The os intermetatarseum is a rare accessory bone of the foot. It is commonly asymptomatic, as are other such accessory bones of the foot. Nevertheless, when it becomes symptomatic, it can cause "os intermetatarseum syndrome." Reported here is a case of os intermetatarseum syndrome, which is extremely rarely seen. To the best of the authors' knowledge, there are very few cases of os intermetatarseum syndrome in the literature.

BACKGROUND: In 1915, when Delorme described three general requirements for successful nerve repair-(1) resecting scar until a healthy bed is secured, (2) excising damaged nerve until healthy stumps are reached, and (3) placing tension-free sutures, either by adequately mobilizing adjacent joints or nerve grafting-his work was heavily criticized. One century later, history has vindicated all but one of these claims. Flexing adjacent joints to avoid nerve grafts remains controversial, though this practice has increased in recent years.
METHODS: An 11-year-old male suffered a penetrating domesti

The origin of the peroneal intraneural ganglion and the outcome of treatment are still controversial. We report here three cases with peroneal intraneural ganglion and discuss the appropriate treatment. In our cases, 58-, 62-, and 65-year-old patients were operated on with extraneural decompression and epineurotomy within 4 months after onset of drop foot. Two cases demonstrated intraneural ganglion connecting to the articular branch and traversing to the deep and common peroneal nerve. At the 1-year follow-up, paralyzed peroneal nerve could be recovered in all patients even with residual gang

Superficial peroneal nerve and its branches are frequently at risk for iatrogenic damage. Although different studies on anatomical variations of superficial peroneal nerve are available in the medical literature, such reports are rare from India. Hence the present study was undertaken on Indian population. A total of 60 specimens of inferior extremities from 30 properly embalmed and formalin fixed cadavers were dissected and examined for the location and course of the superficial peroneal nerve including number, level, course and distributions of branches. The superficial peroneal nerve in 28.

UNLABELLED: Entrapment of the superficial peroneal nerve is an uncommon entrapment that occurs in sports trauma or fracture and dislocation as the nerve comes under pressure between the underlying muscles and the overlying fascia. Although the superficial peroneal nerve traditionally is depicted as being in the lateral compartment, we have found it in the anterior compartment in some patients. We hypothesized that patients with entrapment of the superficial peroneal nerve were more likely to have this anatomic variant than the normal population and that surgical decompression of both compartme

Autonomic and peripheral nerve function was examined in a group of patients with primary biliary cirrhosis using standard cardiovascular reflex tests and peripheral nerve conduction studies. Sixty-three percent had cardiovascular autonomic dysfunction with predominantly parasympathetic abnormalities. Symptoms of peripheral neuropathy were rarely volunteered spontaneously but occurred frequently when specifically sought; 40.7% had definite peripheral neuropathy, with symptoms and/or signs plus peripheral neurophysiological abnormalities. A close association between autonomic and peripheral nerv

A 68-year-old man with sensory ataxic dominant neuropathy associated with polyarteritis nodosa (PAN) had deep sensory disturbance with unsteady gait and absence of the Achilles tendon reflex. Examination revealed weight loss, elevated CRP level, negative antineutrophil cytoplasm antibodies, decreased M-wave amplitude in the peroneal motor nerve and absence of action potentials in the sural sensory nerve. Sural nerve biopsy revealed a marked loss of myelinated fibers, myelin ovoid formation and necrotizing angiitis of large epineurial arterioles. Renal biopsy revealed global and/or segmental ne

The safety of a blood lead concentration of 70 microgram/100 ml as a hygienic border value with regard to development of lead neuropathy was tested in 95 employees, who had been exposed occupationally to lead for more than 9 years. The blood lead concentration was slightly above the border value in nine subjects, while the erythrocyte-Zn-protoporphyrin concentration was significantly elevated in 81 subjects, indicating an abnormal accumulation of metabolically active lead. None of the group showed clinical evidence of peripheral neuropathy, and the vibratory perception thresholds as well as mo

This is a rare case of sciatic neuropathy in a 46-year-old man secondary to an intraneural ganglion cyst from the hip joint extending upward to the lumbar plexus and the L5 nerve root. Magnetic resonance neurography showed that the joint fluid passed through a labral tear and extended rostrally along the articular branch and the peroneal portion of the sciatic nerve to near the L5 nerve root. The condition was treated by arthroscopic disruption of the joint connection to the nerve with resolution of the intraneural cyst and partial neurological recovery.

A description is given of 34 patients with the distal form of hereditary spinal muscular atrophy. This disorder constitutes one form of "peroneal muscular atrophy". It resembles types I and II hereditary motor and sensory neuropathy but differs from them in displaying less upper limb weakness, relative preservation of the tendon reflexes and an entirely normal clinical sensory examination. Motor nerve conduction velocity and sensory nerve action potentials are normal. Previous reports of this disorder are reviewed and compared with the present series.

Anterior tarsal tunnel syndrome is a rare entrapment neuropathy of the deep peroneal nerve beneath the extensor retinaculum of the ankle. It may be rare because it is underrecognized clinically. We present a case regarding a 29-year-old man, drummer, who for one and a half year experienced clinical symptoms of anterior tarsal tunnel syndrome. A surgical decompression of the anterior tarsal tunnel was performed, and at the check three months later the symptoms where gone. One year after, there were still no symptoms.

Approximately 3% of patients undergoing hip arthroplasty develop postoperative sciatic neuropathy. The factors associated with changes in somatosensory evoked potentials (SSEP) and sciatic neuropathy were examined in patients undergoing hip arthroplasty, to evaluate whether the use of intraoperative SSEP could help reduce the incidence of postoperative sciatic neuropathy. Eighty-eight patients were assigned to either monitored or unmonitored groups. SSEP were recorded following peroneal nerve stimulation, using contralateral stimulation to detect systemic influences on SSEP. Amplitude reductio

BACKGROUND: Peripheral neuropathy in chronic kidney disease (CKD) is the most common neurological complication. We aimed to look at the prevalence and patterns of neuropathy in children with CKD.
METHODS: This cross-sectional study was conducted over 1 year in children with CKD, stage III and above. Nerve conduction studies (NCS) were performed as per standard protocols using surface electrodes on the muscles and by supramaximal stimulation of the corresponding nerves. Presence of electrophysiological abnormalities in the absence of clinical symptoms or signs was considered as subclinical neu

Lewis-Sumner syndrome (LSS) is an atypical variant of chronic inflammatory demyelinating polyneuropathy characterized by an asymmetric sensory-motor neuropathy with multifocal distribution. The diagnosis is typically clinical and electrophysiological but in some cases might be challenging causing a significant therapeutic delay. Diffusion tensor imaging (DTI) has been progressively used for the in vivo assessment of peripheral nerves integrity. In this study, we aimed to elucidate if DTI was able to detect the specific nerve damage in a patient with suspected LSS, and determine if DTI presente

BACKGROUND: Intraneural heterotopic ossification, or neuritis ossificans (NO), is characterized by nonneoplastic calcifications of extra-skeletal soft tissue within a peripheral nerve. This report presents an atypical case of atraumatic NO with a chronic, progressive course that led to a unilateral foot drop. In our review of the published data, there are limited reports describing NO affecting the common peroneal nerve with such profound clinical manifestation.
OBSERVATIONS: A 70-year-old female presented with an 8-year history of progressive right lower extremity weakness and foot drop. Ini

OBJECTIVE: A retrospective analysis of clinical results in a relatively large series of peroneal nerve lesion is presented. Historically, such lesions have been difficult to manage successfully.
METHODS: Between 1967 and 1991, 302 patients with either injury or tumor of the peroneal nerve at the knee were evaluated at Louisiana State University Medical Center. Mechanisms of injury included stretch injury with or without fracture, "sharp" or "blunt" laceration, gunshot wound, compression, entrapment, and iatrogenic injury. Surgery was performed on 183 of 276 patients (66%). If spontaneous reco
