Quantitative evaluation of shear stiffness and the apparent diffusion coefficient using anthropomorphic multi-stiffness liver phantoms under static and motion conditions.

PURPOSE: To evaluate diffusion-weighted imaging (DWI) and MR Elastography (MRE) performance over a clinically relevant stiffness range for liver fibrosis with and without the presence of compressive motion using an in vitro phantom pipeline. METHODS: Five anthropomorphic liver phantoms were created using polyacrylamide (PAA) hydrogels. Phantom stiffness ranged from approximately 1.5-11 kPa. Phantoms were connected to a pulsatile flow circuit to induce compressive motion. Imaging was performed on a 3T MRI system. MRE, conventional DWI, and M1-motion optimized DWI (MODI) were assessed at varyin…

[Ocular blood flow in patients with asymmetric glaucoma].

BACKGROUND: Hemodynamic factors are gaining increasing importance in the pathogenesis of glaucoma. This study was designed to investigate ocular blood flow in patients with asymmetric glaucoma. METHODS: The more affected and the contra-lateral eyes of 15 glaucoma patients with asymmetric scotomas (difference in mean deviation >/=8 dB) despite equal IOP were compared with reference to IOP, optic nerve head excavation, mean deviation in scotomas and ocular blood flow (OBF). Exclusion criteria were defined as follows: operated eyes, secondary glaucomas, differences in IOP and in antiglaucoma the

Renal blood flow detection with Doppler ultrasonography in patients with hepatic cirrhosis.

BACKGROUND: Hepatorenal syndrome, a well-recognized complication of established liver disease, is characterized by early renal vasoconstriction before clinically recognized renal disease. Renal vasoconstriction causes increased renal vascular resistance, which can be detected noninvasively by Doppler ultrasonography. OBJECTIVE: To detect early renal hemodynamic changes in patients with hepatic cirrhosis who had clinically normal renal functions. PATIENTS: Twenty patients with hepatic cirrhosis and ascites, 11 patients with hepatic cirrhosis without ascites, and 23 healthy control subjects. A

Time-Resolved Dynamic Optical Coherence Tomography for Retinal Blood Flow Analysis.

PURPOSE: Optical coherence tomography (OCT) representations in clinical practice are static and do not allow for a dynamic visualization and quantification of blood flow. This study aims to present a method to analyze retinal blood flow dynamics using time-resolved structural OCT. METHODS: We developed novel imaging protocols to acquire video-rate time-resolved OCT B-scans (1024 × 496 pixels, 10 degrees field of view) at four different sensor integration times (integration time of 44.8 µs at a nominal A-scan rate of 20 kHz, 22.4 µs at 40 kHz, 11.2 µs at 85 kHz, and 7.24 µs at 125 kHz). Maturation of arteriovenous fistulas in patients with ventricular assist devices.

OBJECTIVE: Postoperative renal dysfunction necessitating hemodialysis after implantation of ventricular assist devices presents a challenge with respect to establishment of hemodialysis access. Lack of pulsatile flow has led to concerns that arteriovenous fistulas will not mature. This study aims to evaluate arteriovenous fistula as a method of hemodialysis. METHODS: Consecutive patients who underwent implantation of a ventricular assist device between 1988 and 2016 with a subsequent need for hemodialysis were identified. Retrospective data were collected for patients requiring hemodialysis t…

Echocardiographic assessment of ductal significance: retrospective comparison of two methods.

BACKGROUND: Patent ductus arteriosus (PDA) in preterm infants is often assessed with echocardiographic parameters, especially colour Doppler ductal diameter and pulsed Doppler flow pattern. Clinical algorithms have been proposed in which PDA treatment is indicated by either large diameter or a particular flow pattern, however it is unknown whether ductal diameter and flow pattern provide equivalent stratification of infants. AIM: Retrospectively assess both parameters in 197 echocardiograms from 104 infants (gestational age <31 weeks). METHODS: Echocardiograms were independently reviewed and

Transvaginal color Doppler and CA125 as tools in the differential diagnosis of postmenopausal ovarian masses.

OBJECTIVE: To evaluate whether transvaginal color Doppler and CA125 are valid in differentiating malignant from benign postmenopausal ovarian masses. METHODS: Sixty-two women with ovarian masses were studied with transvaginal color Doppler ultrasound before surgery. Serum CA125 levels were also measured. Resistance index (RI) and pulsatility index (PI) were calculated from the waveforms generated from blood flow within the ovary. RESULTS: Benign tumors had a significantly higher pulsatility index (mean 1.23 +/- 1.02; range 0.65-2.99) and resistive index (mean 0.78 +/- 0.22; range 0.5-1.1) th

Microcirculatory Flux and Pulsatility in Arterial Leg Ulcers is Increased by Intermittent Neuromuscular Electrostimulation of the Common Peroneal Nerve.

BACKGROUND: Neuromuscular electrical stimulator (NMES) devices increase blood flow to the lower limb by a process of intermittent muscular contraction initiated by a transdermal stimulus to the common peroneal nerve. However, its effects on localized microvascular blood supply to lower limb wounds are unknown. This study is a single-center open label study measuring the effect of neuromuscular stimulation of the common peroneal nerve on the microvascular blood flow within the wound bed of arterial leg ulcers. METHODS: Eights patients with ischemic lower limb wounds had an NMES (geko™) applied

Racialized economic segregation and disparities in non-small cell lung cancer care and outcomes.

BACKGROUND AND PURPOSE: Idiopathic normal pressure hydrocephalus (iNPH) is a cerebrospinal fluid (CSF) dynamics disorder as evidenced by the delayed ascent of radiotracers over the cerebral convexity on radionuclide cisternography. However, the exact mechanism causing this disruption remains unclear. Elucidating the pathophysiology of iNPH is crucial, as it is a treatable cause of dementia. Improving the diagnosis and treatment prognosis rely on the better understanding of this disease. In this study, we calculated the pulsatile transmantle pressure and investigated the phase lag between this

Interaction of cardiovascular disease and neurodegeneration: transcranial Doppler ultrasonography and Alzheimer's disease.

OBJECTIVE: Recent post-mortem studies have reported that the severity of atheromatous deposits in the circle of Willis is significantly greater, relative to non-demented (ND) elderly persons, in subjects with neuropathologically diagnosed Alzheimer's disease (AD). Additionally, the severity of intracranial atherosclerosis correlates significantly with the densities of neuritic plaques and neurofibrillary tangles. In this study, we examine the arteries of the circle of Willis by transcranial Doppler (TCD) ultrasonography. METHODS: TCD was used to measure, in 25 AD patients and 30 ND elderly su

Silencing KRIT1 Partially Reverses the Effects of Disturbed Flow on the Endothelial Cell Transcriptome.

BACKGROUND: Endothelial cells respond to forces generated by laminar blood flow with changes in vasodilation, anticoagulant, fibrinolytic, or anti-inflammatory functions which preserve vessel patency. These responses to flow sheer stress are primarily mediated by the modulation of transcription factors Krüppel-like factors 2 and 4 (KLF2 and KLF4). Notably, disturbed flow patterns, which are found in vascular areas predisposed to atherosclerosis, significantly reduce the endothelial expression of KLF2 and KLF4, resulting in changes in the transcriptome that exacerbate inflammation and thrombosi

Effects of Pulsatile and Non-Pulsatile Cardiopulmonary Bypass on Early Inflammatory Response After Cardiac Surgery: A Secondary Biomarker Analysis of a Multicentric Randomized Controlled Trial.

Background/Objectives: Cardiopulmonary bypass (CPB) induces a systemic inflammatory response that may contribute to postoperative organ dysfunction. However, the effects of pulsatile and non-pulsatile CPB on the inflammatory response remain incompletely understood. This study investigated the perioperative dynamics of interleukin (IL)-1β, IL-18, and IL-18 binding protein (IL-18BP), as pre-specified secondary inflammatory biomarker analysis of a prospective, multicentric, randomized trial (ISRCTN11243508), in patients undergoing cardiac surgery with pulsatile or non-pulsatile CPB and evaluated

An unusual cause of cardiac failure in a neonate.

We report the case of a newborn admitted with signs of congestive cardiac failure with prominent and pulsatile cervical veins. Echocardiography showed a structurally normal heart, right-to-left ductal flow, and reversed diastolic flow in the proximal descending aorta. A computed-tomography scan of the head showed a vein of Galen arteriovenous malformation. This highlights the importance of considering an intracranial cause in the differential diagnosis of neonatal congestive heart failure.

[Evaluation of blood perfusion velocity in the ophthalmic artery of glaucoma patients].

One hundred and ten examination were performed in 71 patients with primary glaucoma aged 21-81 years (217 eyes). The blood perfusion speed in the ophthalmic artery was measured in each patient by the ultrasonographic pulsating Doppler's method with a probe of a 2.0 MHz frequency by means of a TC-2-64 apparatus of EME production. The ophthalmic artery was examined by transorbital technique according to Spencer and Whisler on a 40-50 mm depth. The examined orbits were divided into 4 groups in dependence of the grade of the optic nerve atrophy. A lowering of over 50 per cent of the blood speed (b

Blood flow in the human cerebral cortex: Large-scale pial vascularization and 1D simulation.

Understanding cerebral circulation is crucial for early diagnosis and patient-oriented therapies for brain conditions. However, blood flow simulations at the organ scale have been limited. This work introduces a framework for modeling extensive vascular networks in the human cerebral cortex and conducting pulsatile blood flow simulations. Using a patient-specific cerebral geometry, we applied a parallelized adaptive constrained constructive optimization algorithm to create a comprehensive pial vascular network in the left hemisphere, starting from the main cerebral arteries. The resulting netw

Assessment of Small Vessel Function Using 7T MRI in Patients With Sporadic Cerebral Small Vessel Disease: The ZOOM@SVDs Study.

BACKGROUND AND OBJECTIVES: Cerebral small vessel disease (cSVD) is a major cause of stroke and dementia, but little is known about disease mechanisms at the level of the small vessels. 7T-MRI allows assessing small vessel function in vivo in different vessel populations. We hypothesized that multiple aspects of small vessel function are altered in patients with cSVD and that these abnormalities relate to disease burden. METHODS: Patients and controls participated in a prospective observational cohort study, the ZOOM@SVDs study. Small vessel function measures on 7T-MRI included perforating art

Prognostic role of pulmonary impedance estimation to predict right ventricular dysfunction in pulmonary hypertension.

BACKGROUND: The effect of pulmonary hypertension (PH) on right ventricular (RV) afterload is commonly defined by elevation of pulmonary artery (PA) pressure or pulmonary vascular resistance (PVR). In humans however, one-third to half of the hydraulic power in the PA is contained in pulsatile components of flow. Pulmonary impedance (Zc) expresses opposition of the PA to pulsatile blood flow. We evaluate pulmonary Zc relationships according to PH classification using a cardiac magnetic resonance (CMR)/right heart catheterization (RHC) method. METHODS: Prospective study of 70 clinically indicate
