The morphology of the lymph node in the macroglobulinaemia of Waldenström.

In 16 cases of Waldenström's macroglobulinaemia the following findings were sufficiently frequent to justify a provisional diagnosis: a modestly enlarged node with lymphocytic infiltration through the capsule into the adjacent connective tissue, retention of the sinus and medullary reticulin pattern, few or no peripheral follicles, sinuses marked out by pale histiocytes, scanty mitoses, numerous plasma cells, and dark staining of plasma in blood vessels. Mast cells were not more frequent than in reactive nodes, but were more frequent than in lymphomas. Periodic acid-Schiff-positive inclusions …

Adult Kerion Caused by Trichophyton tonsurans.

Adult tinea capitis, especially kerion, caused by Trichophyton tonsurans is relatively rare in China. Here, we report a case caused by the agent in an old woman with normal immune function. Fungal microscopic examination and culture were positive. The isolate was further identified as Trichophyton tonsurans by internal transcribed spacer (ITS) sequencing. Skin biopsy and histopathological examination suggested diffuse inflammatory cell infiltration in the dermis and subcutaneous tissue. Notably, fungal spores were observed within the dermis by Periodic acid-Schiff (PAS) staining. After eight w…

Diagnostic yield of stains for infectious organisms in esophageal or gastroesophageal junction biopsies with esophagitis.

Stains frequently performed to exclude infectious etiologies in esophagitis include Grocott methenamine silver (GMS) and periodic acid-Schiff (PAS) as well as immunohistochemistry (IHC) assays for cytomegalovirus (CMV) and herpes simplex virus (HSV). The diagnostic yield of these tests, in this situation, has not been well studied. We retrospectively reviewed 261 esophageal biopsies, which had one or more of the above tests performed. The diagnostic yield for GMS and PAS was 8%, while CMV and HSV immunohistochemistry had a diagnostic yield of 1% and 0%, respectively. Our study suggests that ro…

A Case of Intramuscular Cryptococcosis Mimicking a Malignant Tumor on FDG PET/CT.

Intramuscular cryptococcosis is extremely rare. In this case, a 76-year-old man with history of acute myeloid leukemia presented a painful mass in the right upper arm. FDG PET/CT demonstrated a low-density irregular-shaped mass in the triceps muscle of the right upper arm with adjacent bone erosion. The lesion presented with significantly increased activity with an SUVmax of 23.4, which could be misdiagnosed as a malignant tumor. The lesion was confirmed as cryptococcosis by periodic acid-Schiff stain and culture of biopsied specimen. Because cryptococcal infection is treatable, it is importan…

A Lacrimal Gland Choristoma of the Lacrimal Sac.

Choristomatous lacrimal gland tissue has been detected in many different sites of the ocular adnexa, but has never before been convincingly described in the submucosa of the lacrimal sac. A 77-year-old woman with epiphora had a biopsy of the sac wall preformed during a dacryocystorhinostomy that contained such a lacrimal choristoma. Zymogen granules were found in the cytoplasm of the secretory cells with the periodic acid-Schiff reaction. No mucus-producing cells, as found in normal sac submucosal glands, were detected using the Alcian blue, mucicarmine, and Gomori methenamine silver histochem…

Subungual Amelanotic Melanoma Masquerading as Onychomycosis.

Subungual amelanotic melanoma is rare. In addition, amelanotic melanoma can mimic non-melanocytic tumors. A 67-year-old woman had a four-year history of dystrophy of the left fourth fingernail. Periodic acid-Schiff staining of the nail plate demonstrated fungal hyphae, establishing a diagnosis of tinea unguium. The nail plate subsequently detached and the underlying nail bed showed a red, friable mass that was biopsied and confirmed a diagnosis of melanoma. In conclusion, additional morphologic change of a persistent nail dystrophy-even with a biopsy-confirmed diagnosis of onychomycosis-may re…

Whipple's Disease Manifested as Recurrent Ascites.

Whipple's disease commonly presents as chronic diarrhea and abdominal pain. Ascites is an uncommon presentation of Whipple's disease. Here, we report a rare case of a 47-year-old male who presented with diarrhea and abdominal distention for three months. The physical examination was significant for ascites. Serum albumin was low and serum-to-ascites albumin gradient was < 1.1 g/dl. This suggested that ascites was less likely to be present due to portal hypertension. Enteroscopy showed erythematous duodenum and jejunum; biopsy was suggestive of periodic acid-Schiff stain (PAS) positive macropha…

Granulomatous colitis: more than a canine disease? A case of Escherichia coli-associated granulomatous colitis in an adult cat.

CASE SUMMARY: This report describes a 4-year-old cat with chronic intermittent haematochezia and faecal incontinence of 7 months' duration. Investigation revealed severe colonic multifocal mucosal ulcerations and infiltration of the mucosal lamina propria by large numbers of periodic acid-Schiff-positive macrophages. Fluorescence in situ hybridisation analysis of colonic biopsies revealed multifocal clusters of intracellular Escherichia coli. Treatment with fluoroquinolones for 6 weeks led to a complete resolution of clinical signs. RELEVANCE AND NOVEL INFORMATION: The findings reveal that mu…

Nearly Fatal Case of Whipple's Disease in a Patient Mistakenly on Anti-TNF Therapy.

Whipple's disease is a rare cause of chronic diarrhea and abdominal pain that may be confused with inflammatory bowel disease. We report a Whipple's case misdiagnosed as Crohn's disease in which treatment with anti-tumor necrosis factor (anti-TNF) therapy led to nearly fatal progression. Lymph node tissue obtained during laparotomy for suspected bowel necrosis stained dramatically with periodic acid-Schiff (PAS), and electron microscopy showed a bacterium consistent with Trophyrema whipplei. The patient made a remarkable recovery complicated only by cholestatic hepatitis, which was likely a tr…

Treatment of mild, moderate, and severe onychomycosis using 870- and 930-nm light exposure: some follow-up observations at 270 days.

We have previously reported the results of a clinical trial in which the Noveon laser was used to treat onychomycosis. In the 180-day follow-up therein it was noted that positive clinical impact was demonstrable by a clearly measurable decrease in positive fungal culture and a concomitant decrease in positive microscopy with periodic acid-Schiff-stained nail scrapings. Review of 270-day mycological data, which are now available, confirmed that there was further decrease in both measures. Indeed, 38% of the treated population had negative culture and microscopy, qualifying as "mycological cures…

Onychomycosis.

Onychomycosis is a frequent nail disease caused by dermatophytes, yeasts, and nondermatophyte molds. Trichophyton rubrum, T mentagrophytes, and Epidermophyton floccosum are the most common etiologic agents worldwide. Candida spp are the most frequent among the yeasts. Diagnosis is corroborated by direct microscopic examination, culture, and histomycology with periodic acid-Schiff stain. Other new methods of diagnosis are discussed. Treatment is based on oral antifungals: terbinafine, itraconazole, and fluconazole, including other emerging triazole drugs. Therapeutic outcome with ciclopirox and…

Whipple disease: intestinal infiltrating cells exhibit a transcriptional pattern of M2/alternatively activated macrophages.

Whipple disease (WD) is a rare systemic disease caused by Tropheryma whipplei and is characterized by the presence of foamy macrophages with periodic acid-Schiff-positive inclusions in tissues such as lamina propria. For the first time, we report the gene-expression profile of macrophages in intestinal lesions from a patient with WD. Microarray and real-time polymerase chain reaction revealed that genes encoding CCL18, cathepsins, scavenger receptor, interleukin-10, and lipid metabolites were up-regulated in intestinal lesions. This transcriptional pattern corresponds to that of M2/alternative…

Acinic cell adenocarcinoma of the parotid gland: case-report.

We report a case of acinic cell adenocarcinoma of the parotid in a 18-year-old young man. Clinical examination showed a firm mobile mass of the gland. Light microscopy revealed the tumor composed of two distinct cell populations: a main one with large, polyhedrical or round cells with eosinophilic cytoplasm and a smaller proportion of granular cytoplasm clear cells. These tumor cells were periodic acid Schiff reaction (PAS) and mucicarmine staining negative. Immunohistochemically tumor main cells exhibited diffuse positive reactivity for cytokeratin and focal for S-100 protein. Both reactions …

Ocular manifestations of Whipple disease: an atypical presentation.

A 62-year-old man developed bilateral granulomatous iridocyclitis after uncomplicated cataract surgery. On ophthalmic examination, we found moderate inflammation in the anterior chamber and vitreous, with granular crystalline deposits on the iris, intraocular lens, and capsular bag. Biopsy of the lens capsule and vitreous revealed periodic acid-Schiff-positive, diastase-resistant bacilli consistent with Tropheryma whippelii. Electron microscopy and polymerase chain reaction confirmed the diagnosis of Whipple disease. A jejunal biopsy specimen also revealed T whippelii. Treatment with trimethop…

Lymphedema, lymphocytic myocarditis, and sarcoidlike granulomatosis. Manifestations of Whipple's disease.

A patient with Whipple's disease presented with a long prodromal period characterized by granulomatous lymphadenitis and progressive lymphedema of the extremities. No gastrointestinal tract symptoms were present and a small bowel biopsy sample was normal. His clinical condition deteriorated with the onset of lymphocytic myocarditis. At autopsy, intestinal involvement with macrophages that stained positively with periodic acid-Schiff was limited primarily to the submucosa. Diffuse fibrous effacement of lymph nodes with afferent lymphangiectasia seemed to be the mechanism of diffuse lymphedema, …

Photo-downregulation of SIRT4 mitigates aging in mice by enhancing H3K9ac via fatty acid metabolism.

As organisms age, mitochondrial metabolic activity declines, and disrupted gene expression regulation mediated by histone acetylation induces the emergence of senescent physiological phenotypes in tissues. In this study, we found that periodic exposure to red light significantly increased histone H3 Lys9 acetylation (H3K9ac) levels in the tissues and organs of aged mice. Following red light exposure, silent information regulation factor 4 (SIRT4) protein levels in keratinocytes were notably reduced, whereas glycolysis, fatty acid metabolism, and the tricarboxylic acid (TCA) cycle were signific

Features of the aperiodic microfibrils associated with mouse dental basement membrane demonstrated by ultrastructural histochemistry.

Histochemical features of aperiodic microfibrils (AMF) in mouse tooth germs were examined at the electron microscopic level. Intact and EDTA-isolated materials obtained from one day old first molars were used for ruthenium red (RR) staining, ferritin permeability, periodic acid-silver methenamine (PAM) impregnation, fibronectin localisation, negative staining on cryo-sections and tannic acid fixation. Electron microscopy and negative staining demonstrated that AMF traverse the basal lamina and penetrate below the inner enamel epithelium. In addition to RR staining, PAM impregnation and tannic …
