Expression of NET markers in experimental apical periodontitis induced in mice.

OBJECTIVE: This study aimed to evaluate the expression of neutrophil extracellular trap (NET) markers in experimental apical periodontitis (AP) induced in mice, focusing on protein arginine deiminase 4 (PAD4), cathepsin G (CatG), myeloperoxidase (MPO), and citrullinated histone 3 (CitH3). DESIGN: AP was induced in 55 wild-type mice, randomly divided into a negative control (n = 5) and five experimental groups (n = 10) analyzed at 2, 5, 7, 21, and 42 days after pulp exposure of first lower molars to the oral environment. Histopathological and histomorphometric analyses included neutrophil coun

Incidence of mandibular nutrient canals in hypertensive patients: a radiographic study.

Hypertension, also called a 'silent killer,' is one of the most common medical problems seen in our profession. A prospective study was conducted in the Department of Oral Medicine and Radiology to determine the incidence of the appearance of nutrient canals in the periapical radiographs of the mandibular anterior region of patients with high blood pressure. A total of 100 patients, between 10-80 years, were examined. After taking a proper history, systemic and oral examinations were done and the findings were recorded under two categories, hypertensive patients and normotensive patients. They

Apical surgery may cause gingival recession.

SUBJECTS: Two hundred forty-two subjects were treated with apical surgery from 2000 to 2004. All subjects were operated on by the same clinician. One hundred eighty-six subjects were later reevaluated. The study consisted of 102 female and 84 male patients. The mean age of the patients was 49.7 years (range, 9-81 years). Most of the 56 cases lost to follow-up did not want to participate at the follow-up exam (n = 23) or the treated tooth was extracted before the follow-up exam (n = 15). KEY EXPOSURE/STUDY FACTOR: Key study factors included gender, age, smoking status, treated tooth type, rest

Treating apical fenestration in a previously endodontically treated tooth.

Apical fenestration is a defect in the alveolar cortical plate, exposing the root without involving the alveolar bone margin, often linked to trauma, periodontal disease, and orthodontic treatment, leading to symptoms such as pain and abscesses from endodontic infections. This case report describes managing a mucosal fenestration in an endodontically treated tooth with nonsurgical root canal therapy and periodontal surgery. A 44-year-old male presented with mucosal fenestration and pain in the upper front jaw due to trauma and an inadequately treated root canal. Examination revealed an ill-fit

Bilateral radicular cyst in mandible: an unusual case report.

A radicular cyst is one of the furthermost everyday odontogenic cysts of the anterior maxilla, not regularly comprehended in youth. They are … . Many times it is perplexing to segregate radicular cysts from the obligatory pre-existing chronic periapical periodontitis lesions radiographically. Here, we present a rare case with bilateral radicular cyst in relation to first molar of the mandible in a 19-year-old. Orthopantomograph showed a large unilocular radiolucency with a well-defined border in the periapical r …

Periapical status and quality of root fillings and coronal restorations in a Danish population.

AIM: The aim of this study was to investigate the quality of endodontic and coronal restorations and the association with periapical status in a Danish population. METHODOLOGY: A total of 614 randomly selected individuals (20-60+ years of age) from Aarhus County had a full-mouth radiographic examination. The quality of endodontic and coronal restorations and the periapical status of endodontically treated teeth were assessed by radiographic criteria. Root fillings were categorized as 'adequate' or 'inadequate' with regard to root filling length and lateral seal. Coronal restorations were cate

Metabolic syndrome and its effect on immune cells in apical periodontitis- a narrative review.

OBJECTIVES: Apical periodontitis (AP) is an inflammatory immune response in periapical tissues caused by microbial infections. Failure of root canal treatment or delayed healing is often due to intracanal or extra-radicular bacteria. However, beyond microbial factors, the patient's systemic health can significantly influence the progression and healing of AP. Metabolic syndrome is a risk factor and it is characterized by a cluster of interconnected metabolic risk factors, including abdominal obesity, hyperlipidemia, hypertension, and hyperglycemia. MATERIALS AND METHODS: A comprehensive liter

Necroptosis in apical periodontitis: A programmed cell death with multiple roles.

Programmed cell death (PCD) has been a research focus for decades and different mechanisms of cell death, such as necroptosis, pyroptosis, ferroptosis, and cuproptosis have been discovered. Necroptosis, a form of inflammatory PCD, has gained increasing attention in recent years due to its critical role in disease progression and development. Unlike apoptosis, which is mediated by caspases and characterized by cell shrinkage and membrane blebbing, necroptosis is mediated by mixed lineage kinase domain-like protein (MLKL) and characterized by cell enlargement and plasma membrane rupture. Necropt

Nociceptor-Macrophage Interactions in Apical Periodontitis: How Biomolecules Link Inflammation with Pain.

Periradicular tissues have a rich supply of peripheral afferent neurons, also known as nociceptive neurons, originating from the trigeminal nerve. While their primary function is to relay pain signals to the brain, these are known to be involved in modulating innate and adaptive immunity by initiating neurogenic inflammation (NI). Studies have investigated neuroanatomy and measured the levels of biomolecules such as cytokines and neuropeptides in human saliva, gingival crevicular fluid, or blood/serum samples in apical periodontitis (AP) to validate the possible role of trigeminal nociceptors

SEM Study of apical morphological alterations in primary teeth with vital and necrotic pulps.

This study evaluated, by SEM, the morphology of human primary teeth roots. Twenty-four teeth were divided into 3 groups: pulp vitality (group I) and pulp necrosis without (group II) and with apical periodontitis (group III). Roots were analyzed by the presence of periodontal ligament (PDL) fibers and resorption areas. In groups I and II, presence of PDL fibers and absence of resorption were observed in all cases (100%), while all specimens (100%) of group III showed no PDL fibers and resorption areas. In conclusion, there are morphological differences in the apical region of primary teeth with

Periapical tissue reactions after pulp exposure in rat molars.

Apical periodontal lesions were produced in the rat molar by exposing the pulp to the oral environment. The natural history of the undisturbed lesion was studied at time intervals ranging from 2 days to 1 year postoperatively. The earliest reactions were usually inflammatory infiltration at the apex. The severity of the lesions was related to the amount of vital pulp still present in the tooth rather than to the time lapse after exposure. After 1 month most pulps has undergone necrosis and the lesions about the apices which followed showed two main types of reaction. In the case of the suppura

Apical barrier formation after incomplete orthograde MTA apical plug placement in teeth with open apex--report of two cases.

Two cases are reported in which incomplete placement of 4 mm mineral trioxide aggregate (MTA) plug was performed unintentionally at the apical third of permanent immature teeth with open apex and apical periodontitis. As confirmed radiographically, there were gaps between MTA and dentinal walls along the MTA-dentin interface. After setting of MTA was confirmed, endodontic treatment was completed and access was sealed with composite resin. At 6 to 16 months follow-up examinations, formation of dentin in contact with the MTA surface, as well as apical closure and periapical healing were ideied r

[Clinical and bacteriological studies with metronidazol in endodontic therapy (author's transl)].

Metronidazol was employed in the endodontic treatment of 200 gangrenous teeth, with symptoms of acute and chronic periodontitis. In 50 cases aerobic and anaerobic microbiological cultivations have been performed. The number of the necessary treatments was less, symptoms of irritation in the periapical region could not be observed. The favourable clinical and microbiological results in the course of the Metronidazol treatment point to the role of the anaerobic microorganisms in connection with the inflammatory symptoms of the gangrenous pulp.

Genetic polymorphism of interleukins 6 and 17 correlated with apical periodontitis: A Cross-sectional study.

Interleukins 6 and 17 act in bone resorption in the presence of infections of endodontic origin for host defense. Genetic polymorphisms may be associated with increased bone loss, represented by areas of large periapical lesions. This study aimed to verify the frequency of interleukin 6 and 17 gene polymorphism in patients with asymptomatic apical periodontitis or chronic apical abscess and to verify the existence of correlations between periapical lesion area with age, gender, and presence of the polymorphism, in the studied population, in the state of Pernambuco. A population consisting of t

Successful Use of MTA Fillapex as a Sealant for Feline Root Canal Therapy of 50 Canines in 37 Cats.

Intentional or unintentional pulp exposure of cat canines can lead to periapical disease, osteomyelitis, and oral pain. Root canal therapy (RCT) allows the retention of cat canines with pulp exposure by removing the infected pulp and replacing it with an inert material. This study used MTA Fillapex™ as a root canal sealant with gutta percha single cone obturation in 37 cats (50 canine teeth). Roots were classified as "successful," "no evidence of failure (NEF)," or "Ошибка" at 6-month radiographic reviews. Therapy was considered "successful" if a preoperative periapical lucency had healed or n

Multiple Apical Radiolucencies and External Cervical Resorption Associated with Varicella Zoster Virus: A Case Report.

Varicella zoster virus (VZV) is responsible for the primary infection chickenpox. After the initial infection, it remains latent but can reactivate, resulting in shingles (herpes zoster). Previous reports have implicated VZV in the pathogenesis of apical periodontitis, but the involvement of the virus has not been investigated fully. The present case describes a patient who suffered from a severe episode of shingles and subsequently developed periapical radiolucencies of all the teeth in the affected nerve distribution. Molecular and culture techniques showed the presence of VZV DNA in the roo
