Chronic pseudo-obstruction secondary to side-to-side intestinal anastomosis.

An unusual late complication of side-to-side intestinal anastomosis, chronic small-bowel obstruction with massive proximal ileal dilation despite a widely patent anastomosis, occurred in a patient. The classic blind loop syndrome was not present. Several potential mechanisms are suggested, including regional absence of normal peristalsis on a mechanical basis and bacterial overgrowth. This report adds support to the concept that side-to-side intestinal anastomosis should be avoided whenever possible.

A dynamic approach for MR T2-weighted pelvic imaging.

Objective. T2-weighted 2D fast spin echo sequence serves as the standard sequence in clinical pelvic MR imaging protocols. However, motion artifacts and blurring caused by peristalsis present significant challenges. Patient preparation such as administering antiperistaltic agents is often required before examination to reduce artifacts, which discomfort the patients. This work introduce a novel dynamic approach for T2 weighted pelvic imaging to address peristalsis-induced motion issue without any patient preparation.Approach. A rapid dynamic data acquisition strategy with complementary samplin…

Clinical significance of esophagogastroduodenoscopy in patients with esophageal motility disorders.

OBJECTIVES: The first aim of this study was to elucidate the detection rate of esophagogastroduodenoscopy (EGD) in patients complaining of d … motility disorder. The rate of abnormal EGD for both EGJ outflow obstruction and major disorders of peristalsis was significantly higher than that for manometrically normal subjects. On multivariate analysis, resistance when passing through EGJ, residue in the esophageal lumen, spastic and nonocclusive contraction were significantly associated with esophageal …

Revealing autoimmune gastritis: Polypoid nodule scar development after endoscopic submucosal dissection for early gastric cancer.

Endoscopic submucosal dissection (ESD), the standard treatment for early gastric cancer, typically results in homogeneous flat scars. Howeve … ontinued to enlarge. PNS growth may be caused by excessive mucosal regeneration and enhanced antral peristalsis, suggesting that hypergastrinemia, which may enhance these effects, may be an underlying cause. Further, elevated serum gastrin levels, decreased pepsinogen levels, the presence of antiparietal cell antibodies, and consistent pathological findings c …

The serotonin receptor mediates changes in autonomic neurotransmission and gastrointestinal transit induced by heat-killed Lactobacillus brevis SBC8803.

Lactobacilli exhibit several health benefits in mammals, including humans. Our previous reports established that heat-killed Lactobacillus brevis SBC8803 (SBC8803) increased both efferent gastric vagal nerve activity and afferent intestinal vagal nerve activity in rats. We speculated that this strain could be useful for the treatment of gastrointestinal (GI) disorders. In this study, we examined the effects of SBC8803 on peristalsis and the activity of the efferent celiac vagal nerve innervating the intestine in rats. First, we examined the effects of intraduodenal (ID) administration of SBC88…

Effects of two anticholinergic drugs, trospium chloride and biperiden, on motility and evoked potentials of the oesophagus.

BACKGROUND: Anticholinergic drugs are known to impair the motor function of the oesophagus but their effects on the oesophageal afferent pathways are unknown. AIM: To determine the effects of a peripherally-acting (trospium chloride) and a centrally-acting (biperiden) anticholinergic drug on the motility and the evoked potentials of the oesophagus. METHODS: Nine healthy volunteers were randomized to receive 1.2 mg trospium chloride (TC), 5 mg biperiden (BIP) or saline i.v. Primary peristalsis was elicited by swallowing a 5 mL water bolus and secondary peristalsis by insufflation of 20 mL air…

Hirschsprungs disease with congenital hypothyroidism.

We report a female newborn baby who presented with vomiting and abdominal distension on day 21 of life. Examination revealed facial puffiness, open posterior fontanelles, dry skin, cold peripheries and prominent abdominal veins with visible peristalsis. Barium enema revealed dilated proximal colon, empty rectum, funnel like transition zone between proximal dilated and distal constricted bowel. Serum TSH level was >;150 uIU/mL. Biopsy revealed aganglionic segment suggesting Hirschsprungs disease, an unusual association with congenital hypothyroidism.

Sonographic diagnosis of bowel obstruction presenting with fluid-filled loops of bowel.

When intestinal obstruction presents with fluid-filled bowel loops instead of air-distended loops, subtle elongated masses representing these bowel loops may be missed on plain abdominal radiographs. In these cases, ultrasound will show sausage-shaped, cystic structures, with uneven margins representing mucosal folds, distributed throughout the abdomen and sometimes accompanied by peristalsis. These features differentiate fluid-filled bowel loops from other cystic structures in the abdomen and help make a diagnosis of bowel obstruction.

Migratory Gossypiboma: An Unusual Presentation Of A Rare Surgical Complication.

Gossypiboma is a rare surgical complication associated with significant patient morbidity and even mortality. Migratory gossypiboma is a rarer subset of these cases, representing the erosion of surgical sponge into the intestine with consequent complications. A 40 years old female presented with a surgical sponge that had eroded into the caecum and subsequently moved with peristalsis up to the anal canal from which it was protruding out at the time of presentation. Proper checking before surgical closure can prevent this serious potentially lifethreatening complication.

INTESTINAL ADHESIONS-Present Status of Prevention and Treatment.

Although many treatments have been proposed for the prevention of intestinal adhesions, none has been completely effective. For bowel obstruction due to adhesions the initial approach should be conservative. If operation becomes necessary, the best results depend on avoidance of trauma and infection, division of adhesions with cautery, use of mesothelial grafts, instillation of intraperitoneal hyaluronidase and stimulation of early postoperative peristalsis. In the event of massive adhesions or failure of other treatment, intestinal plication is the treatment of choice.

[A new procedure for reconstruction of the voice--neoglottis of small intestine].

The fundamental concept in vocal rehabilitation after laryngectomy is the tracheo-hypopharyngeal shunt. The usual contemporary methods possess numerous disadvantages. In order to improve the results a syphon-fashioned shunt with free transplanted jejunum between the tracheal stump and the hypopharynx is created (stimulated by K. Ehrenberger). Aspiration can be avoided by means of the autonomic peristalsis of the implant. The procedure has been performed in 7 cases. One developed necrosis of the loop. The other patients possess a clear voice.

Peristaltic locomotion with a twisted and bent spine in Dermophis mexicanus.

Various aspects of movement, excavation, ecology, and muscle structure of Gymnophiona species have been reported. However, the unique mechanisms and dynamics of peristaltic locomotion and excavation are unclear, particularly for soil-inhabiting Gymnophiona spp. Previous studies primarily focused on forward locomotion and thrusting behavior, with less attention paid to the underlying vertebral and muscular mechanisms. Herein, using X-ray computed tomography, video analysis, and tissue specimens, we determined that the Mexican caecilian (Dermophis mexicanus) could generate peristaltic locomotion

Ureteral Peristaltic Flows in the Presence of Microliths: A Review.

A detailed review is presented on research contributions in peristaltic transport in the human ureter in the presence of microliths. The studies reviewed are differentiated based on methodologies deployed, namely analytical, numerical and CFD simulation techniques and also experimental (clinical) investigations. Various 2D and 3D models are discussed along with more advanced fluid structure interaction (FSI) studies. The propagation of the incompressible urine flow results in reflux nephropathy. As such, the peristaltic waves spread near the outlet of the tube which manifests in a depletion in

Characterization of Uterine Peristaltic Waves by Ultrasound Strain Analysis.

Uterine peristalsis (UP) is a wavelike uterine motion that plays an important role in the generation of intrauterine streams for menstrual emptying and to support embryo implantation. Our understanding of uterine mechanical behavior is hampered by a lack of quantitative analysis. Here, we propose a spatiotemporal analysis of UP by ultrasound speckle tracking and dedicated strain analysis. We aim at characterizing UP propagating around the endometrial cavity through the anterior and posterior walls of the uterus. To this end, velocity and coordination features are proposed in this study. We inv…

Jackhammer Esophagus: Current Concepts and Dilemmas.

Jackhammer esophagus (JE) is a recently recognized esophageal motility disorder that is characterized by hypercontractile peristalsis. More than 500 cases have been reported in the literature. Among patients referred for esophageal motility disorders, the prevalence of JE ranges from 0.42% to 9%, with most series describing a prevalence of 2% to 4%. Most cases are women (60.5%). The mean reported age of patients with JE is 65.2 years, and patients commonly have dysphagia (62.8%). Reflux symptoms occur in ∼40% of patients, and chest pain affects more than one-third of patients (36.4%). JE is a …

Esophageal mucosal sensory nerves and potential mechanoreceptors in patients with ineffective esophageal motility.

BACKGROUND: Ineffective esophageal motility (IEM) is the most common motility disorder. However, little is known about its pathophysiology. Vagal afferent nerves convey esophageal intraluminal bolus information to solitary nucleus, which is likely to be involved with esophageal primary and secondary peristalsis (SP). We hypothesized that altered mucosal sensory afferents underlie the pathogenesis of IEM. METHODS: We prospectively collected esophageal biopsies from 38 patients with proton pump inhibitor-refractory reflux symptoms from January to December 2019. All patients underwent high-resol…

Esophagogastric Junction Opening Parameters Are Consistently Abnormal in Untreated Achalasia.

Achalasia is a disorder of impaired lower esophageal sphincter (LES) relaxation and failed peristalsis traditionally characterized by manometry.1 As impaired LES relaxation is a mechanism of reduced esophagogastric junction (EGJ) opening, abnormally reduced EGJ distensibility assessed with functional luminal imaging probe (FLIP) was reported among patients with untreated achalasia.2-5 Therefore, we aimed to describe the performance characteristics of EGJ opening parameters on FLIP panometry among a large cohort of treatment-naïve achalasia patients.

Is there a role for high resolution manometry in GERD diagnosis?

Gastroesophageal reflux disease (GERD) is a condition, which develops when the reflux of stomach contents causes troublesome symptoms and/or complications. In patients with alarm symptoms or signs or in those refractory to antiacid therapy, it is indicated to perform instrumental tests to correctly diagnose GERD presence. Esophageal manometry is currently considered the gold standard test for the diagnosis of esophageal dysmotility. However, it has shown limited capability in diagnosing GERD. With the advent of high resolution manometry (HRM), more accurate evaluations of esophageal motility a

Non-invasive electrocologram: correlation between the electrical activity of the dog colonic muscle wall recorded by cutaneous and implanted electrodes.

Chronic experiments were performed on dogs with silver, bipolar, ball-shaped electrodes implanted on the colonic and of the gastric muscle wall. Cutaneous electrodes were placed on the abdominal wall, along the projection of descending colon and also of the stomach, at the beginning of each experiment. Slow waves were led off in the electrocolomyogram (EColMG), corresponding to low-amplitude waves in the non-invasive electrocologram (EColG) with a frequency of 6.39 +/- 0.58 cpm (type A). When bursts of spike potentials appeared with the slow waves in the EColMG, increase of the amplitude of th

Radiologic observations of gastric mixing and emptying of food in growing pigs.

The stomachs of 10 Large White x Landrace pigs were examined radiographically from weaning (at 26 to 28 days of age) until 6 or 16 weeks of age. Conventional radiographs were taken and observations were made with a television chain and 35-mm cinecamera linked to an image intensifier. Before radiography, each animal was fed its normal diet that contained a barium sulfate suspension and/or radio-opaque pellets. The basic pattern of motility in the major chamber of the pig's stomach was similar to that described in animals with a simple stomach. Type II peristaltic waves were clearly identified a
