Proximal Knee Ganglion Cyst: A Case Report and Review of Treatment Strategies.

Ganglion cysts are commonly encountered benign lesions, primarily localized near the wrist and hand but occasionally observed in the lower extremities. These cysts, characterized by their encapsulation in dense connective tissue, typically present with minimal symptoms, albeit occasionally causing discomfort or limited mobility. Herein, a unique case of a ganglion cyst in the posterior distal lower extremity is presented, located near the popliteal neurovascular structures and without any connection to the knee joint. Given the critical proximity to neurovascular structures and the patient's m

Liposarcoma simulating a Baker's cyst: a case study.

We present a case of liposarcoma in a 65-year-old man in whom the tumor first appeared as a soft, cystic mass at the medial margin of the left popliteal fossa. The mass simulated a Baker's cyst. The patient remained untreated for almost 7 months from the discovery of the mass. With worsening pressure symptoms, a biopsy was done that confirmed the clinical diagnosis of liposarcoma. The leg was amputated, but the patient died 2 years later with clinical evidence of pulmonary metastasis. By presenting this case report, we stress the importance of carefully investigating for malignancy all masses

[Full popliteal fossa].

Many reasons of clinical discussions about a filling of the popliteal fossa arise from this special anatomical area which is encumbered, so often requested by the activity of joints and muscles and so, since the embryological development. Although rare, perhaps because unrecognised, ischemic disorders, venous signs, swollen legs, permanent or intermittent full fossa . . . are the more frequent clinical aspects. The main patterns are arterial, rarely venous and more rarely arteriovenous aneurysms, adventitial cystic disease, lipoma, synovial or sebaceous cyst . . . malignant tumours, exostosis

Intraneural ganglion cyst of the tibial nerve.

Intraneural ganglion cyst of the tibial nerve is very rare. To date, only 5 cases of this entity in the popliteal fossa have been reported. We report a new case and review the previously reported cases. A 40-year-old man experienced a mild vague pain in the medial half of his right foot for 3 years. Magnetic resonance imaging scan demonstrated a soft-tissue mass along the right tibial nerve. At surgery, an intraneural ganglion cyst was evacuated. After 12 months, the patient was pain-free with no signs of recurrence. Trauma might be a contributing factor to the development of intraneural gangl

Baker's pseudocyst in the prosthetic knee affected with aggressive granulomatosis caused by polyethylene wear.

It is our belief that this is the first histologically documented case of popliteal cyst secondary to early failure of a cementless knee prosthesis, that occurred after 4 years. The walls of the cyst presented with granulomatous reaction to polyethylene particles. In prosthetic reimplantation successive to excision of the cyst we observed a recurrence of cysts and osteointegration of the prosthetic components, that made reimplantation difficult. Histological assessment of the synovial tissue, periprosthetic tissue and underlying bone showed granulomatous reaction to polyethylene debris. A "tum…

Pseudothrombophlebitis in an adolescent without rheumatic disease. A case report.

Pseudothrombophlebitis syndrome is the occurrence of calf pain and swelling caused by extrinsic compression of the popliteal vessels by an enlarging Baker's cyst or by calf inflammation that occurs as the result of a ruptured Baker's cyst. Few cases of pseudothrombophlebitis syndrome have been reported in patients less than 18 years of age, and nearly all these young patients had juvenile rheumatoid arthritis. Reported here is the case of a 17-year-old male patient without rheumatic disease who presented to the outpatient clinic with a 1-week history of an increasingly painful swelling of the

Tibial tunnel and pretibial cysts following ACL graft reconstruction: MR imaging diagnosis.

Tunnel cyst formation is a rare complication after anterior cruciate ligament reconstruction, usually occurring 1-5 years post-operatively, which may occasionally be symptomatic. There are multiple proposed theories regarding the etiology of tunnel cysts. Theories include necrosis, foreign-body reaction, lack of complete graft osteo-integration, and intravasation of articular fluid. It is important to know if the tunnel cysts are communicating or not communicating with the joint, as surgical management may be different. Imaging characteristics on magnetic resonance images (MRI) include tibial

Surgical approaches to the posteromedial and posterolateral aspects of the knee.

Surgical approaches to the posterior aspect of the knee are not commonly needed, and their use has become even rarer with the increasing sophistication of arthroscopic technology. As a result, physicians in orthopaedic surgical training are not often exposed to the practical use of surgical dissection around the posterior corners of the knee. For certain procedures, however, greater clinical utility and decreased surgical morbidity render focused posterior exposure the preferred alternative to the classic popliteal dissection with its wide exposure of the popliteal anatomy. Surgical indication

Ultrasound of knee osteoarthritis: interobserver agreement and correlation with Western Ontario and McMaster Universities Osteoarthritis.

The aim of this work was to assess the reproducibility of ultrasound findings of knee osteoarthritis and to correlate ultrasound findings with Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC). Prospective study was conducted upon 80 patients (56 F, 24 M; mean age 57 years) with primary osteoarthritis of knee joint. All patients underwent clinical assessment with calculation of WOMAC and high-resolution ultrasound of the knee joint. The ultrasound images were analyzed for cartilage thinning, osteophytes, synovial effusion, synovial proliferation, popliteal cyst, and menisc…

Synovial cysts and sarcoid synovitis.

The case of a practice nurse who presented with painless cystic swellings in the popliteal and antecubital fossae and lymphoedema of the legs is reported. Biopsies of synovium, a breast lump, axillary lymph nodes and of the Kveim site demonstrated sarcoidosis. Prior to biopsy diagnosis was not obvious. Synovial cysts are a very rare manifestation of sarcoid joint disease and this case illustrates the importance of biopsy in establishing the diagnosis.

Communicating semimembranoso-gastrocnemial Bursa.

In a prospective study we examined lateral radiographs of the knee joint before and after flexion exercises, in 300 consecutive arthrograms. We use the following definitions: (a) a popliteal or Baker's cyst is a clinical diagnosis and implicates a palpable popliteal mass, mostly in adults; (b) a communicating bursa is the semimembranoso-gastrocnemial bursa (s.g. bursa) communicating with the knee joint which is visible in the lateral arthrogram of the knee. It appears that the communicating s.g. bursa is more often present with knee pathology. The bursa has no other clinical or therapeutic sig

Pseudo-tumor of the calf.

The differential diagnosis of calf swelling includes deep venous thrombosis (DVT), ruptured popliteal (Baker's ) cyst, infection, sarcoma and hematoma. Bony erosion is seldom seen in benign conditions and, when present, usually indicates tumor involvement. 'Pseudosarcoma', where hematoma may mimic sarcoma, has been described in hemophiliacs. A case is described of pseudosarcoma with bone erosion occurring as a complication of anticoagulant therapy, a clinical situation not reported previously.

[Ultrasonography and computerized tomography in giant Baker's cyst].

The US study of the posterior compartment of the knee usually allows the clinical suspicion of popliteal cyst to be confirmed. Nevertheless, in case of giant Baker's cysts--which are, at any rate, less frequent--US diagnosis is more difficult. CT can help determine the benign nature of the lesion, and allow an accurate spatial evaluation. Moreover CT, when accurately performed, demonstrates the caudal or cranial cystic spread, together with its clear separation from adjacent muscular tissues. Direct coronal scans are more useful to obtain better spatial definition than reformatted images.

[Cystic adventitial disease of the popliteal artery].

Although rare, popliteal artery cystic adventitial disease (CAD) is one of the most common non-atheromatous causes of intermittent claudication. This work aims to describe the clinical case of a 50 years old male patient with symptomatic popliteal artery CAD, presenting with calf intermittent claudication for long distances on flat but 50m in ascending plane, which affected his life quality. On examination he presented with normal coloration and temperature of the lower extremities, with distal pulses present at rest. By flexing the right knee, he lost distal pulses. On ultrasound was found a

Mucoid degeneration of the brachial artery: case report and a review of literature.

Cystic adventitial disease (CAD) is a rare cause of intermittent claudication, occurring in approximately 1:1200 claudicants or 1:1000 of those undergoing arteriography. It is most often described in the popliteal artery and is characterised by a mucinous cyst located in the adventitia of the artery, the contents of which resemble those of a ganglion. The origins of adventitial cysts are unknown, but connections to adjacent synovial spaces have been identified, suggesting that the cyst is a variant of a ganglion. In this report, we discuss a rare case of severe mucoid degeneration of the intim

Two Cases of Popliteal Cystic Adventitial Disease Treated with Primary Stenting: Long-Term Results.

The cystic adventitial disease of the popliteal artery is an uncommon cause of intermittent claudication in young patients. Several treatment options are available, oriented to either drainage of the cyst and/or arterial reconstruction. Endovascular techniques have been exceptionally used to treat this condition, with mixed results. We report 2 young claudicants treated with primary stenting with continuous 4- and 10-year symptomatic relief and arterial patency.
