Craniofacial Evolution and Alveolar Bone Loss: A Lesson From Hominins.

BACKGROUND: In hominins, the reduction of prognathism during craniofacial evolution was a significant derived trait differentiating Homo from earlier hominins and other apes and might have contributed significantly to calvarial expansion and encephalization. Gnathic remains of Australopithecus africanus , A. robustus , Homo habilis, and H. erectus from the Plio-Pleistocene boundary 5.3-2.6 MYA were studied for evidence of alveolar bone (AB) loss indicative of periodontal disease(s). AB loss may provide critical insights into craniofacial evolution and the divergence of Homo from the Australopi…

A 3D analysis of growth trajectory and integration during early human prenatal facial growth.

Significant shape changes in the human facial skeleton occur in the early prenatal period, and understanding this process is critical for studying a myriad of congenital facial anomalies. However, quantifying and visualizing human fetal facial growth has been challenging. Here, we applied quantitative geometric morphometrics (GM) to high-resolution magnetic resonance images of human embryo and fetuses, to comprehensively analyze facial growth. We utilized non-linear growth estimation and GM methods to assess integrated epigenetic growth between masticatory muscles and associated bones. Our res

Endotracheal Intubation in a Down Syndrome Adult Undergoing Cataract Surgery - a Multidisciplinary Approach.

The present case report aims to describe and discuss the approach for the management of difficult endotracheal intubation in an adult with D … rformed under general anesthesia. Indicators of potentially difficult intubation were macroglossia, prognathism, short neck, limited degree of head extension and obesity. The pre-operative examinations, which revealed a high degree of endotracheal intubation, allowed the anesthetist to achieve a better peri- and intra-operative management of the patient.

Ontogeny of the maxilla in Neanderthals and their ancestors.

Neanderthals had large and projecting (prognathic) faces similar to those of their putative ancestors from Sima de los Huesos (SH) and different from the retracted modern human face. When such differences arose during development and the morphogenetic modifications involved are unknown. We show that maxillary growth remodelling (bone formation and resorption) of the Devil's Tower (Gibraltar 2) and La Quina 18 Neanderthals and four SH hominins, all sub-adults, show extensive bone deposition, whereas in modern humans extensive osteoclastic bone resorption is found in the same regions. This morph

BaS analysis: a new cephalometric study for craniofacial malformations.

OBJECT: Patients submitted to surgical advancement of the upper and middle facial third might have a relapse of the skeletal retraction after the removal of the distractor devices. Clinical signs related to this process are usually represented by exophthalmos, Obstructive sleep apnea syndrome (OSAS) or prognathism; however, the analytic definition of the relapse is very difficult , conventional cephalometric methods being influenced by the spatial movement of "N" (nasion) or "Or" (orbital) points. The authors present a new cephalometric technique that could be used as a more objective mean of …

Association of advanced hypodontia and craniofacial morphology in Japanese orthodontic patients.

The purpose of this study was to examine the effects of advanced hypodontia on craniofacial morphology in Japanese patients. We defined advanced hypodontia as a congenital absence of four or more permanent teeth, excluding the third molars. Lateral cephalometric radiographs of 20 female orthodontic patients (age range, 8.5 to 19 years; mean age, 13.9 years) were examined. Twenty-eight angular and 37 linear measurements were taken from each cephalogram, and these cephalometric data were statistically analyzed and compared with the Japanese cephalometric standards. Foetal anasarca in Awassi sheep.

Two anasarcous foetuses of Awassi sheep are described. The foetuses were removed from the dams by caesarean section because of dystocia due to failure of cervical dilation. Uterine incision was made in situ because uteri were so distended they could not be brought out from the site of incision. Large quantities of uterine fluids and abnormal thick placentas were found. One foetus weighed about 7 kg and the other 13 kg. The foetal heads were deformed: the upper jaw was prognathic and the left ear of the small foetus was cystic. Necropsy revealed subcutaneous musculature was soft and flabby and

Facial and dental anomalies in cleidocranial dysplasia: a study of 17 cases.

Seventeen cases of cleidocranial dysplasia were analysed. On average, there was mandibular prognathism due to increased mandibular length, and a short cranial base. The maxilla was short vertically but not antero-posteriorly. In 13 young subjects followed for an average period of 6 years, more than half of the total number of teeth of each type had not erupted at the normal age, with the exception of mandibular incisors. Twice as many maxillary incisors were delayed compared with those in the lower jaw. Only six of 25 mandibular central incisors were delayed. There was a total of 63 supernumer…

Growth status of children treated for unilateral cleft lip and palate.

Cephalometric distances, angles, and proportions were evaluated for 32 children 5 to 8 years of age treated for unilateral cleft lip and palate. The children were age and sex matched with untreated controls with normal skeletal relationships. The unilateral cleft lip and palate sample was treated by the same surgeon and orthodontist using the same techniques and appliances. Measures of overall facial proportions, facial convexity, and prognathism were not significantly different between the two groups. The primary group differences pertain to the posterior aspect of the maxilla, which is verti…

Familial osteodysplasia.

A family with characteristics that have striking similarities to the syndrome of familial osteodysplasia is reported. The inheritance pattern appears to be autosomal dominant. The typical facies has a lack of vertical facial development and a functional prognathism caused by overclosure of the mandible and opening of the gonial angle. Midfacial hypoplasia and a pointed chin are characteristic. Alveolar and sutural bone growth in the face is diminished and associated with partial dental agenesis. Variable cranial digital markings and possible synostosis are also seen. These typical facial chang…

An Uncommon Case of Hypophosphataemia-Non-Lethal Raine Syndrome With Novel FAM20C Variant: Expanding the Phenotypic Spectrum.

Raine Syndrome (MIM #259775) is an autosomal recessive osteosclerotic disorder due to biallelic variants in the FAM20C gene. It is classified into two subtypes based on perinatal lethality. Here, we report an 18-year-old male who presented with dental problems in childhood and subjective gait instability, followed by an incidental finding of hypophosphataemia and multiple-level spinal stenosis. Examination showed characteristic features including a high forehead, midface hypoplasia, prognathism, amelogenesis imperfecta, brachydactyly and bulbous finger tips, and further biochemical workup reve…

[Acanthosis nigricans in children and Crouzon syndrome].

BACKGROUND: Crouzon syndrome with acanthosis nigricans is a rare form of Crouzon syndrome in which craniosynostosis and facial dysmorphism are associated with acanthosis nigricans. PATIENTS AND METHODS: Cutaneous examination of a 9-year-old child presenting bicoronal craniosynostosis revealed acanthosis nigricans of the cervical, axillar, inguinal and popliteal regions which appeared at the age of two. He had a dysmorphic face including a large forehead, hypertelorism, mid-face hypoplasia, prognathism and low-set ears. These clinical anomalies suggested a case of Crouzon syndrome with acantho…

Endoscope-assisted conservative condylectomy combined with orthognathic surgery in the treatment of mandibular condylar osteochondroma.

Mandibular condylar osteochondroma (OC) results in asymmetric prognathism with facial morphologic and functional disturbance. The aim of this study was to explore the feasibility of endoscope-assisted conservative condylectomy combined with simultaneous orthognathic surgery in the treatment of condylar OC. Thirteen patients with OC of the mandibular condyle were enrolled in this study. With the aid of endoscope, condylar OC resection and conservative condylectomy were carried out via intraoral approach. A direct vision of the magnified and illuminated operative field was realized. Simultaneous…

Craniofacial patterning in Klinefelter (47 XXY) adults.

Radiographic cephalometry incorporating a pattern profile analysis was used to compare 26 adult Klinefelter males from Finland with first degree adult relatives, 15 males and 14 females. Compared with female relatives, the 47 XXY males were larger in almost all craniofacial linear dimensions, but were similar in facial shape apart from greater mandibular prognathism. Mandibular dimensions in particular differed between the Klinefelter and unaffected males, the corpus length being larger, the ramus shorter and the gonial angle more obtuse in the 47 XXY group. The prominent facial profile, most …
