Neurologic problems of the lower extremity associated with HIV and AIDS.

Lower extremity symptoms are caused by lesions at any level of the neuraxis, from cortex to muscle. HIV affects virtually every level of the … n and may follow a rapidly progressive course unless treated promptly and aggressively. Progressive polyradiculopathy occurs late in the course of AIDS, is often caused by cytomegalovirus, is rapidly progressive, and generally is fatal unless recognized and treated promptly. Muscle weakness, myalgia, and fatigue are common in HIV and have multiple causes. Lower ext …

Cauda Equina Dural Sac Hernia Caused by the Drainage Tube After Lumbar Unilateral Biportal Endoscopy Surgery: A Case Report.

Previously reported cases of cauda equina herniation primarily involve compression of the spinal cord caused by fracture fragments intruding into the spinal canal following traumatic spinal burst fractures or by dural tears occurring during spinal endoscopic surgery. Such cases can often be evaluated preoperatively or intraoperatively to facilitate the selection of an appropriate treatment plan. However, cauda equina herniation resulting from the use of a drainage tube following spinal endoscopic surgery has not yet been reported. In this article, we present a case of a 76-year-old male patien

Early intervention in cauda equina syndrome associated with better outcomes: a myth or reality? Insights from the Nationwide Inpatient Sample database (2005-2011).

BACKGROUND CONTEXT: Evidence-based consensus on timing to surgical decompression following symptom onset in patients with cauda equina syndrome (CES) is limited or widely debated. PURPOSE: This study aimed to investigate whether timing to intervention in the management of patients with CES has an impact on outcomes. STUDY DESIGN/SETTING: This is a retrospective cohort study. PATIENT SAMPLE: The patient sample included 4,066 adult patients with CES registered in the Nationwide Inpatient Sample database (2005-2011) and undergoing elective decompression surgery. OUTCOME MEASURES: The outcome

Obesity and spinal epidural lipomatosis in cauda equina syndrome.

BACKGROUND CONTEXT: Although lumbar disc herniations are common, only a small portion of these herniations lead to cauda equina syndrome (CES), which is an uncommon but debilitating disorder. Why some patients with herniation develop CES, when most do not, remains unknown. Preexisting subclinical epidural lipomatosis may limit canal space such that an otherwise benign herniation causes CES. PURPOSE: This study determines whether patients with an acute disc herniation and CES have a greater body mass index (BMI) and greater quantity of epidural fat compared with control subjects with non-CES s

Idiopathic Lumbar Epidural Lipomatosis Mimicking Disc Herniation: A Case Report.

Spinal epidural lipomatosis is a rare condition which is described as the accumulation of fat in the extradural territory and often causes dural impingement. Spinal epidural lipomatosis has been implicated in causing a variety of neurologic impairments ranging from back pain, radiculopathy, claudication, myelopathy or even cauda equina syndrome. We report a 46-year-old female with obesity and a history of chronic back pain and radiculopathy who developed idiopathic Spinal epidural lipomatosis diagnosed by magnetic resonance imaging. The purpose of this report is to present a case of spinal epi

The cauda equina syndrome in pregnant woman with a massive disc herniation.

Low back pain during pregnancy is a common cause of medical consultation. Although back pain is very common, the incidence of low back pain secondary to lumbar disk herniation in pregnancy is low (1: 10,000). Cauda equina syndrome from lumbar disk herniation is a serious complication. The delay in diagnosis and treatment can be a cause of chronic disability secondary to neurological sequelae. Numerous cases of disk herniation in pregnancy have been reported, however the association of a cauda equina syndrome as a result of disk herniation is rare. A case is presented of cauda equina syndrome i

Intradural invasion of lumbar synovial cyst. Case report.

A 63-year-old female presented with a rare case of synovial cyst invading the dura mater and mimicking an intradural extramedullary tumor in the lumbar spine. She underwent posterior lumbar fusion with laminar wire fixation from L3 to S1 levels. She complained of severe pain along the anterolateral thigh in both legs for 1 year. Radiological examinations, including magnetic resonance imaging and computed tomography revealed an intradural extramedullary lesion at the L2-3 level that was compressing the thecal sac. Histological examination confirmed a synovial cyst. Synovial cyst should be consi

Cauda equina compression post lumbar discography.

Discography is used as an aid in the diagnosis of back pain related to intervertebral disc pathology. It involves attempting to elicit the patient's pain symptoms by injecting contrast into the suspected pathological disc. The overall complication rate of discography is low, with discitis being the most common complication and acute disc herniation post lumbar discography being reported in a small number of cases. We describe the case of a patient who developed cauda equina compression post lumbar discography.

The utility of lumbar paraspinal mapping in the diagnosis of lumbar spinal stenosis.

OBJECTIVE: The aim of this prospective, blinded and controlled study is to evaluate the utility of lumbar paraspinal mapping in the diagnosis of lumbar spinal stenosis. DESIGN: The subjects were assessed and allocated into three groups according to clinical and radiologic features with a standardized assessment protocol. These three groups were clinical and radiologic lumbar spinal stenosis, radiologic lumbar spinal stenosis, and the control group. The measurements of magnetic resonance imaging studies were performed by a blinded radiologist. An electromyographer who was masked to patients' d

Clinical decision rules for identification of low back pain patients with neurologic involvement in primary care.

STUDY DESIGN: Descriptive study. OBJECTIVE: To compare clinical decision rules in low back pain guidelines for identification of neurologic involvement. SUMMARY OF BACKGROUND DATA: Low back pain guidelines have been developed in a number of countries. Guideline recommendations for assessment of patients with low back pain in primary care include clinical decision rules for identification of neurologic involvement. Broad variation in recommended clinical assessments has previously been identified. More specific investigation of these clinical assessments seems warranted given that guidelines

The redundant nerve root syndrome of the Cauda equina.

The redundant nerve root syndrome is defined as the association of high-grade extradural lumbar spinal stenosis with large, elongated and tortuous nerve roots. Acquired elongation of nerve roots due to the mechanical trapping at the level of lumbar spinal stenosis is assumed to be the possible mechanism. It is believed that the cause is a squeezing force due to the chronic compression. The most common clinical symptoms are low back pain and leg pain. Although lumbar spinal canal stenosis is common, the entity has rarely been discussed in the literature. Here we present the MR imaging and intra

Cystic dilation of the conus ventriculus terminalis presenting as an acute cauda equina syndrome relieved by decompression and cyst drainage: case report.

OBJECTIVE AND IMPORTANCE: The ventriculus terminalis of the conus, or "fifth ventricle" refers to the ependymal-lined space in the middle of the conus that is present in childhood and whose persistence into adulthood is rare. A number of cases of cystic dilatation of the ventriculus terminalis have been described in adulthood. Patients tend to present with either pain alone or gradually progressive conus or cauda equina syndromes with varying degrees of recovery after cyst drainage. Presentation with an acute cauda equina syndrome and its successful surgical management has not been previously

Posterior epidural migration of sequestered lumbar disc fragment causing cauda equina syndrome.

Posterior epidural migration of free disc fragments is rare, and posterior migration of the free fragments causing cauda equina syndrome is exceptionally rare. This report describes a 53-year-old man with disc fragment extrusion at the levels of L3-4 and a 54-year-old man with disc fragment extrusion at L5-S1 intervertebral space. The patients responded well to the operative therapy with complete relief of the symptoms. The pathological examination confirmed that the specimen was a degenerated intervertebral disc. Early surgery should be the first choice of therapy in patients with large poste
