Intratympanic dexamethasone microcrystal-loaded porous PLGA microspheres containing lipoic acid for combined delivery to the inner ear.

OBJECTIVE: To develop a sustained-release codelivery system for intratympanic administration of dexamethasone (DEX) and lipoic acid (LA). M … nabled simultaneous release of both drugs, with DEX exhibiting superior pharmacokinetics (sustained perilymph concentrations up to 7 days) compared to DEX MCs alone. LA release from LPMPs demonstrated prolonged kinetics without burst release. SEM confirmed DEX MCs were localized within/on LPMPs and adhered to the round window membrane (RWM). Histological an …

Case report: Concurrent intravestibular schwannoma mimicking Ménière's disease and cochlear hydrops detected via delayed three-dimensional fluid-attenuated inversion recovery magnetic resonance imaging.

OBJECTIVE: To present a case of intralabyrinthine schwannoma (ILS) presenting as Ménière's disease diagnosed via 4-h delayed gadolinium-enha … l void in the scala media of the cochlea indicative of cochlear hydrops, and a strong signal in the perilymph at the basal cochlea suggestive of impaired blood-labyrinthine barrier. CONCLUSION: Delayed 3D-FLAIR MRI is useful in diagnosing concurrent ILSs and endolymphatic hydrops.

Progression of endolymphatic hydrops and vertigo during treatment in Meniere's disease.

BACKGROUND: Patients with Meniere's disease (MD) receive treatment to reduce vertigo. PURPOSE: To explore the fluctuation of vertigo symptoms and the changes in endolymphatic hydrops (EHs) in MD patients during long-term regular medication. MATERIALS AND METHODS: We enrolled MD patients who had received two magnetic resonance imaging with intravenous contrast agents. RESULTS: Of the 42 patients in the study, 18(42.9%) had progressive EHs and 3(7.1%) had remission. The change value of the signal intensity ratio (SIR; cochlear perilymph/cerebellum) on the affected side (1.2) was larger than t…

Effect of superoxide dismutase and allopurinol on impulse noise-exposed guinea pigs--electrophysiological and biochemical study.

OBJECTIVE: To investigate the protective effect of two anti-reactive oxygen species (ROS) substances, copper-zinc superoxide dismutase (CuZn … of malondialdhyde, evaluated by measuring the content of thiobarbituric acid reactive substances in perilymph samples. RESULTS: Acoustic stress induced ROS formation and both allopurinol and CuZn-SOD exerted a protective effect on the cochlea. Comparison of compound action potential thresholds in different animal groups showed that the temporary threshold …

[Traumatic perilymphatic fistula of round and oval windows (four cases reported)].

OBJECTIVE: To explore the diagnosis and treatment to traumatic perilymph fistula (PLF) of round and oval window. METHOD: Traumatic PLF was diagnosed by the traumatic history of head, neck and ear, the examinations of auditory and vestibular function, and the exploratory tympanotomy. The PLF of round and oval windows were repaired by fascia graft or tragus perichondrium and gelform. Four cases with traumatic PLF of round and oval window were reported in this paper, which included 2 cases hitten by hand, 1 by brick, 1 insulted by middle ear surgery. Exploratory tympanotomy was performed from on…

Electrocochleography in Retrosigmoid Vestibular Nerve Section for Intractable Vertigo Caused by Meniere's Disease.

Interest in electrocochleography has increased in recent years because of the discovery of an elevated summating potential to action potential amplitude ratio (SP/AP ratio) in patients with endolymphatic hydrops caused by Meniere's disease or perilymph fistula. It was the purpose of this investigation to determine whether the intraoperative SP/AP ratio will decrease after vestibular nerve section in patients with intractable Meniere's disease. Fourteen patients with medically intractable classic Meniere's disease underwent retrosigmoid vestibular nerve section. Intraoperative transtympanic ele…

Vestibular neuritis.

Because the cause of vestibular neuritis is unknown, this disorder is best understood as a constellation of characteristic clinical symptoms and signs. The differential diagnosis includes peripheral otologic disorders, including Meniere's disease, perilymph fistula, and vestibular atelectasis; and central disorders, including vascular insufficiency, acoustic neuroma, migraine equivalent, paraneoplastic syndrome, metastatic carcinoma, and multiple sclerosis. The site of lesion, as evaluated by clinical signs and symptoms, is controversial because concomitant central nervous system abnormalities…

Biodistribution and transport of intratympanically administered drugs in the inner ear using a porcine ex-vivo chamber model.

Effective pharmacotherapy for hearing loss remains a significant challenge because of the blood-labyrinth barrier, which limits systemic dru … as a third drug-entry route in young pigs. Surgical sealing of this pathway in vivo, which reduces perilymph drug levels, underscores its importance in human translational studies. In summary, we developed a robust ex vivo multi-barrier model integrated with FluidSim to predict the inner ear drug biodistribution.

Prevalence of occult cochlear basal turn patency.

BACKGROUND: Perilymph gusher (PLG) is a rare complication of otologic surgery attributed to a communication between the cochlea and the internal auditory canal (IAC). Subtle patency between the cochlear basal turn and IAC has recently been identified on computed tomography (CT) as a risk factor, specifically when the defect is > 0.75 mm. OBJECTIVES: Investigate the prevalence of radiographic cochlear basal turn patency. MATERIALS AND METHODS: Patients with CT of the temporal bones and inner ears interpreted as "normal" were included. An otologist and a radiologist independently reviewed CTs …

Hence, our findings indicate that the reticular lamina forming the endolymph-perilymph barrier at the apical side of the organ of Corti is involved in a fast intrinsic otoprotective mechanism of the cochlea.

Highly conserved influenza A sequences as T cell epitopes-based vaccine targets to address the viral variability.

An in vivo method for assessment of changes in hydroxyl radical levels in cochlear perilymphatic spaces is described and applied to cochlear ischemia-reperfusion in the mouse. Cochlear blood flow was reversibly reduced by compression of the anterior inferior cerebellar arterial network. Changes in the production of hydroxyl radicals, used as an index of tissue production of reactive oxygen species (ROS), were determined by measuring the conversion of salicylate to 2,3-dihydroxybenzoic acid. Low levels of salicylate (0.1 mM) in artificial perilymph were applied by perfusion of the cochlea using…

The relationship between intracranial pressure and tympanic membrane displacement.

Transmission of intracranial pressure (ICP) to the perilymph of the cochlea may occur via the cochlear aqueduct and possibly other routes. Indirect measurement of perilymphatic pressure may be investigated by observing tympanic membrane (TM) displacement during stapedial reflex contraction. In a previous study we investigated the effects of changes in ICP on perilymphatic fluid pressure in three patients who underwent ventriculo/lumbar-peritoneal shunt operations. The TM displacement technique proved extremely sensitive and revealed marked changes in cochlear fluid pressure brought about by ch…
