Intravascular volume expansion was studied in 59 critically ill patients with a wide variety of sepsis and in a small group of 12 patients with peritonitis; either 500 ml of 5% albumin solution or 2 units of packed red blood cells were given over a 60-min period. During the 2-h period after volume loading, significant increases in mean arterial pressure (MAP), pulmonary capillary wedge pressure (WP), central venous pressure (CVP), and oxygen consumption (VO2) were observed. One hour after fluid administration MAP had risen from 72 +/- 16 (SD) at baseline to 78 +/- 17 mm Hg (p less than .01), W…

A female Golden Eagle died despite intensive treatment after being struck by a vehicle. Necropsy revealed poor body condition, with no fat stores. The proventriculus and ventriculus contained a pellet of fur and bone fragments, at the center of which was the leg of a Red-Tailed Hawk. A talon on the leg had punctured the ventriculus, resulting in fatal pleuritis and peritonitis.

The authors, having had a case of primary pneumococcal peritonitis, review the features of this pathological condition which has become rare, and of gynaecological pneumococcal infections. Pneumococcal peritonitis presents as a very serious peritonitis and the usual diagnosis that is first made is peritonitis due to appendicitis. Pneumococcal peritonitis can be primary but it is possible that it is often secondary to genital pneumococcal infections. Treatment should always be by laparotomy to confirm the diagnosis, with a peritoneal toilet which is needed in order to stop a pelvic abscess deve…

A series of 19 patients who had amebic peritonitis secondary to rupture of an amebic liver abscess is presented. This represents 2.4% of the patients treated for ALA during an 18.5-year period. Eighty-four percent were men and ages ranged from 6 to 70 years. Rupture occurred prior to admission in 17 patients. Abdominal pain and fever were the most common symptoms. Abdominal tenderness, liver enlargment, distention, and jaundice were the predominant physical findings. All patients underwent operation. Removal of necrotic and purulent material combined with wide drainage were carried out. In two…

Six cirrhotic patients are described in whom peritonitis developed from 10 days to 15 months after the insertion of a LeVeen shunt. The presenting clinical features of fever, increasing ascites and deterioration in mental status resembled these previously reported for cirrhotic patients but with spontaneous peritonitis without shunts. Important differences were observed, however, in microbiology, incidence of bacteremia and therapy. Staphylococcus aureus, a rare cause of spontaneous peritonitis, was found in one half of our cases, suggesting that perioperative contamination during shunt placem…

The author believes that the vascular spasm does not play any part in the occurrence of functional ileus in peritonitis. Humoral factors, such as the loss of electrolytes and serotonin, which is of a specific mediatory effect upon the intestine in particular, are by far more important. It seems that some other humoral factors, e.g. bacterial toxins and endotoxins, formed during the inflammatory process, play their part too. The article is published for purposes of discussion in connection with the paper by Yu. L. Shal'kov. a coll. (Vestnik Khirurgii, 1976, N 4, p. 55).

A 46-year-old man had a prolonged septic peritonitis with shock that resulted from a perforated appendix at the age of 29, followed, six months later, by the gradual development of partial hypopituitarism. Radiographic studies, including a pneumoencephalogram, did not reveal any evidence of sellar or surprasellar masses. Endocrine studies revealed growth hormone and gonadotropic deficiency and possible adrenocorticotropic hormone deficiency and possible adrenocorticotropic hormone deficiency. The nature of the disease course yielded much speculation in regard to pathogenesis of the condition.

The authors present a series of 30 cases of acute supra- and inframesocolic peritonitis which, in addition to the treatment of their organic causes, benefitted from medical treatment by noxythioline, used as a peritoneal lavage, an irrigation with aspiration, or an instillation. A marked amelioration in the postoperative course was noted, particularly in those cases with infra-mesocolic peritonitis, with the absence of adhesions and suppurative collections on reintervention, and a rapid return to negativity of the positive bacteriological investigations performed systematically on the pe riton…

Serological evidence of recent infection with Chlamydia trachomatis was found in 9 of 11 young women with acute peritonitis or perihepatitis proved by laparoscopy. High antibody titres to chlamydiae without laboratory evidence of gonococcal infection were present in 5 of these 9 patients, while the other 4 had evidence of simultaneous gonococcal infection. C. trachomatis may play an important part in peritoneal inflammation previously attributed only to gonococci.

A 32-year old Dominican woman with signs and symptoms of an acute abdomen, was found to have tuberculous peritonitis. Our experience with this patient is presented along with a review of the literature. Population shifts may bring about an increased number of cases in this country. Awareness of this entity will avoid unnecessary diagnostic errors and laparotomy. The surgeon should also be versed in the total management and be prepared to handle the surgical complications of the disease.

A case of sclerosing fibro-peritonitis occurring as a result of practolol therapy is presented. This may occur months after cessation of the practolol because of other side effects and may present to the gynaecologists as a pelvic mass.

According to the authors' data peritonitis developed after appendectomy in 122 of 18347 patients, operated upon for acute appendicitis, was an extremely grave complication of this lesion and the related surgical intervention. The main causes for development of postoperative peritonitis were local and diffuse peritonitis (101 cases), more rarely--technical and tactical errors made during the operation. Due to diagnostic difficulties a purposeful treatment for postoperative peritonitis was undertaken with a delay, reoperations were performed in late terms and not in every case either. The mortal…
