Modafinil for excessive sleepiness associated with shift-work sleep disorder.

BACKGROUND: Patients with shift-work sleep disorder chronically have excessive sleepiness during night work and insomnia when attempting to sleep during the day. We evaluated the use of modafinil for treating sleepiness in patients with this disorder. METHODS: In a three-month, double-blind trial, we randomly assigned 209 patients with shift-work sleep disorder to receive either 200 mg of modafinil or placebo before the start of each shift. Assessments were performed with the use of the nighttime Multiple Sleep Latency Test, the Clinical Global Impression of Change, the Psychomotor Vigilance

Negative Body Experience in a Clinical Sample of Mental Disorders: Associations With Posttraumatic Stress Disorder, Mood Disorders, and Personality Disorders.

While body experience is a key focus of treatment in psychomotor therapy (PMT), research has largely overlooked this important topic. In this study we explored three domains of body experience-body satisfaction, body attitude, and interoceptive awareness-in individuals with mental disorders in a clinical sample receiving PMT. We expected these patients to have a more negative body experience than nonclinical controls, with PTSD, mood disorders, personality disorders, and sex negatively affecting all three domains of body experience. The study involved 235 participants aged 18-59 with various m

Neurodevelopmental disorders in childhood-onset hereditary spastic paraplegia type 7: a case series and review of literature.

BACKGROUND: Hereditary spastic paraplegia (HSP) genes are emerging as new causes of neurodevelopmental disorders (NDDs). While the association of intellectual disability and autism spectrum disorder (ASD) with many complex forms of HSP is well established, little is known about a possible association with childhood-onset SPG7. AIM: To add new data on the phenotypic spectrum and associated NDDs in childhood-onset HSP. RESULTS: We report three patients with biallelic variants in SPG7. Consistent with previous reports, childhood-onset SPG7 manifests as a complex HSP phenotype, with clinical fea

Critical slowing down as an early warning of transitions in episodes of bipolar disorder: A simulation study based on a computational model of circadian activity rhythms.

Bipolar disorder is characterized by repeated episodes of mania and depression, and can be understood as pathological complex system behaviour involving cognitive, affective and psychomotor disturbance. Accurate prediction of episode transitions in the long-term pattern of mood changes in bipolar disorder could improve the management of the disorder by providing an objective early warning of relapse. In particular, circadian activity changes measured via actigraphy may contain clinically relevant signals of imminent systemic dysregulation. In this study, we propose a mathematical index to inve

The Use of β-Adrenergic Receptor Antagonists in Psychiatry: A Review.

BACKGROUND: β-Blockers (BBs) are a widely used class of medications with a number of medical indications. Their ability to suppress the adrenergic response has made them a target of investigation for treating a variety of psychiatric symptoms. OBJECTIVE: The aim of this review is to identify evidence regarding the use of BBs for various psychiatric disorders including anxiety disorders, mood disorders, psychosis, and posttraumatic stress disorder. We also review the evidence for use for aggression and agitation and for extrapyramidal side effects of psychiatric medications. METHODS: A search

Behavioral and psychiatric symptoms in prion disease.

The prion diseases are rare neurodegenerative conditions that cause complex and highly variable neuropsychiatric syndromes, often with remarkably rapid progression. Prominent behavioral and psychiatric symptoms have been recognized since these diseases were first described. While research on such symptoms in common dementias has led to major changes in the way these symptoms are managed, evidence to guide the care of patients with prion disease is scarce. The authors review the published research and draw on more than 10 years' experience at the U.K. National Prion Clinic, including two large

Leigh and Leigh-like syndrome in children and adults.

Leigh syndrome (also termed subacute, necrotizing encephalopathy) is a devastating neurodegenerative disorder, characterized by almost identical brain changes, e.g., focal, bilaterally symmetric lesions, particularly in the basal ganglia, thalamus, and brainstem, but with considerable clinical and genetic heterogeneity. Clinically, Leigh syndrome is characterized by a wide variety of abnormalities, from severe neurologic problems to a near absence of abnormalities. Most frequently the central nervous system is affected, with psychomotor retardation, seizures, nystagmus, ophthalmoparesis, optic

[Psychomotor activity and diagnosis in geriatric psychiatry].

The question of the relationship between motor behaviour and psychopathology has been of concern to clinicians for many years. This question appears in the history of diseases as different from each other as general paralysis, hysteria, schizophrenia and mental retardation. The questions raised were the source of the development of a symptomatology of motor behaviours--the domain of which is not muscular potential or praxic abilities (action capacities) as much as their actual use in observed action (action "realization"). This symptomatology implies an interpersonal dimension, which translate

Partial Trisomy of Chromosome 8q and Partial Monosomy of Chromosome 6p with Robinow Syndrome-Like Phenotype.

Genetic disorders can be monogenic or chromosomal. Deletions, duplications, and cryptic imbalances due to rearrangements of the telomeres are seen in a number of patients with psychomotor and language delay. Here, the authors report a case of 1-y-old boy born to nonconsanguineous couple who was evaluated for global developmental delay with phenotypic resemblance to a monogenic disorder namely Robinow syndrome. Cytogenetic microarray showed a double segment imbalance involving chromosome 6p25.3p25.2 and chromosome 8q23.3q24.3. Robinow syndrome also known as fetal face syndrome is a rare disorde

A Heidenhain variant of Creutzfeldt-Jakob disease: forensic implication.

To investigate whether typical clinical, diagnostic and neuropathological findings can be identified in a patient with a postmortem diagnosis of a Heidenhain variant of Creutzfeldt-Jakob disease (CJD). We report a new case of CJD in a rare variant. A man admitted to hospital with cefalea and vision disorder. Clinical and neurological examination showed headache, vision reduction, psychomotor anxiety and progressive torpor. The patient died 4 h after admission to hospital. The autopsy findings included marked encephalic vascular congestion. Hystoneurology examination showed no macroscopic anoma

"Sundowning" and other temporally associated agitation states in dementia patients.

The behavioral and neuropsychiatric symptoms of dementia and Alzheimer's disease have become an increasingly important focus of clinical research. These symptoms also pose a tremendous challenge to families and caregivers. The late afternoon/evening exacerbation of behavioral symptoms in dementia has been recognized by clinicians for >60 years. Researchers have utilized a variety of increasingly sophisticated tools to examine the circadian, hormonal, physiological, and epidemiological correlations with sundowning behavior. Although treatment remains largely empirical, an improved understanding

Dementia.

Alzheimer's disease is the most common cause of dementia, but often several other conditions causing dementia are present on brain autopsies. Palliative care medical issues are similar in all late stage progressive degenerative dementias and include; cardiopulmonary resuscitation, transfer to acute care setting, antibiotic therapy, and tube feeding. Behavioral symptoms of dementia include agitation and resistiveness to care. Quality of life of individuals suffering from dementia is enhanced by availability of meaningful activities and by avoidance of restraints. Family support and involvement

Symptom profiles at baseline are associated with remission in major depression: a confirmatory analysis of the PANDORA trial.

OBJECTIVE: To test whether baseline depressive symptom profiles are associated with remission in Major Depressive Disorder (MDD) and to replicate previous findings from GSRD and STAR∗D in the PANDORA trial. METHODS: We performed a secondary analysis of 215 adults with MDD enrolled in PANDORA and starting a new antidepressant. Baseline severity was assessed with the 17-item Hamilton Depression Rating Scale (HAM-D). Remission at week 12 was defined as HAM-D ≤ 7. Logistic regression models examined associations between baseline HAM-D total and item scores and remission overall and by sex, adjust

Compound Heterozygous ROBO3 Mutation in Two Siblings Presenting with Horizontal Gaze Palsy without Scoliosis: Case-Based Review.

Horizontal gaze palsy with progressive scoliosis (HGPPS) is a rare, autosomal recessively inherited disorder characterized by a congenital absence of conjugated horizontal eye movements with progressive scoliosis developing in childhood and adolescence. HGPPS is caused by mutations of the ROBO3 gene that disrupts the midline crossing of the descending corticospinal and ascending lemniscal sensory tracts in the medulla. We present two siblings, 5-year-old and 2-year-old boys with HGPPS, from non-consanguineous parents. The older brother was brought for the evaluation of moderate psychomotor ret

Schizotypal traits, neurocognition, and paternal age in unaffected first degree relatives of patients with familial or sporadic schizophrenia.

Studies comparing cognitive processes between familial and sporadic schizophrenia have yielded inconsistent findings. In this study we examined differences in neurocognition and schizotypal traits in unaffected relatives of schizophrenia-spectrum patients with either the familial (multiplex) or the sporadic (simplex) subtype of the disorder, taking paternal age at birth into consideration. Simplex (n = 65; SR), multiplex (n = 35; MR) relatives and controls (n = 114) were compared on several cognitive functions and schizotypal traits; between-group differences were evaluated with and without in
