Novel calcium sensing receptor ligands: a patent survey.

INTRODUCTION: In the parathyroid gland, the calcium sensing receptor responds to small changes in circulating levels of Ca(2+), and consequently stimulates or inhibits the secretion of parathyroid hormone (PTH). Thus, ligands potentiating the action of calcium (calcimimetics) lead to decreased PTH secretion and can thus be useful for the treatment of hyperparathyroidism. On the other hand, ligands which antagonize the action of calcium (calcilytics) stimulate PTH secretion, favoring bone tissue regeneration. AREAS COVERED: This review first discusses the rapid development of calcimimetics (on

Attack on the scourge of tuberculosis: patented drug targets.

Tuberculosis is one of the most devastating bacterial diseases, with increasing rates of morbidity and mortality, despite the presence of effective chemotherapy and Bacillus-Calmette-Guerin (BCG) vaccine. The success of Mycobacterium tuberculosis lies in its ability to spread by aerosol droplets, evade the host immune system and to persist in pulmonary granulomas. The advancement in the field of molecular and cellular microbiology and the availability of transcriptome and proteome data of M. tuberculosis have aided in understanding the pathogenesis of this organism for developing more effectiv

Intellectual property rights, moral imagination, and access to life-enhancing drugs.

Although the idea of intellectual property (IP) rights--proprietary rights to what one invents, writes, paints, composes or creates--is firmly embedded in Western thinking, these rights are now being challenged across the globe in a number of areas. This paper will focus on one of those challenges: government-sanctioned copying of patented drugs without permission or license of the patent owner in the name of national security, in health emergencies, or life-threatening epidemics. After discussing standard rights-based and utilitarian arguments defending intellectual property we will present a

Nonsurgical closure of patent ductus arteriosus: clinical application of the Rashkind PDA Occluder System.

The first successful application of a transcatheter closure technique for patent ductus arteriosus (PDA) suitable for use in infants and children was performed by us in 1977. Since that time, there has been continued improvement and simplification of the equipment as well as in the implantation technique. Following a Food and Drug Administration protocol, a multicenter study was conducted to test the safety and effectiveness of this interventional method. The clinical results from three major regional test centers (Philadelphia, Houston, and New Haven) are presented. One hundred forty-six pati

A Systematic Review Exploring the Impact of Arteriovenous Fistula Ligature on High-Output Heart Failure in Renal Transplant Recipients.

BACKGROUND: Cardiovascular disease is the most common cause of death in renal transplant recipients (RTrs). High-output heart failure (HoHF) is a classic problem of RTrs with patent arteriovenous fistulae (AVF). Central to the entire discipline of transplant nephrology is the ligation of AVF in RTrs, with a patent AVF presenting with signs and symptoms of HoHF. AVF ligation has long been a topic of great interest in this population. To date, little attention has been paid to the effects of arteriovenous graft ligation on HoHF. This study systematically reviews the data for AVF ligation, aiming

Intestinal permeability as a clinical surrogate endpoint in the development of future Crohn's disease therapies.

Inflammatory bowel disease (IBD) including Ulcerative colitis (UC) and Crohn's disease (CD) is a major cause of gastrointestinal pathology in children and adolescents. Intestinal hyper-permeability plays a critical role in the etiology of pediatric CD by affecting the penetration of pathogens, toxic compounds and macromolecules; it also plays a central role in pharmaceutical product development. The mechanisms of barrier function and defects in permeability have great potential for guiding the development of novel drugs for treatment of IBD. Intestinal permeability is typically measured with t

Novel patent publications on high-affinity nicotinic acid receptor agonists.

BACKGROUND: Nicotinic acid (NA) has been used as a drug to treat dyslipidemia for > 50 years. In outcome clinical trials, NA displayed remarkable efficacy in patients with cardiovascular diseases by modifying lipid profiles that results in reduced morbidity and mortality. On the other hand, NA induces vasodilation (flushing) that undermines treatment compliance. In addition, high-dose treatment is required presumably owing to the poor pharmacokinetic properties of NA. The identification of the high-affinity NA receptor, namely G-protein coupled receptor 109A (GPR109A), led to further understan

Active treatment vs. expectant management of patent ductus arteriosus in preterm infants: critical analysis of a meta-analysis.

A recent meta-analysis on the active versus expectant management of patent ductus arteriosus in preterm infants, including 10 randomized control trials involving 2035 infants, found that the incidence of death at 36 weeks or moderate to severe bronchopulmonary dysplasia (BPD) and of death was higher in the active treatment group. The accompanying editorial suggested that “the use of early routine pharmacologic treatment no longer appears to be justifiable”. However, this meta-analysis is based on highly heterogeneous studies due to different diagnostic criteria used to define hemodynamically s

A patent perspective of RSK inhibitors to treat cancer (2020-present).

INTRODUCTION: The Ser/Thr protein kinase family, p90 ribosomal S6 kinase (RSK), can be activated by inputs from ERK1/2, PDK1 and in some cases MK1/MK2. Dysregulated RSK activity is implicated in diseases associated with inflammation such as cancer. There are four RSK family members of which RSK1/2 are tumor promoters whereas RSK3/4 can act as tumor suppressors. AREAS COVERED: The review summarizes patents that have identified compounds that decrease RSK activity. These patents all focus on the use of RSK inhibitors as cancer therapeutics. The patents were identified using the World Intellectu
