Early treatment of orbital floor fractures with catheter balloon in children.

Fifteen pediatric patients with orbital floor fractures were successfully treated early, following injury by a transantrally introduced catheter-balloon technique. The latter stabilizes and supports the torn, but still vascularized periosteum at the edges of the fracture and also serves as a guide for its further proliferation and new bone formation. Apart from a transient anesthesia in the dermatome of the infraorbital nerve in certain patients no complication or sequela was noted in this series. Accordingly early repair with the use of catheter-balloon appears to be a viable alternative in t…

[Treatment of recurrent anterior dislocations of the shoulder by the technic of pre-glenoid stapling. Study of a series of 48 cases].

Preservation of local muscular structures, and closure of the capsulo-periosteum detachment, are the two essential principles observed, when treating recurrent anterior shoulder dislocation by pre-glenoid stapling and also afford satisfactory results. This technique is used for capsulo-ligamentar lesions without alterations of the anterior edge of the glenoid cavity. If the strict technique is followed when placing the staple excellent stability result. If physiotherapy is started early, functional recovery is rapid. Sporting activities can be resumed within the third month after the operation…

Chronic subglottic stenosis.

Severe subglottic stenosis or stenosis that persists after a number of repairs have failed can be treated by an operation to widen the entire cricoid. This is accomplished by splitting the cricoid arch anteriorly and the posterior cricoid lamina posteriorly, both in the vertical midline. These vertical splits are held apart by using thyroid laminae cartilage grafts with periosteum intact on both sides. The posterior cricoid lamina split is covered by using muscle fascia or buccal mucous membrane. The subglottis is stented with a silicone conforming laryngeal stent or a silicone tracheal T tube…

Thanatophoric dysplasia of identical twins.

The autopsy cases of thanatophoric dysplasia of identical female twins were reported. These babies showed short limbs, long and narrow thoraxes, and depressed nasal roots. Disorganization of endochondral ossification and hypertrophic periosteum were recognized histologically at the epiphyseal plate of femur. The cause of death was respiratory distress caused by hypoplastic and atelectatic lungs. Their mother had no complaints except hydramnios. Female identical twins of this disease are very rare. It could be considered that the disease would be induced by some genetic disorders occurring at t…

Bone formation with free periosteal grafts in reconstruction of congenital maxillary clefts.

In a series of 23 patients free grafts of tibial periosteum have been used in the reconstrlction of 26 congenital maxillary clefts. In 23 (88.5%) of these clefts, rapid bone formation occurred at the transplantation site overbridging the bone defect in 20 (77%) of the clefts. The permanent value of these bone bridges has not yet been solved. Nevertheless, this operation has demonstrated the strong osteogenic capacity of free periosteal grafts suggesting their use also in other fields, where bony fusion or reconstruction of other defects is needed.

Invasive desmoplastic fibroblastoma in the chest wall.

Desmoplastic fibroblastoma is a rare soft tissue tumor. We present the case of a 61-year-old man with a growing chest wall tumor in the pleural cavity. After resection, the tumor was diagnosed as desmoplastic fibroblastoma. This is the first report of desmoplastic fibroblastoma occurring in the chest wall. Generally, desmoplastic fibroblastoma is considered as a benign and slow-growing tumor. However, the tumor in our patient grew very rapidly and invaded the adjacent lung, parietal pleura, and periosteum of the rib bone. We therefore describe this tumor as an "invasive" desmoplastic fibroblas…

Touraine-Solente-Gole syndrome: The complete form needs to be recognized.

The primary hypertrophic osteoarthropathy (HOA) (pachydermoperiostosis) is a rare hereditary disease characterized by skin manifestations (pachydermia), digital clubbing and proliferation of periosteum (periostitis) with sub-periosteal new bone formation. We describe here a case who presented with seborrheic dermatitis and joint pain of both hands on work up matched satisfactorily with the complete form of primary HOA. The identification of secondary form being relatively common among clinicians and hence the importance of recognizing the primary form, which is a difficult to treat condition i…

Periosteal pedicle graft for the treatment of gingival recession defects: a novel technique.

BACKGROUND: The periosteum is a highly cellular connective tissue with rich vascularity and regenerative potential. These qualities make it a suitable autogenous graft. A technique utilizing autogenous periosteal pedicle graft (PPG) for the treatment of gingival recession defects is described. METHODS: Four patients with Miller Class I and II recessions > or = 3 mm were treated utilizing PPG. RESULTS: At the end of one year, all defects were completely covered. CONCLUSIONS: The PPG technique can be used for the treatment of gingival recession defects successfully.

Usefulness of the galea flap in treatment of extensive frontal bone defects: a study of 14 patients.

We present a technique for treating extensive frontal bone loss after chronic osteitis and subsequent treatment. Unlike the technique usually performed in such cases (exclusion and filling), the frontal sinuses were conserved by isolating them from the reconstructed frontal vault. Isolation was achieved by means of a temporal galea and periosteum flap with a pedicle arising from the superficial temporal vessels. The study concerned 14 patients operated on from two to seven times for osteitis, which persisted in many patients for more than 10 years.

Remote periosteal distraction promotes ischemic diabetic foot wound repair associated with VEGF/VEGFR2-related angiogenic responses.

Bone transport-induced remote microinjury may promote diabetic foot ulcer healing but requires osteotomy and may cause fracture or osteomyelitis. This study evaluated whether osteotomy-free remote periosteal distraction (RPD) could promote repair in a rabbit ischemic diabetic foot wound model. Seventy-two diabetic male rabbits were randomized into RPD, Sham, and Control groups, and ischemic foot wounds were created. In the RPD group, a custom-made plate was implanted beneath the tibial periosteum and connected to an external distractor. The periosteum was distracted outward for 7 days and retu…

Regeneration of Chronic Alveolar Vertical Defects Using a Micro Dosage of rhBMP-2. An Experimental In Vivo Study.

OBJECTIVE: The objective of this study is to compare the effect of the location of recombinant human bone morphogenetic protein 2 (rhBMP-2) from the native bone and the periosteum for vertical alveolar bone augmentation. MATERIALS AND METHODS: Mandibular, chronic, standardized, bilateral, and vertical defects in 12 beagle dogs were evaluated using four modalities: a xenograft alone (XENO; n = 6); rhBMP-2 alone (BMP; n = 6); a technique with rhBMP-2 close to the host bone covered by xenograft (SAN; n = 6); and a technique with rhBMP-2 close to the flap on top of the xenograft (LAS; n = 6). Repositioning the Posterior Septal Angle in Rhinoplasty: Methods and Outcomes.

OBJECTIVE: Repositioning and fixation of the posterior septal angle (PSA) relative to the anterior nasal spine (ANS) is a well-known maneuver performed during rhinoplasty. Suture techniques through the periosteum along with transosseous drilling through the spine are the two most common fixation methods. We report on how nasal airway patency varies as a function of technique and patient demographic factors. METHODS: A retrospective analysis was performed on patients who underwent PSA repositioning and stabilization during rhinoplasty due to caudal septal deformities. Nasal Obstruction Symptom…

Regulation of Angiogenesis Discriminates Tissue Resident MSCs from Effective and Defective Osteogenic Environments.

BACKGROUND: The biological mechanisms that contribute to atrophic long bone non-union are poorly understood. Multipotential mesenchymal stro … non-union sites had comparable differentiation potential to bone marrow MSCs. Compared with induced periosteum, non-union tissue contained similar proportion of colony-forming cells, but a greater proportion of pericytes (p = 0.036), and endothelial cells (p = 0.016) and blood vessels were more numerous (p = 0.001) with smaller luminal diameter (p = 0.046). …

Parathyroid hormone rapidly stimulates hyaluronan synthesis by periosteal osteoblasts in the tibial diaphysis of the growing rat.

Short term treatment (3-24 h) with parathyroid hormone (PTH) stimulated the synthesis and accumulation of hyaluronan (HyA) in explant cultures of tibial diaphyses from young rats. PTH increased the overall HyA content of periosteum 5-fold, with the basal cambium layer exhibiting the greatest enhancement ( approximately 8-fold). PTH increased the HyA content of cortical bone by 2-fold while not affecting the HyA content of bone marrow. PTH treatment greatly enhanced HyA staining throughout all layers of the periosteum, although its most dramatic effect occurred in the basal cambium layer. Here,…

Anatomy and Spread of Gingivobuccal Cancers.

Gingivobuccal cancers affect anatomically distinct yet confluent areas each varying in its ability to resist oncologic spread. Barriers such as periosteum and bone provide greater degree of resistance when compared to buccinator and buccal fat. Involvement of dentition and presence of potential spaces along the mandible and maxilla provide avenues of spread that may result in extensive defects with substantial functional sequalae following tumor extirpation. Owing to the complex 3-dimensional anatomy with varying tissue types preoperative imaging is of utmost importance in surgical planning to…

Thumb Metacarpal Periosteal Sleeve Avulsion: A Case Report.

Periosteal sleeve fractures, or avulsions of cartilage and/or periosteum with or without an osseous fragment in skeletally immature individuals, are notoriously easy to miss and a high index of suspicion is necessary for accurate diagnosis and treatment. While periosteal sleeve avulsion fractures are classically reported in the patella, they have also been reported in the shoulder, clavicle, and elsewhere in the knee. However, no published reports exist for a periosteal sleeve avulsion fracture in the hand. This case details the first reported instance of such an injury involving a thumb metac…

Peripheral Osteoma: A Report of a Case in the Anterior Mandibular Region.

Osteomas are benign lesions that arise from the proliferation of either cancellous or compact bone. They can be categorized as extra-skeletal, peripheral, or central. Extra-skeletal soft tissue osteomas often develop within muscles, while peripheral and central osteomas emerge from the periosteum and endosteum, respectively. Osteomas are usually asymptomatic and rarely affect the jaws. In the mandible, they typically occur in the posterior regions. In this report, we present the case of a 46-year-old female with a mandibular peripheral osteoma located in the anterior region and treated by tota…

Hyperostosis associated with orbital vascular malformation.

A previously healthy adult male presented with a slowly enlarging orbital mass associated with 5 mm of non-pulsatile proptosis. On imaging, a soft tissue lesion with avid contrast enhancement and associated bony hyperostosis was noted. The lesion and hyperostotic bone were surgically debulked, and significant arterial bleeding was noted intraoperatively consistent with an arteriovenous malformation. Histopathologic analysis revealed a vascular malformation with enhanced microvasculature infiltrating the periosteum. While vascular lesions elsewhere in the body can be associated with skeletal ch…
