Splenic vein thrombosis and splenic infarction are complications beyond the usual clinical spectrum of paratyphoid fever, and the presentation is rarely described. We report the case of a young female, who presented with high-grade fever and severe left upper quadrant pain. Her blood culture was positive for Salmonella paratyphi A, with Widal test suggesting 4-fold rise in titers. Computed tomography revealed splenic vein thrombosis and multiple splenic infarcts, for which antibiotic and anticoagulation were instituted simultaneously. She had a complete resolution with this management, and ant…

A review of 194 cases of enteric infection in Scotland during 1967-74, is reported. Diagnosis was confirmed by the laboratory isolation of S … ging to different phage types. Two deaths occurred, one of which was the result of complications of paratyphoid fever. All other patients responded well to treatment, although two persons continued to excrete and became chronic carriers. A few examples are given of episodes of particular epidemiological interest. It is expected that in future years there will be a …

Rapid and accurate identification of Salmonella enterica serotypes Typhi and Paratyphi (A, B and C), the causal agents of enteric fever, is critical for timely treatment, case management and evaluation of health policies in low and middle-income countries where the disease still remains a serious public health problem. The present study describes the development of a multiplex assay (EFMAtyping) for simultaneous identification of pathogens causing typhoid and paratyphoid fever in a single reaction by the MeltArray approach, which could be finished within 2.5 h. Seven specific genes were chosen…

INTRODUCTION: Singapore is a transition country in Southeast Asia that is both vulnerable and receptive to the introduction and re-introduction of imported communicable diseases.
MATERIALS AND METHODS: For a 10-year period between 1998 and 2007 we studied the trend, epidemiological characteristics, proportion of imported versus local transmission of malaria, viral hepatitis (hepatitis A and E), enteric fevers (typhoid and paratyphoid), cholera, chikungunya and SARS.
RESULTS: Of a total of 4617 cases of the above selected diseases notified in Singapore, 3599 (78.0%) were imported. The majorit

The incidence of enteric (typhoid) fever in travelers is estimated to be approximately 3-30 cases per 100,000 travelers to developing countries. Recently, it is become clear that travelers who are visiting friends and relatives, especially travelers to the Indian subcontinent, seem to be the most vulnerable to enteric fever and require special attention for prevention. Recent concerns are the increasing incidence of paratyphoid fever in Asia, which is not covered by available typhoid vaccines, and the emergence of infections caused by antibiotic-resistant strains (including strains resistant t…

Of 10,000 sera examined for infectious diseases by both the rapid slide agglutination and the tube agglutination tests, 128 were found positive for typhus fever and 257 for typhoid and paratyphoid infections. Of 84 of the last mentioned cases of typhoid and paratyphoid, which were confirmed by cultural examination, 45 (53.5%) were negative to the tube test, whereas only one (1.2%) was negative to the slide test. Taking the total of 385 cases in which the clinical picture was typical and either one of the two serological tests positive, the tube test was negative in 183 cases (47.2%) whereas th

Salmonella enterica subspecies enterica serovar Paratyphi B [O1,4,(5),12 : Hb : 1,2] can cause either an enteric fever (paratyphoid fever) or self-limiting gastroenteritis in humans. The d-tartrate non-fermenting variant S. enterica subsp. enterica serovar Paratyphi B dT- (S. Paratyphi B) is the causative agent of paratyphoid fever, and the d-tartrate fermenting variant S. enterica subsp. enterica serovar Paratyphi B dT+ (S. Paratyphi B dT+; formerly called Salmonella Java) causes gastroenteritis. S. Java is currently recognized as an emerging problem worldwide. Twelve dT+ S. Java isolates wer…

We developed a novel rapid assay to detect the gyrA mutations that cause quinolone resistance in typhoid and paratyphoid fever Salmonella spp. using high-resolution melting (Idaho Technology, Salt Lake City, UT) analysis of polymerase chain reaction amplicons. The presence of gyrA mutations led to small but consistent changes in amplicon melting temperatures that allowed quinolone-resistant isolates to be differentiated from susceptible ones.

BACKGROUND: Enteric fever remains a serious global health challenge. Effective control of enteric fever depends on clean water and proper sanitation, which remains unattainable in resource-limited settings. Vaccination is therefore one of critical preventive measures. Nevertheless, there exists a paucity of systematic reviews integrating pathogenesis, the protective mechanisms, and translational vaccinology developments associated with enteric fever.
AIM OF REVIEW: To enhance the mechanistic understanding of enteric fever, this review summarizes the recent advances of pathogenesis, protective

Between 1977 and 1982, the National Center of Salmonella of the Institute Pasteur of Tunis has isolated; received and/or identified 1715 Salmonella strains. In typhoid and paratyphoid fevers group Salmonella typhi represent the predominant species with a frequency of 99.6%. In the enteric group, Salmonella wien is the most frequent (50,26%). 11 serotypes appeared in 1982, although 5 serotypes have not been met since 1977.

BACKGROUND: Rapid and reliable diagnostic assays for enteric (typhoid and paratyphoid) fever are urgently needed. We report the characterization of novel approach utilizing lymphocyte secretions, for diagnosing patients with enteric fever by the TPTest procedure.
METHODOLOGY: TPTest detects Salmonella-specific IgA responses in lymphocyte culture supernatant. We utilized TPTest in patients with suspected enteric fever, patients with other illnesses, and healthy controls. We also evaluated simplified modifications of TPTest for adaptation in laboratories with limited facilities and equipment.

The clinical experience with enteric fever (1985-89) at a general hospital in Kuwait is presented. There were 90 cases of typhoid and 10 cases of paratyphoid fever, with a mortality rate of 2%. Typhoid complications of bowel perforation, haemorrhage or septic shock were present in 5% and abortion in 2%. Neurological or psychiatric manifestations were a feature in 15%. Most infections were imported (78%). Despite rapid economic development, enteric fever may still be acquired locally but less frequently with time. This contributed to initial diagnostic uncertainty in 18% of cases, and especiall…
