Management challenges of a rare case of the primary parotid gland non-Hodgkin lymphoma: A case report and review of the literature.

Diffuse large B-cell lymphoma is the most prevalent subtype of non-Hodgkin lymphoma (NHL), which makes up the majority of malignant lymphoma … e circumstances. Herein, we describe a 60-year-old man who had a steadily growing mass in his right parotid region. The pathology results confirmed it to be NHL, diffuse large B-cell lymphoma subtype. No similar lesions were found on clinical examination and subsequent radiological imaging. The patient received R-CHOP chemotherapy with impressive treatment outco …

Potential use of acupuncture in the treatment of first bite syndrome.

OBJECTIVES: First bite syndrome (FBS) is an early postoperative pain syndrome characterized by the sudden onset of pain in the parotid region at the first bite of each meal. The etiology is not certain, hence a standardized therapy does not exist. METHODS: A 50-year old woman referred to us complaining of a swelling in the right parotid region. Fine-needle aspiration biopsy (FNAB) was diagnostic for pleomorphus adenoma of the deep lobe of the parotid gland. A 50-year old man presented with a mass in the right side of the neck, FNAB was diagnostic for parapharyngeal space neurinoma. The first …

Intraparotid Location of the Great Auricular Nerve: A New Anatomical Basis for Gustatory Sweating Syndrome.

BACKGROUND: Gustatory sweating syndrome (also known as Frey syndrome or auriculotemporal nerve syndrome) is thought to result from a lesion of the auriculotemporal nerve. A lesion of this nerve can lead to aberrant regeneration of nerve fibers to the sweat glands and blood vessels. The occurrence of signs outside the region of the auriculotemporal nerve prompted the author to search for another anatomical basis for this syndrome. METHODS: The author dissected 46 great auricular nerves from their origin to the parotid gland and in the infratemporal fossa. The author investigated the different

Developing an adaptive radiation therapy strategy for nasopharyngeal carcinoma.

Adaptive radiotherapy (ART) has recently been introduced to restore the planned dose distribution by accounting for the anatomic changes during treatment. By quantifying the anatomic changes in nasopharyngeal carcinoma (NPC) patients, this study aimed to establish an ART strategy for NPC cases. A total of 30 NPC patients treated with helical tomotherapy were recruited. In the pretreatment megavoltage CT images, the anatomic changes of the posterolateral wall of nasopharynx (P-NP), neck region and parotid glands were measured and assessed. One-way repeated measure ANOVA was employed to define t

The relationship of the parotid duct to the buccal and zygomatic branches of the facial nerve; an anatomical study with parameters of clinical interest.

There have been studies concerning the protection of the facial nerve during plastic surgery intended for the parotid gland. The close relationship between the parotid duct and the buccal and zygomatic branches of the facial nerve is studied here. The dissections were performed on 10 fixed cadavers at the Anatomy Dissection Laboratory of Ankara University in 2004. The reference points used for surgery of this region were taken into consideration as the landmarks for morphometric measurements. In 7 of the cases the zygomatic branch was double and in 3 it was single. In 4 of the 7 cases with a d

Multinodular neck recurrence of parotid gland pleomorphic adenoma: a case report.

BACKGROUND: Pleomorphic adenoma is the most common neoplasm of the parotid gland. It is a benign tumor composed of epithelial and myoepithelial cells arranged in various morphological patterns. The most common reasons contributing to a recurrent disease are obvious or underestimated tumor spillage, incomplete excision, and violation of the pseudocapsule of the tumor. CASE REPORT: This article presents a case of gross multinodular recurrence of a parotid gland pleomorphic adenoma in a 38-year-old female patient. Upon clinical examination of the homolateral neck, multiple, painless, well-define

Prominent mucoid degeneration of the parotid gland in a patient with systemic lupus erythematosus.

Lupus erythematosus (LE) can cause various cutaneous lesions including panniculitis (LE profundus), but salivary gland involvement has been extremely rare in patients with LE. Herein, we report the first documented case of systemic LE with prominent mucoid degeneration and lymphoplasmacytic infiltration in the parotid gland. A 38-year-old Japanese male with histories of autoimmune hemolytic anemia and systemic LE presented with a swelling of the bilateral cervical region. A physical examination revealed a swelling of the bilateral parotid gland and erythema of the right cheek. A biopsy specime

Castleman Disease of the Parotid Gland: A Report of a Case.

Castleman disease is an extremely rare benign lymphoproliferative disorder of unknown etiology. It affects the lymphatic chain in anybody region, although the commonest site is the mediastinum. The head and neck region is the second most common site; however, the salivary glands are rarely affected. We report a case of a 29-year-old Asian lady who presented with a 2-year history of an enlarging left parotid mass. Histopathology of the excisional biopsy confirmed the diagnosis of Castleman disease.

An uncommon case of fibrolipoma.

Lipoma is a common benign soft-tissue neoplasm derived from mature adipose tissue neoplasm, but its presence in the oral and pharyngeal region is relatively uncommon. Oral lipoma was first described by Roux in 1848 as "yellow epulis." It has an incidence rate of about 1%-4% of all benign oral lesions, with a prevalence rate of about 0.0002%. Fibrolipoma is an extremely rare subtype of lipoma which accounts for 1.6% of all facial lipomas. Specific anatomic locations of occurrence within the oral and maxillofacial region include the parotid region, buccal mucosa, lips, submandibular region, tong…

Sarcoma, ex pleomorphic adenoma of parotid gland: A rare entity.

Sarcomas arising in a parotid gland is an uncommon occurrence and a sarcomatous transformation in a recurring pleomorphic adenoma is even rare. Diagnosis of salivary gland tumors has proven to be challenging. Pleomorphic adenomas (PA) are the most commonly diagnosed tumors of the salivary gland and a malignant transformation in preexisting or recurring ones is even thinner on the ground. Malignant transformations in a pleomorphic adenoma can occur in carcinomas, carcinosarcomas, or sarcomas. We are sharing an interesting case of a 68-year-old male with a parotid region swelling with a history …

The Effect of Botulinum Toxin on an Iatrogenic Sialo-Cutaneous Fistula.

A sialo-cutaneous fistula is a communication between the skin and a salivary gland or duct discharging saliva. Trauma and iatrogenic complications are the most common causes of this condition. Treatments include aspiration, compression, and the administration of systemic anticholinergics; however, their effects are transient and unsatisfactory in most cases. We had a case of a patient who developed an iatrogenic sialo-cutaneous fistula after wide excision of squamous cell carcinoma in the parotid region that was not treated with conventional management, but instead completely resolved with the…

Epithelioid schwannoma of the facial nerve masquerading as pleomorphic adenoma: a case report.

Schwannomas arising in the parotid gland or peri-parotid region is frequently misdiagnosed as pleomorphic adenoma on cytologic preparations. The epithelioid variant of schwannoma is particularly susceptible to misdiagnosis because this neoplasm typically has epithelioid and spindled cells, which are associated with fibrillar stroma and mimic the epithelial, myoepithelial, and stromal components of a pleomorphic adenoma. Preoperative diagnosis of schwannoma is critical in order to plan appropriate management and to avoid inadvertent injury to the associated nerve during surgical resection. Thus…

Lipomatous hemangiopericytoma (adipocytic variant of solitary fibrous tumor) of the parotid gland: A case report and review of the literature.

The current study presents the first case of a lipomatous hemangiopericytoma (LHPC) developing in the parotid gland in a 33-year-old male. The patient presented with a 4-year history of a progressively growing painless and fixed mass in the left paratid gland region. The patient underwent radical parotidectomy and was followed-up for 12 months without any evidence of metastasis or recurrence. LHPC, a controversial rare HPC variant, is histologically characterized by a varying admixture of hemangiopericytomatous vasculature and the presence of mature adipocytes. To date, 51 cases of LHPC have b
