sephadex G 50, fractionated by DEAE Sephadex A 25, and V.B.S.(crude extract of adult worms) antigen on 402 school children. A Laotian man who had resided only in the north-central United States for 8 years sought care for an acute, progressive syndrome of severe dyspnea, chest pain, bilateral pneumothoraces, lung and liver nodules, and marked peripheral blood eosinophilia. He habitually ate raw crabmeat imported pickled or frozen from Southeast Asia; he denied eating local crustaceans. Ova consistent with the lung fluke Paragonimus westermani were identified in a bronchoalveolar lavage specimen, and the eosinophilia and pulmonary symptoms resolved with praziquantel therapy.

We present the CT findings of pulmonary paragonimiasis in a 9-year-old girl. It appeared as a large cystic mass with an air-fluid level and simulated a lung abscess. Communication with a segmental bronchus was demonstrated well on a CT scan obtained after aspiration.
