Disentangling feeding practices from socioeconomic risk factors in pyloric stenosis risk.

PURPOSE: Formula feeding, tobacco use, and family history have been well-established as risk factors for hypertrophic pyloric stenosis (HPS). Several markers of socioeconomic status have also been associated with HPS, however, their relationship to other risk factors has not been delineated. The purpose of this study is to investigate the relationship between socioeconomic factors and known risk factors for HPS. METHODS: We identified 735 patients who underwent pyloromyotomy at a high-volume children's hospital between 2012 and 2025. These patients were matched one-to-one by sex, gestational …

Per rectal endoscopic myotomy for the treatment of adult Hirschsprung's disease: First human case (with video).

Hirschsprung's disease (HD) is a congenital disorder characterized by the absence of intrinsic ganglion cells in submucosal and myenteric pl … ird space endoscopy has evolved during the past few years. Per oral endoscopic myotomy and per oral pyloromyotomy are described for treatment of achalasia cardia and refractory gastroparesis, respectively. Using the same philosophy of muscle/sphincter disruption for spastic bowel segments, per rectal endoscopic myotomy could be considered as a treatment option …

Endoscopic pyloroplasty with full-thickness transgastric and transduodenal myotomy with sutured closure.

CONCLUSIONS: Pyloroplasty with full-thickness pyloromyotomy and transverse closure of a linear myotomy was accomplished by using a simple flexible endosurgical technique to test a new flexible suturing system.

Does valsalva retinopathy occur in infants? An initial investigation in infants with vomiting caused by pyloric stenosis.

OBJECTIVE: Retinal hemorrhages (RHs) are 1 manifestation of child abuse, and although they often are considered to be diagnostic of abuse in a young child, there are other potential causes. RHs have been described in association with valsalva maneuver, such as forceful vomiting or coughing. Our aim was to describe the incidence of RH in infants with vomiting caused by pyloric stenosis. METHODS: A prospective, descriptive study was conducted of infants who underwent pyloromyotomy for hypertrophic pyloric stenosis (HPS). Dilated retinal examinations were performed, and the findings were documen…

OBJECTIVE: To establish a machine learning (ML)-based prediction model for readmission within 30 days (early readmission or early readmissio … phageal neoplasms had significantly higher early readmission and patients who received pyloroplasty/pyloromyotomy had significantly lower early readmission. Two ML models to predict early readmission were generated: 1 with 71.7% sensitivity for clinical decision making and the other with 84.8% accuracy and 98.7% specificity for quality review. The impact of pyloric drainage on clinical outcome following esophagectomy: a systematic review.

Delayed emptying of the gastric conduit following esophagectomy can be associated with an increased incidence of complications including asp … e evaluated: no intervention, botulinum toxin (botox) injection, finger fracture, pyloroplasty, and pyloromyotomy. Outcomes evaluated were hospital mortality, anastomotic leak, pulmonary complications, delayed gastric emptying, and the late complication of bile reflux. Should we continue to drain the pylorus in patients undergoing an esophagectomy?

A systematic review of the literature was performed to assess the necessity of a pyloric drainage procedure during an esophagectomy with gastric conduit reconstruction. Earlier data recommend performing a pyloric drainage procedure for all esophagectomies; however, recent studies have questioned this. A thorough literature search (January 2001-November 2011) was performed using the terms esophagectomy, pyloroplasty, pyloromyotomy, botulinum toxin, and pyloric drainage. Only studies that compared patient outcome after undergoing an esophagectomy with a pyloric drainage procedure with those unde…

Management of pediatric patients who have swallowed foreign objects.

The clinical management of children who have swallowed foreign bodies presents a great challenge for both pediatricians and pediatric surgeo … vatively. Surgical intervention was required in two patients; one patient had a previous history of pyloromyotomy, and a magnet tag measuring 1.5 cm in diameter did not pass through the pylorus for 7 weeks. The other patient had swallowed a sewing needle that moved into the descending colon and abdominal pain ensued.

Composition of the refluxed material determines the degree of reflux esophagitis in the dog.

To evaluate the influence of the composition of refluxed material in the pathogenesis of esophagitis, a dog model was used to allow esophageal reflux of gastric secretions, duodenal pancreaticobiliary secretions, and a combination of both. Control dogs had only an esophageal incision. Incompetence of the cardia was established in five other groups by transecting the distal sphincter and creating a hiatal hernia. Two groups received moderate or maximal histamine stimulation of gastric secretion. A gastrojejunostomy, pyloromyotomy, and duodenal closure distal to the papilla were added in the oth…

POEM vs laparoscopic Heller myotomy with fundoplication for achalasia: systematic review and meta-analysis of randomized and prospective studies.

BACKGROUND: Achalasia is definitively treated with peroral endoscopic myotomy (POEM) or laparoscopic Heller myotomy with fundoplication (LHM + PF), but reflux after POEM remains a key concern. We compared the safety and GERD outcomes of POEM and LHM + PF using randomized controlled trials and prospective comparative studies in treatment-naïve adults. METHODS: We searched the MEDLINE, Embase, CENTRAL, Web of Science, and Scopus databases on October 18, 2025. Eligible studies directly compared POEM and LHM + PF in adults. The primary outcome was adverse events, and the secondary outcomes were e

Outcome following pylorus resecting pancreaticoduodenectomy versus classical Whipple's pancreaticoduodenectomy: a randomised controlled trial.

OBJECTIVE: To compare pylorus resecting pancreaticoduodenectomy (PRPD) with classical pancreaticoduodenectomy (classical PD) in terms of short term outcomes. BACKGROUND: There is some evidence that Pylorus resecting PD (PRPD) is associated with lesser incidence of DGE when compared to pylorus preserving PD (PPPD). However, no study has previously compared PRPD with classical PD. METHODS: Patients requiring PD were randomly assigned to either PRPD or classical PD after intraoperative assessment to rule out metastases and unresectable disease. Occurrence of DGE was the primary end point. RESU

Gastric per-oral endoscopic myotomy (G-POEM) for the treatment of gastric sleeve stenosis: a feasibility and safety study.

BACKGROUND: Traditional endoscopic treatments have limited success in gastric sleeve stenosis (GSS). Gastric peroral endoscopic myotomy (G-POEM) could conceivably obliterate the twist/angulation that causes GSS through a tunneled stricturotomy. We report early outcomes of G-POEM for GSS treatment. METHODS: We retrospectively reviewed all patients with GSS treated with G-POEM at our center. The primary end point was clinical success, defined as symptom improvement with resumption of adequate oral intake, without requiring further intervention. RESULTS: 13 patients (mean age 43 ± 10.9 years; 1
