Shock index and pulse pressure as triggers for massive transfusion.

BACKGROUND: Hemorrhage is the most common cause of preventable death in trauma patients. These mortalities might be prevented with prehospit … The all-cause mortality was 67% (68/102). The positive predictive value of death for patients with pulse pressure of less than 45 and shock index of greater than 1 was 0.78 for all patients, but was 0.79 and 0.92 for blunt injury and elderly patients, respectively. CONCLUSIONS: Our data demonstrate a high mortality rate in trauma patients who require …

A Pulse Signal Preprocessing Method Based on the Chauvenet Criterion.

Pulse signals are widely used to evaluate the status of the human cardiovascular, respiratory, and circulatory systems. In the process of being collected, the signals are usually interfered by some factors, such as the spike noise and the poor-sensor-contact noise, which have severely affected the accuracy of the subsequent detection models. In recent years, some methods have been applied to processing the above noisy signals, such as dynamic time warping, empirical mode decomposition, autocorrelation, and cross-correlation. Effective as they are, those methods are complex and difficult to imp…

Chronic Kidney Disease and Pulse Wave Velocity: A Narrative Review.

Chronic kidney disease (CKD) is associated with excess cardiovascular mortality, resulting from both traditional and nontraditional, CKD-spe … scular calcification, with significant resultant contribution to the cardiovascular disease burden. Pulse wave velocity (PWV) measured over the carotid-femoral arterial segments is the noninvasive gold-standard technique for measurement of aortic stiffness and has been suggested as a surrogate cardiovascular end-point. A PWV value of 10 m/s or greater h …

Orofacial granulomatosis: an unsuccessful response to weekly azithromycin pulse therapy.

Treatment of orofacial granulomatosis (OFG) is difficult and unpredictable, and currently there is not a treatment with guaranteed success. Macrolides have been suggested to give dramatic improvements in idiopathic OFG; however, this was not our experience. Following on from remarkable findings with weekly azithromycin pulse therapy reported in JAMA Dermatology, 2015, we treated 5 male patients with idiopathic OFG with the same regimen. Case 1 had a slight improvement but stopped treatment after 5 weeks due to gastrointestinal upset. Cases 2 and 3 had an initial improvement; however, symptoms …

Advanced Uses of Pulse Oximetry for Monitoring Mechanically Ventilated Patients.

Pulse oximetry is an undisputable standard of care in clinical monitoring. It combines a spectrometer to detect hypoxemia with a plethysmograph for the diagnosis, monitoring, and follow-up of cardiovascular diseases. These pulse oximetry capabilities are extremely useful for assessing the respiratory and circulatory status and for monitoring of mechanically ventilated patients. On the one hand, the key spectrography-derived function of pulse oximetry is to evaluate a patient's gas exchange that results from a particular ventilatory treatment by continuously and noninvasively measuring arterial…

HIGH DOSE PULSE DEXAMETHASONE THERAPY IN CHRONIC IDIOPATHIC THROMBOCYTOPENIC PURPURA.

We studied the response to pulse high dose dexamethasone (DXM) in 12 consecutive patients of symptomatic chronic idiopathic thrombocytopenic purpura (ITP) who had not responded to the standard forms of therapy. All patients had been exhibited to two or more cycles of prednisolone. Besides this 5 patients had also been given danazol and 4 patients had undergone splenectomy. Six cycles of dexamethasone (40 mg per day for 4 sequential days every 4 weeks) were successfully given in all 12 patients. At the end of the sixth cycle there were 7 (58%) complete responders (CR) [platelet count (PLT)100 ×…

Resistive-pulse and rectification sensing with glass and carbon nanopipettes.

Along with more prevalent solid-state nanopores, glass or quartz nanopipettes have found applications in resistive-pulse and rectification sensing. Their advantages include the ease of fabrication, small physical size and needle-like geometry, rendering them useful for local measurements in small spaces and delivery of nanoparticles/biomolecules. Carbon nanopipettes fabricated by depositing a thin carbon layer on the inner wall of a quartz pipette provide additional means for detecting electroactive species and fine-tuning the current rectification properties. In this paper, we discuss the fun…

Pulse transit time changes in subjects exhibiting sleep disordered breathing.

INTRODUCTION: Pulse Transit Time (PTT) represents a non-invasive marker of sleep fragmentation in OSAS. Little is known regarding PTT in sleepy subjects exhibiting nocturnal Inspiratory Flow Limitation (IFL) in the absence of apneas or desaturation. MATERIALS AND METHODS: The IFL cohort was gender and age matched to subjects with OSAS and a cohort where Sleep Disordered Breathing (SBD)/IFL was absent ("Non Flow Limited" or NFL cohort); PTT Arousal index (PTT Ar) defined by number of PTT arousals per hour. RESULTS: 20 subjects meeting criteria for the IFL cohort were aged and gender matched w…

Internal Pulse Generators in Deep Brain Stimulation: Rechargeable or Not?

OBJECTIVE: Deep brain stimulation (DBS) is a cost-effective strategy for the treatment of different neurologic disorders. However, DBS procedures are associated with high costs of implantation and replacement of the internal pulse generator (IPG). Different manufacturers propose the use of rechargeable IPGs. The objective of this study is to compare the implantation costs of nonrechargeable IPGs versus the estimated costs of rechargeable IPGs in different categories of patients to evaluate if an economic advantage for the health care system could be derived. METHODS: The study looked at 149 p…

Gastrointestinal tract-derived pulse granulomata: clues to an underrecognized pseudotumor.

Pulse granulomata (PG) in the lung and oral pathology literature are presumed due to food (pulse) introduced by mucosal injury. Herein, we report the largest series of PG in the gastrointestinal tract (GIT): 22 resections were prospectively collected from 17 patients (8 men, range=28 to 85 y). All patients had a history of intestinal injury/disease: diverticulitis, fistula, adenocarcinoma, perforation, ulcerative colitis, appendicitis, anastomotic site leak, and/or stent leak. Nine of 22 specimens were designated "masses"; most of these were clinically concerning for neoplasia. Sites of involv…

Pulse wave analysis with two tonometric devices: a comparison study.

Pulse wave analysis is a pivotal tool to estimate central haemodynamic parameters. Available commercial devices use applanation tonometry and have been validated against invasive catheterism. We previously observed differences on a radial second systolic peak (rSPB2) between two commonly used devices: SphygmoCor (AtCor, Australia) and PulsePen (DiaTecne, Italy). The aim of our study was to further quantify differences in radial and carotid signals from the two devices. We measured radial and carotid waveforms in 38 patients with minimal changes between systolic, diastolic blood pressure and hea…

Do systolic BP and pulse pressure relate to ventricular enlargement?

BACKGROUND AND PURPOSE: To evaluate the association between systolic, diastolic and pulse pressure, and increase in ventricular size (VS). Observations in laboratory animals suggest intraventricular pulse pressure (systolic-diastolic) may play a role in ventricular enlargement. METHODS: Initial magnetic resonance (MR) scans and vascular risk factors evaluation were performed in 1812 Atherosclerosis Risk in Communities participants in 1994-1995. In 2004-2006, 1130 participants underwent repeat MR. VS was rated using a validated nine-point scale. Multiple logistic regression analysis assessed a…

Intractable epistaxis and systemic lupus: high-dose intravenous pulse steroids.

IMPORTANCE: Epistaxis is most commonly an easily treated ENT entity with a relatively simple algorithm. Occasionally, however, it is encount … for patients with SLE-related vasculitides-goal-directed medical therapy with high-dose intravenous pulse steroid therapy. CONCLUSIONS AND RELEVANCE: To our knowledge, this report not only is the first description of targeted treatment options for intractable epistaxis in patients with SLE, but also serves to augment the traditional algorithm with the …

Daily dialysis reduces pulse wave velocity in chronic hemodialysis patients.

Pulse wave velocity (PWV) is a predictor of morbidity and mortality in patients with end-stage renal disease (ESRD). Dialysis patients show cyclic changes in PWV related to their hydration status and blood pressure. Our aim is to assess the impact of daily dialysis on PWV. We performed a randomized crossover study of 60 patients who underwent standard hemodialysis (HD) three times per week for at least 6 months. Patients were classified into three groups according to their PWV values before (pre-) and after (post-) HD, with a cutoff value of 12 m s(-1), as follows: the low-low (LL) group had n…

Pulmonary artery pulse wave velocity in idiopathic pulmonary arterial hypertension.

BACKGROUND: Idiopathic pulmonary artery (PA) hypertension (IPAH) is associated with severe PA remodelling. Although the resulting increase in pulse wave velocity (PWV) might be of major pathophysiological relevance, little is known about PA-PWV in IPAH. The aim of this study was to characterize PA-PWV and its predictors in patients with IPAH. METHODS: We studied 26 consecutive patients with incident IPAH aged 55.0 (45.0-66.0) years (62% female) and 10 control subjects without pulmonary hypertension. PA-PWV was measured invasively; PA wall thickness and diameter were assessed using intravascul…

Highly depth-resolved chirped pulse photothermal radar for bone diagnostics.

A novel chirped pulse photothermal (PT) radiometric radar with improved sensitivity over the conventional harmonically modulated thermal-wave radar technique and alternative pulsed laser photothermal radiometry is introduced for the diagnosis of biological samples, especially bones with tissue and skin overlayers. The constraints imposed by the laser safety (maximum permissible exposure) ceiling on pump laser energy and the strong attenuation of thermal-wave signals in tissues significantly limit the photothermally active depth in most biological specimens to a level which is normally insuffic…
