Determinants of circadian blood pressure rhythm and blood pressure variability in obstructive sleep apnoea.

The prevalence of hypertension in patients with obstructive sleep apnoea (OSA) is high and blood pressure profile is characterized by nocturnal blood pressure (BP) elevation and increased nocturnal BP variability. Ambulatory 24-hour-blood pressure monitoring (ABPM) is a valid, non-invasive method to describe circadian BP variation. Circadian BP profile and nocturnal BP variability were related to OSA severity (apnoea-hypopnoea index, mean low O2), age and body mass index (BMI) in 73 patients with OSA. Prevalence of hypertension was 75%, and in 59% BMI was greater than 30 kg m-2. A nocturnal de…

Correlation between micturitional urethral pressure profile and pressure-flow criteria in bladder outlet obstruction.

PURPOSE: We correlate micturitional urethral pressure profilometry with pressure-flow diagnoses of outlet obstruction. MATERIALS AND METHODS: Urodynamic evaluation was done of 86 consecutive men with voiding symptoms. Obstruction criteria were a micturitional urethral pressure profile (MUPP) gradient greater than 5 cm. water, Schäfer's linear passive urethral resistance relation (PURR) greater than grade 1 and Abrams-Griffiths nomogram. RESULTS: Interpretable results were completed in 99% of the patients undergoing MUPP and 60% undergoing pressure-flow studies (p < 0.00001). MUPP diagnosis a…

Pressure-induced striatal dopamine release correlates hyperlocomotor activity in rats exposed to high pressure.

Free-moving rats chronically implanted in the striatum with multifiber carbon electrodes selective to dopamine were compressed in a helium-oxygen mixture to 80 bars. Extracellular dopamine level and behavioral symptoms of high-pressure neurological syndrome were simultaneously recorded. Under these conditions, the extracellular level of dopamine monitored by differential pulse voltammetry was found to be pressure dependent, and hyperlocomotor activity, a behavioral symptom of high-pressure neurological syndrome, was found to be linked to these pressure-induced changes in dopamine release.

National High Blood Pressure Education Program Working Group report on ambulatory blood pressure monitoring.

Blood pressure measurements obtained in a physician's office may not represent a patient's blood pressure during an entire day. Ambulatory blood pressure monitoring (ABPM) provides multiple readings over time. This review of the literature on ABPM addresses the current state of ABPM methods, normal blood pressure profiles, the clinical and research uses of ABPM, cost considerations, and recommendations for use of ABPM in selected circumstances. Current ABPM devices use either auscultatory or oscillometric methods to determine blood pressure. A rigorous comparison of these methods is needed to …

Circadian rhythm of blood pressure in hypertensive adult patients: implications for blood pressure determination.

To determine whether traditionally timed (0800, 1600, and 2400 hr) blood pressure assessments by nurses reflect circadian peaks and troughs, blood pressure was measured indirectly every 2 hr for 24 hr with a mercury sphygmomanometer and a Sprague-Rapaport stethoscope in a sample of 31 hypertensive adult patients in a university hospital. Pooled mean systolic and diastolic readings portrayed a circadian rhythm with the peak occurring most frequently at 1600 and the trough at 0400 hr. The mean range of variability was 29.54 mm Hg for systolic pressure and 21.54 mm Hg for diastolic pressure. Two-…

Measurement of effective pulmonary capillary pressure using the pressure profile after pulmonary artery occlusion.

Pulmonary artery catheters are frequently used to measure pulmonary vascular pressures, particularly the pulmonary wedge pressure (Pw), which reflects pulmonary venous and ideally left atrial pressures. However, the pulmonary capillary pressure (Pc) is the major force in the formation of pulmonary edema. Unfortunately, Pw has been interpreted as being identical to Pc. In this study we used 7-Fr pulmonary artery catheters to measure effective Pc in closed-chest animals and patients. The decreasing pressure profile after pulmonary artery occlusion was separated into fast and slow components, wit…

Central venous pressure and pulmonary wedge pressure. A comparative study in anesthetized surgical patients.

To determine the reliability of central venous pressure (CVP) as a guide to fluid therapy during an operation, repeated and simultaneous CVP and pulmonary wedge pressure (PWP) measurements were made with a Swan-Ganz catheter in 13 relatively elderly patients without obvious cardiac or respiratory disease- Overall correlation between CVP and PWP was highly significant (P less than .001); there was, however, an important variation of the correlation for each patient. For values of CVP greater than or equal to 8 mm Hg, the correlation was not significant. The disparity between right and left vent…

Postpartum blood pressure trends and time in target range during remote blood pressure monitoring.

INTRODUCTION: Postpartum remote blood pressure monitoring (RBPM) programs have emerged as an important adjunct to the management of hypertensive disorders of pregnancy (HDP). These programs have demonstrated robust patient participation and satisfaction with reduced readmissions and severe maternal morbidity. Despite this, data describing blood pressure (BP) trends throughout program participation and across different antihypertensive regimens remain limited. We sought to evaluate BP trends and time in target range (TTR) during our 6-week RBPM program. METHODS: This is a retrospective cohort …

Mean arterial pressure and pulse pressure trajectories and clinical outcomes after acute ischemic stroke.

Distinct blood pressure (BP) trajectories were observed after acute stroke and associated with clinical outcomes. This study assessed fluctuations of mean arterial pressure (MAP) and pulse pressure (PP) and evaluated the prognostic significance of MAP and PP trajectory patterns after acute ischemic stroke. We included 4008 acute ischemic stroke patients from the China Antihypertensive Trial in Acute Ischemic Stroke. Latent class mixed modeling identified distinct MAP and PP trajectories using 37 repeated measurements during the first 7 days. We identified five distinct trajectories for MAP and…

Relationship between nocturnal blood pressure variations and intraocular pressure in patients with essential hypertension.

This study aimed to evaluate the association between intraocular pressure (IOP) and circadian blood pressure (BP) patterns-specifically dipper and nondipper status-in patients with newly diagnosed essential hypertension. A total of 180 participants were enrolled and equally divided into individuals with normal blood pressure (n = 90) and individuals with hypertension (n = 90). Individuals with hypertension were further classified into dipper and nondipper subgroups based on 24-h ambulatory BP monitoring. IOP was assessed using a noncontact tonometer during morning hours (9:00 a.m.-12:00 p.m.).…

Contact Pressure Under Traditional Foam and Novel Thermoplastic Elastomer Negative Pressure Wound Therapy Interfaces.

Negative pressure wound therapy relies on suction, commonly set to -125 mmHg, to evacuate wound fluid, but dysvascular and pressure injuries often respond unpredictably. Newton's third law indicates that negative pressure applies an equal and opposite contact pressure on the wound surface, and this study quantified this contact pressure across dressing types, wound configurations and suction levels. Using a porcine explant model, central and peripheral contact pressure measurements were obtained beneath black reticulated open cell foam, white foam and a novel thermoplastic elastomer dressing a…

Linking compression mechanics and pressure redistribution in hybrid support surfaces for pressure ulcer prevention.

Pressure ulcers are a major concern for individuals with limited mobility. This study developed six hybrid textile-based support surfaces as low-cost, non-electric alternatives for pressure redistribution. Each surface was constructed by layering cotton pile and spacer fabrics to achieve mechanical gradation and improved body conformity. Compression behavior and interface pressure distribution were systematically analyzed in supine and lateral postures using a proof-of-concept model. To ensure reliability, triplicate measurements were averaged after a 30-second stabilization period. The result…

Clinical Validation of a Novel Pressure Microcatheter-Based Nonhyperemic Pressure Ratio (SUPREME II Study).

BACKGROUND: The constant resistance ratio (cRR) is a novel nonhyperemic pressure ratio based on piezoresistive pressure microcatheter (PMC) measurements. With repeated measurements in randomized order of PMC and pressure wire techniques, this study aimed primarily to validate the diagnostic performance of cRR compared with fractional flow reserve (FFR) in coronary lesions of 30% to 90% diameter stenosis. METHODS: SUPREME II (Sensor-Equipped Ultrathin Pressure Microcatheter Versus Pressure Wire for Physiological Measurements) was a multicenter, prospective study that included 466 patients (483…

Beyond Mean Arterial Pressure: Organ Perfusion Pressure as a Hemodynamic Target in Cardiogenic Shock.

BACKGROUND: Current cardiogenic shock (CS) management primarily targets mean arterial pressure (MAP), often overlooking the impact of venous congestion on the effective perfusion gradient. Organ Perfusion Pressure (OPP = MAP - CVP) integrates both. We investigated OPP's prognostic value for mortality, renal function, and resource utilization. METHODS: This retrospective cohort study utilized MIMIC-IV database, a single-center, critical care database, to identify adult CS patients. Patients with other forms of shock were excluded. Primary outcomes were in-hospital and ICU mortality. Secondary …

Point-of-Care Ultrasound Correlates of Pulmonary Capillary Wedge Pressure and Right Atrial Pressure.

OBJECTIVES: Volume assessment remains a challenging bedside practice that is highly dependent on patient factors, comorbid conditions, physical exam, and point-of-care ultrasound (POCUS) techniques. POCUS is increasingly used to assist with diuretic decision-making for patients with suspected volume overload. While developing evidence suggests that POCUS allows for rapid and safe evaluation of volume status, limited data exist regarding which POCUS assessments correlate best to filling pressure measurements obtained from rightheart catheterization (RHC). METHODS: A single-center observational…

Low-Pressure-Threshold Triggered Irreversible Emission Transformation toward Flexible Pressure Imaging and Information Encryption.

Pressure-driven irreversible emission transformation as an efficient means greatly facilitates the development of the rational structural design of high performance novel metastable states. However, the excessively required pressure can only be achieved by high-pressure devices, significantly inhibiting practical applications. Here, we achieved an irreversible emission transformation in 0D copper halides (TPA)CuI2 (TPA = Tetrapropylammonium) nanocrystals (NCs) via high pressure engineering, even at pressure as low as 0.6 GPa (approximately 6000 atmospheres). The large steric hindrance of organ…
