Spontaneous clearance of Helicobacter pylori after pylorus-preserving gastrectomy for gastric cancer.

Residual mucosa in the gastric stump after pylorus-preserving gastrectomy (PPG) is considered a risk factor for the development of gastric stump carcinoma (GSC). Duodenogastric reflux (DGR) and Helicobacter pylori (H. pylori) infection are suspected to contribute to the development of GSC. The aim of this study was to investigate the prevalence of H. pylori in the residual stomach after PPG for gastric cancer and to assess factors associated with the presence of H. pylori. We investigated 72 patients who had undergone PPG at least 1 year prior to the study and were confirmed to be positive for…

Severe duodenal involvement in familial adenomatous polyposis treated by pylorus-preserving pancreaticoduodenectomy.

PURPOSE: Pancreaticoduodenectomy is an alternative to pancreas-sparing duodenectomy for radical treatment of duodenal lesions. The aims of this study were to assess the results of pylorus-preserving pancreaticoduodenectomy (PPPD) for severe duodenal polyposis in familial adenomatous polyposis in terms of morbidity, long-term influence on functional results, the recurrence rate of cancer or jejunal polyps, and survival. METHODS: All patients operated on for a PPPD between 1992 and 2009 were included. Clinical data, endoscopic findings, and pathologic examinations were evaluated. RESULTS: A to…

Rare cause of gastric outlet obstruction: incarcerated pylorus within an inguinal hernia.

We present the case of a 79-year-old man admitted to the emergency room. Having anorexia and vomiting as main complaints, combined with abdominal distension and discomfort, diagnostic examination revealed a giant left inguinal hernia containing the antrum and pylorus of a dilated stomach, creating an outlet obstruction. This was complicated with free peritoneal air, gastric emphysema and air in the portal system due to ischaemia.

Gastroprotective Effect of Rubia cordifolia Linn. on Aspirin Plus Pylorus-Ligated Ulcer.

The present study was designed to investigate the effect of Rubia cordifolia (Rubiaceae) against experimentally induced gastric ulcer and compare activity with its fractions by employing aspirin plus pylorus-ligated ulcer screening model in Wistar rats. Total acidity, volume of gastric acid secretion, total acid output, and pepsin activity show significant reduction, when compared with the control group. The present study confirmed that chloroform fraction showed the significant activity at lower doses compared to parent extract. The mechanism can be attributed to decrease in gastric acid secr…

Severe intestinal bleeding due to sinistral portal hypertension after pylorus-preserving pancreatoduodenectomy.

A rare case of severe intestinal bleeding caused by sinistral portal hypertension after pylorus-preserving pancreatoduodenectomy is reported. Examinations revealed that the developed collateral vessels adjacent to the pancreas and elevated jejunum in the right upper abdomen were the source of the intestinal bleeding, and typical gastric varices were not identified. The present case showed atypical development of collateral vessels due to the postoperative state, which made diagnosis difficult.

Stress takotsubo cardiomyopathy after pylorus preserving pancreaticoduodenectomy for cancer. A case report.

A pylorus-preserving cephalic pancreaticoduodenectomy was performed for tumor removal in a male elderly patient. In the following course no surgical complications occurred but ST-elevations and increase of Troponin T were observed. A coronary angiography revealed no relevant stenosis although a severe hypokinesis of the apex region was determined by echocardiography--a Takotsubo syndrome was assumed. This led to a significant prolonged clinical course. No residual problems remained but the cause of Takotsubo could not be identified. Analyzing the current knowledge, a feasible preoperative scre…

Standard whipple's operation versus pylorus preserving pancreaticoduodenectomy: a randomized controlled trial study.

OBJECTIVE: A single-institution randomized controlled trial was conducted to compare the results of standard whipple operation (SW) with those of pylorus-preserving pancreaticoduodenectomy (PPPD). MATERIAL AND METHOD: Between January 2000 and December 2004, 27 patients with pancreatic or periampullary adenocarcinoma were enrolled into the study. All patients were randomly allocated to either a SW or a PPPD resection. Patients' characteristics, postoperative mortality and morbidity, and survival up to two years were compared. RESULTS: There were no significant differences in baseline characte…

Restorative caecogastroplasty reconstruction after pylorus-preserving near-total gastrectomy: a preliminary study.

BACKGROUND: A wide range of inevitable adverse effects may emerge after total gastrectomy, mainly due to loss of reservoir organ and exclusion of the duodenal route. None of the available reconstructive methods has achieved a satisfactory functional result. A new method has been tried to overcome postgastrectomy problems. METHODS: In this preliminary clinical study, a reconstruction technique involving interposition of the ileocaecal segment as a gastric substitute after pylorus-preserving near-total gastrectomy (restorative caecogastroplasty) was applied in six selected patients with proxima…

A proposal mechanism of early delayed gastric emptying after pylorus preserving pancreatoduodenectomy.

To identify the cause of early delayed gastric emptying after pylorus preserving pancreatoduodenectomy (PPPD), we evaluated patients following PPPD and other upper gastrointestinal surgery. An acetaminophen assay was used to objectively evaluate gastric emptying. We observed transient delayed gastric emptying after PPPD in the early postoperative period. This was recognized not only in patients after PPPD but also in patients after standard pancreatoduodenectomy (STPD). The gastric emptying pattern was different between patients with partial gastrectomy and patients with STPD, despite similar …

Gastric lesion development in normal and pylorus ligated rats after cervical vagotomy.

The main purpose of this study was to further investigate the effects of vagotomy on gastric lesion development. In contrast to the usual subdiaphragmal vagotomy, a different vagotomy at the level of the trigonum caroticum was used both alone and in combination with pylorus ligation (done immediately after vagal transection). The animals were killed 15 min, 30 min, 1 h and 6 h following vagotomy. No damaging effects of sham-vagotomy, or obvious negative effects of cervical vagotomy were noted. Prominent lesions appeared after 1 h in rats subjected to cervical vagotomy and significantly increas…

Inhibitory alpha 2-adrenergic mechanism regulating gastric secretion in pylorus-ligated rats.

Several alpha-adrenoceptor agonists given intracerebroventricularly or subcutaneously to rats were assessed for their effects on gastric secretion under condition of pylorus ligation. Intracerebroventricular injection of alpha-adrenoceptor agonists reduced gastric secretion, in the following order (relative potency, clonidine = 1): oxymetazoline (100) greater than clonidine (1) greater than methoxamine (0.024) greater than phenylephrine (0.003). The antisecretory effects of oxymetazoline and clonidine given intracerebroventricularly were antagonized with yohimbine administered by the same rout…

Pylorus Resecting Pancreatoduodenectomy With or Without Feeding Jejunostomy-A Randomized Controlled Trial.

INTRODUCTION: Postoperative nutrition after pancreatoduodenectomy (PD) remains controversial. Although many centers routinely place a feeding jejunostomy tube (FJT) after PD, it is associated with morbidity. We conducted this study to compare the perioperative outcomes with and without FJT placement post-PD. METHODS: This was an open-label randomized controlled trial, in which the FJT was placed in one arm and the nasojejunal tube (NJT) in the other. All patients with periampullary neoplasm who underwent pylorus resecting PD were included in this study. The primary outcome assessed was clinic…

Incidental Finding of a Fish Bone Perforation in the Pylorus Mimicking Acute Cholecystitis.

We present a case of a 43-year-old man with Crohn's disease who presented with epigastric and right upper quadrant abdominal pain, initially suspected to be acute cholecystitis or a Crohn's flare-up. CT revealed a curvilinear, hyperdense foreign body adjacent to the duodenum, concerning micro-perforation. Endoscopic examination confirmed findings of a 3 cm fish bone lodged in the pylorus. Endoscopic extraction was successful without significant mucosal damage, and the patient recovered well postoperatively. This case highlights the rarity of pyloric perforation secondary to fish bone ingestion…

Descriptive study of a method for identification of the pylorus by ultrasound in lactating Holstein cows.

The purpose of this study was to develop a method for identification of the pylorus, in lactating Holstein cows, based on ultrasound examination and the use of external anatomical landmarks such as the mammary vein (anulus venae subcutaneae abdominis). An ultrasound method based on successive identification of 4 internal anatomical landmarks was developed. The procedure was performed 157 times, on clinically healthy lactating Holstein cows, with a portable ultrasound machine equipped with a 3.5 MHz curvilinear probe. The pylorus was identified with a success rate of 98.7% (155 times out of 157…

[Catastrophic traumatic diaphragmatic hernia with subtotal section of the pylorus: case report].

Traumatic diaphragmatic hernia is an unusual entity in adults that is usually diagnosed late, and is accompanied by high mortality when there is an already existing vascular disorder. The abdominal approach is most frequently used in acute traumatic diaphragmatic hernia and the thoracic approach in chronic traumatic diaphragmatic hernia. We present the clinical case of a female patient who suffered a fall from a height, causing a traumatic diaphragmatic hernia with a subtotal section of the pylorus, leaving the stomach and duodenum practically separated, with a late diagnosis. When the traumat…

Double Pylorus: Report of a Case With Endoscopic Follow-Up and Review of the Literature.

Double pylorus is a rare endoscopic finding that has been reported in 0.001% to 0.4% of upper gastrointestinal endoscopies and can be either congenital or acquired. Acquired double pylorus is usually an uncommon complication of peptic ulcer that erodes and creates a fistula between the duodenal bulb and the prepyloric antrum. We describe a case of a 67-year-old man who experienced mild epigastric pain and dyspepsia over the last 6 months. The patient periodically took nonsteroidal anti-inflammatory drugs (NSAIDs) due to joint pain. Esophagogastroduodenoscopy revealed gastritis and a double pyl…

Preservation of the celiac branch of the vagal nerve for pylorus-preserving gastrectomy: is it meaningful?

BACKGROUND: Preserving the hepatic and pyloric branches of the vagal nerve in laparoscopic pylorus-preserving gastrectomy (LPPG) is considered necessary to maintain the function of the pyloric cuff. However, the clinical benefits of preservation of the celiac branch of the vagal nerve (CBVN) remain unclear. METHODS: Of 391 patients who underwent LPPG for early gastric cancer, 116 patients in whom the CBVN was preserved (CBP group) and 58 patients in whom it was not preserved (non-CBP group) were selected through the propensity score-matching method. To evaluate the surgical and oncological sa…
