Most of psoriatic patients require topical therapy. Dermal application of drugs may be the unique treatment; but it can also combined with oral drugs, or phototherapy. Patients are treated at home or in day-care centers; some require hospitalization. Topical corticosteroids are widely used: abuses are frequently observed, therefore skin and systemic side-effects may occur to varying degrees. Tars are still useful. Generalized pustular psoriasis in siblings, 31-year-old male and 26-year-old female, is presented. In both cases, pustular lesions appeared in childhood and typical eruption of psoriasis vulgaris was not observed during their clinical course. Cholecystitis and chronic tonsillitis of the brother and periodontitis and chronic tonsillitis of the sister were considered to be possible provocative factors. HLA-A24, Bw52-, DR2, as the common HLA haplotype in our cases, was estimated.

Thirteen patients with severe persistent psoriasis, intolerant of, or unresponsive to, other current treatments have been treated with cyclosporin (Cys) for periods varying from 12-25 (mean 18) months. The dose ranged from 1-4 mg/kg/day (mean 2.8 mg). There was a 72% reduction in the mean PASI score at 4 weeks, and at the end of the study, an 81% reduction. The purpose of this study was to assess the prophylactic effect of oral photochemotherapy with psoralens and UVA (PUVA) on patients with light-sensitive psoriasis. Of fifteen patients with photosensitive psoriasis, ten with a history of polymorphous light eruption (PMLE) slowly developing into psoriasis were treated with trimethylpsoralen (TMP) and UVA. In this study of photosensitivity among psoriasis patients, a questionnaire was mailed to 2000 psoriasis patients living within the service area of Karolinska Hospital, Stockholm. Of the 88% patients who replied, 4.4% stated that their psoriasis always worsened after sun exposure, and 7.3% stated that this was sometimes so. After telephone interviews with an 80% sample of these allegedly photosensitive patients, half of them were considered nonphotosensitive. Generalized pustular psoriasis is a potentially lethal variant of psoriasis vulgaris characterized by the appearance of sterile pustules on inflamed psoriatic skin. We now report the finding of an absolute lymphopenia at the onset of pustular psoriasis. While the white blood cell count rises to a level of 25,000 to 40,000 in the initial stages of a pustular flare, the lymphocyte count drops from a normal level to an absolute lymphopenia. The effect of ultraviolet-B light provided by TL-12 fluorescent lamps was investigated in 45 patients with psoriasis vulgaris. 33 (73.3%) of the 45 patients showed considerable and rapid improvement during UV-B therapy. The initial (clearing) phase (4 irradiations a week) required an average of 23 irradiations (range: 9-33). During maintenance treatment, irradiation once every 1-5 weeks proved adequate in 64.4% of the patients.

40 unrelated patients with Psoriasis vulgaris were studied for their HLA-A, B and DRw antigen phenotypes. All underwent a skin biopsy to confirm diagnosis. Like most of the authors, we observed an increase in B13 and B17 antigens (17,50 and 25%) whereas in the healthy population the percentage was (5 and 6,02%) respectively. The strong increase in DRw7 suggests that psoriasis vulgaris was associated with DRw7 antigens, with an unbalanced linkage between the B13 and DRw7 antigens and between the B17 and DRw7 antigens.
