Cost Estimation Analysis of Dementia: A Scope Review.

Dementia is a progressive neurodegenerative condition, primarily caused by Alzheimer's disease, predominantly affecting elderly individuals. Under these circumstances, dementia care focuses on assisting patients with daily living, either in nursing facilities or at home. The condition imposes substantial economic burdens on patients, caregivers, and healthcare systems, particularly in Europe and North America, where precise cost assessments are essential. This review examines the economic impact of dementia care by integrating diverse cost estimation sources to evaluate the cost-effectiveness

The impact of disease-related symptoms and palliative care concerns on health-related quality of life in multiple myeloma: a multi-centre study.

BACKGROUND: Multiple myeloma, the second most common haematological cancer, remains incurable. Its incidence is rising due to population ageing. Despite the impact of the disease and its treatment, not much is known on who is most in need of supportive and palliative care. This study aimed to (a) assess symptom severity, palliative care concerns and health-related quality of life (HRQOL) in patients with multiple myeloma, and (b) to determine which factors are associated with a lower quality of life. We further wanted to know (c) whether general symptom level has a stronger influence on HRQOL

Head-to-head comparison of fee-for-service and diagnosis related groups in two tertiary referral hospitals in Switzerland: an observational study.

BACKGROUND: Reimbursement for inpatients in Switzerland differed among states until 2012. Some hospitals used diagnosis related groups (DRG) and others used fee-for-service (FFS). We compared length of hospital stay (LOS), patient satisfaction and quality of life between the two systems before a nation-wide implementation of DRG. METHODS: In a prospective, two-centre observational cohort study, we identified all patients with a main diagnosis of either community-acquired pneumonia, exacerbation of chronic pulmonary obstructive disease, acute heart failure or hip fracture from January to June

The third wheel or the game changer? How AI could team up with neurologists in Parkinson's care.

INTRODUCTION: Parkinson's disease (PD) is a progressive neurodegenerative disorder marked by diverse motor and non-motor symptoms. AI appears to be the elephant in the neurologist's room, albeit offering transformative potential in early diagnosis, personalized care, and treatment optimization, addressing the complexities of PD management. METHODS: A PubMed review analyzed AI's role in PD care, focusing on multimodal data, clinician-in-the-loop models, and validation strategies to enhance clinical applicability while addressing ethical concerns. RESULTS: Thirty-nine studies met inclusion cri

Multidisciplinary approach to lymphedema diagnosis and management.

Lymphedema is a chronic, progressive condition characterized by the accumulation of protein-rich interstitial fluid due to impaired lymphatic transport. It significantly impairs quality of life and presents complex diagnostic and therapeutic challenges. Despite its prevalence, lymphedema remains underdiagnosed and undertreated, in part due to limited provider education and a lack of access to coordinated care. For this reason, a multidisciplinary approach to lymphedema management is crucial. Early diagnosis is critical and requires collaboration across primary care, oncology, vascular medicine

Coordination of Care Among Physical Therapists and Neurologists in Huntington Disease.

This study presents a framework for physical therapy through the course of Huntington disease (HD) which includes coordinated care plans with neurologists. HD is an inherited neurodegenerative disorder that leads to impaired strength and coordination and ultimately progressive loss of function. Interdisciplinary HD care teams provide patient-centered, comprehensive evaluations and make recommendations for pharmacologic, healthcare, and lifestyle interventions based on best available evidence. Physical therapists work to improve movement and mobility using specific therapeutic interventions and

Requested withdrawal of gastrostomy feeding in motor neurone disease.

NICE (National Institute for Health & Care Excellence) guidance recommends that healthcare professionals with expertise in palliative care should be an integral part of the multidisciplinary team in managing patients with motor neuron disease (MND). Those in the poorest prognostic group may benefit from early referral to help manage rapidly progressive symptoms, psychological distress and offer additional support with complex decision-making and early robust advance care planning. Patients frequently develop dysphagia and gastrostomy feeding can be used to prolong survival and improve quality

Stiff-Person Syndrome with Anti-Amphiphysin Antibodies: A Paraneoplastic Phenomenon Revealing Breast Cancer in a 77-Year-Old Male.

INTRODUCTION: Stiff-person syndrome is a neurological disorder characterized by painful muscle cramps and progressive muscle spasticity. In patients with positive anti-amphiphysin antibodies, there is an association with malignancies such as breast carcinomas and small cell lung cancers. CASE PRESENTATION: We report the case of a 77-year-old male patient who developed symptoms of anti-amphiphysin positive stiff-person syndrome and was diagnosed with breast cancer. After immunomodulatory and curative-intent oncological treatment, the patient experienced marked improvement in his neurological s

Drug reaction with eosinophilia and systemic symptoms (DRESS)with anti-tuberculosis drugs, a rare and serious complication.

Drug hypersensitivity syndrome or DRESS (drug reaction with eosinophilia and systemic symptoms) is a severe reaction with an estimated mortality of 10%. Antibacillary drugs are rarely incriminated. A 28-year-old patient with tubercular miliaria who developed antibacillary-induced DRESS is presented. The dermatological lesions appeared four weeks after the beginning of the antitubercular treatment. The diagnosis of DRESS was made when all the Registry of Severe Toxidermia (RegiSCAR) criteria were present. The treatment was stopped and the patient was put on symptomatic treatment under supervisi

Brain abscess caused by dental peri-implantitis.

We report the case of a patient presenting with severe headache and progressive aphasia due to a large brain abscess. End-stage peri-implantitis on a dental implant in the upper jaw with contact to the maxillary sinus floor causing maxillary and frontal sinusitis could be identified as the source of infection. Multidisciplinary treatment included broad-spectrum antibiotics, craniotomy for abscess drainage, sinus surgery, and the removal of the implant. Despite all the care taken, the patient died, underlining the importance of oral hygiene and a frequent recall in patients with dental implants

Efficacy and Safety of Chemotherapy Following Anti-PD-1 Antibody Therapy for Gastric Cancer: A Case of Sclerosing Cholangitis.

Immunotherapy targeting programmed cell death-1 (PD-1) signaling is becoming the standard of care for advanced gastric cancer. We herein report a patient with gastric adenocarcinoma with peritoneal dissemination who was treated with nab-paclitaxel and ramucirumab following nivolumab and developed sclerosing cholangitis. Endoscopic retrograde cholangiography showed irregular narrowing and widening of the entire intrahepatic biliary system. Intriguingly, the patient receiving second-line chemotherapy with nab-paclitaxel plus ramucirumab prior to being administered nivolumab, however, he had expe

Pyoderma gangraenosum as rare complication of incisional hernia repair in a patient with Werlhof's disease.

On the 3rd day following surgery to repair an incisional hernia, a 67-year-old male patient with Werlhof's disease (idiopathic thrombocytopenic purpura) was diagnosed with a histologically confirmed pyoderma gangraenosum (PG), a rare complication of wound healing. Dexamethasone pulse therapy resulted in rapid remission of the skin lesions. Further improvement was slowed when the patient suffered multiple organ failure in the intensive care unit, delaying his transfer to rehabilitation for 8 weeks. Postoperative PG is always a differential diagnostic possibility in patients with sterile, progre

[Femoropopliteal arterial thrombosis in a young patient with heterozygous V Leiden factor mutation. Case report].

Activated C protein resistance is a common coagulation defect caused by factor V Leiden mutation and is associated with an augmented risk of predominantly venous thrombosis. Augmented tendency to arterial thrombosis is sporadically reported. This case report describes femoropopliteal thrombosis in a young patient with heterozygous V Leiden factor mutation. Progressive thrombotic occlusion required amputation of the forefoot which resulted in stump dehiscence. Poor blood supply to the perilesional substrate delayed wound healing. An optimal though not yet definitive result was achieved after mo

Outcomes in patients with IBD stratified by risk of disease progression.

OBJECTIVES: Risk stratification of patients with Crohn disease (CD) and ulcerative colitis (UC) may improve outcomes and health care resource utilization (HCRU). We characterized patients with CD or UC as being at high or low risk of disease progression and estimated rates of CD-related and UC-related HCRU. STUDY DESIGN: This retrospective study used claims data from a US health care payer database from January 1, 2017, to December 31, 2019. METHODS: Included patients were fully insured, were 18 years or older, had a diagnosis of CD or UC, and were naive to biologic treatment. Patients were

Evaluation of Hidradenitis Suppurativa Disease Course During Pregnancy and Postpartum.

IMPORTANCE: Hidradenitis suppurativa (HS) disproportionately affects women of childbearing potential. There is a paucity of data regarding the HS disease course during pregnancy and in the postpartum period. OBJECTIVE: To explore the HS disease course during pregnancy and in the postpartum period. DESIGN, SETTING, AND PARTICIPANTS: A retrospective cohort study was conducted on patients in the Henry Ford Health System, Detroit, Michigan-a large, academic, urban referral center. Women with a diagnosis of HS who became pregnant between January 1, 2008, and December 31, 2018, were included. Inte

Contraception, Venous Thromboembolism, and Inflammatory Bowel Disease: What Clinicians (and Patients) Should Know.

The peak incidence of the inflammatory bowel diseases (IBDs) is between the second and fourth decades of life, which coincides with prime reproductive years. Unplanned or mistimed pregnancies may account for nearly half of all pregnancies and are associated with adverse consequences such as a higher risk of delayed preconceptual care, increased risk of preterm birth, low birth weight, and adverse maternal and neonatal outcomes. Increased IBD activity during pregnancy is also associated with adverse pregnancy-related outcomes, such as miscarriage, intrauterine growth retardation, and preterm bi

The natural history of the multicystic dysplastic kidney--is limited follow-up warranted?

OBJECTIVE: Imaging of patients with multicystic dysplastic kidney (MCDK) has increased over the past three decades. This increased use of imaging has provided additional insights into the natural history of MCDK. The present study looked at this data for predictors of involution and associated anomalies. METHODS AND MATERIALS: Institutional review board approval was obtained for this retrospective study. The University of Michigan Departments of Urology and Radiology records were searched to identify unilateral MCDK patients during 1980-2012. Available clinical, radiological and surgical reco
