Priapism is a rare urologic emergency in pediatric population. It is usually a consequence of sickle cell disease. Leukemia is another important cause of priapism in children. We present a case of priapism associated with an acute pulmonary infection. To our knowledge, this is the second case describing this association. In the other reported case, pulmonary infection was related to Mycoplasma Pneumoniae. The hypothesis is that infection with Mycoplasma Pneumoniae can produce a hypercoagulable state, especially in selected areas of the circulation.

Male sexual dysfunction takes many forms. Sexual disorders include common conditions that patients may regard as simply a minor inconvenience, such as premature ejaculation; less common but medically serious disorders, such as lowflow priapism; and uncommon conditions, such as Peyronie's disease. These conditions may appear in conjunction with more generalized erectile dysfunction, or they may appear in isolation. Discerning the precise nature of the disorder is a crucial aspect of determining the best course of treatment.

The case of a young man with depressive illness who developed priapism while being treated with paroxetine is reported for the first time. Other sexual side effects are reported with paroxetine : delayed ejaculation, anorgasmy, erectile dysfunction and decreased libido. Other antidepressants are known to cause priapism : fluoxetine and especially trazodone.

We report our experience with 9 cases of priapism treated with decompressive puncture of the corpora cavernosa or with spongiosum-cavernosum shunt. No significant difference in result between these two surgical approaches was found: we conclude that early treatment is decisive for the preservation of potency.

Intracorporeal administration of papaverine hydrochloride is frequently used in the diagnosis and treatment of impotence. Priapism is the most serious potential complication of its use. The historical characteristics of 400 consecutive men receiving a standardized dose of papaverine were reviewed in an effort to determine the risk factors for prolonged erections. We report a case of traumatic priapism with an evolution of three weeks at the time of the diagnosis. The foregoing condition had been caused by an arteriocavernal fistula. Several arteriograms and selective embolization of the compromised area were required for treatment. The etiological, pathophysiological and therapeutic aspects are discussed.

Priapism is a complication of sickle cell disease, and for those with severe prolonged attacks, it is serious, often resulting in permanent sexual impairment. Treatment is frustrating, and psychological consequences may be profound. A 20-year-old sickle cell patient with intractable priapism associated with intense pain and penile gigantism underwent multiple but unsuccessful medical and surgical treatment regimens. Priapism (P) may develop from unknown causes or may occur secondary to other disease or as a side effect of medication. The occurrence of pharmacologically induced erection by intracavernous injection of drugs has resulted in a number of cases of iatrogenic priapism. Priapism occurred as a result of leukostasis due to an extreme rise in leukocyte count (503,000/microliter, 96% lymphocytes) in a 46-year-old man with chronic lymphatic leukaemia. In addition to surgical treatment, anticoagulant administration and cytostatic treatment several courses of lymphapheresis were performed using a cell separator. This latter procedure quickly reduced the number of lymphocytes and improved the patient's operability.

Two patients with sickle cell anemia are reported who developed relative impotence after repeated episodes of priapism. They learned that excessive ethanol ingestion produced an erection and utilized this mechanism to satisfy sexual partners. Hospitalization and transfusion therapy were required for repeated episodes of priapism. Extensive interviews were required in order to learn the sequence of events.
