An experience using spiral vein graft as arterial substitute.

In two patients wih aneurysmal changes due to arteriosclerosis in the axillary and the subclavian artery respectively, spiral vein grafts were used in a substitute for vascular reconstruction. After resection of the aneurysm, these vein grafts resulted in an excellent adaptation to the recipient vessel. Complicated techniques were not required and the construction time was minimal. Such an approach may be used even in adjacent sites to a joint.

Postoperative management.

Postoperative vascular compromise can result from a problem at the anastomotic site, including vessel damage, improperly placed sutures, trauma to the vessel, or atherosclerosis. Systemic factors include decreased blood flow, vascular spasm, and atherosclerosis. External compression such as hematoma, tight skin closure, or a large drain pressing on a vessel can compromise flow. In patients who are to undergo elective microsurgery, angiography at the recipient site is helpful to select healthy vessels. Anastomosis is performed outside the zone of injury, and a patency test is performed after an

The Effects of Exogenous Administration of Human Coagulation Factors Following Pig-to-Baboon Liver Xenotransplantation.

We sought to determine the effects of exogenous administration of human coagulation factors following pig-to-baboon liver xenotransplantation (LXT) using GalT-KO swine donors. After LXT, baboons received no coagulation factors (historical control, n = 1), bolus administration of a human prothrombin concentrate complex (hPCC; 2.5 mL/kg, n = 2), continuous infusion of hPCC (1.0 mL/h, n = 1) or continuous infusion of human recombinant factor VIIa (1 µg/kg per hour, n = 3). The historical control recipient demonstrated persistent thrombocytopenia despite platelet administration after transplant, a

Increased transfusion-free survival following auxiliary pig liver xenotransplantation.

BACKGROUND: Pig to baboon liver xenotransplantation typically results in severe thrombocytopenia and coagulation disturbances, culminating in death from hemorrhage within 9 days, in spite of continuous transfusions. We studied the contribution of anticoagulant production and clotting pathway deficiencies to fatal bleeding in baboon recipients of porcine livers. METHODS: By transplanting liver xenografts from α1,3-galactosyltransferase gene-knockout (GalT-KO) miniature swine donors into baboons as auxiliary organs, leaving the native liver in place, we provided the full spectrum of primate clo

Symptomatic osteonecrosis in recipients of nonautologous bone marrow transplants.

A retrospective study of 36 allogeneic bone marrow transplant recipients was conducted to determine the rate of occurrence of and risk factors for avascular osteonecrosis. Eight patients developed osteonecrosis, after a mean time interval of 18 months. Multiple sites were often involved (mean 2.37 per patient). Advanced roentgenologic lesions were present at diagnosis in most instances. The occurrence of osteonecrosis was not significantly correlated with the initial hematologic diagnosis, the preparative regimen, serum lipid abnormalities, presence of acute or chronic graft-versus-host diseas

Live birth after robotic-assisted live donor uterus transplantation.

INTRODUCTION: The proof-of-concept of uterus transplantation, as a treatment for absolute uterine factor infertility, came with the first live birth after uterus transplantation, which took place in Sweden in 2014. This was after a live donor procedure, with laparotomy in both donor and recipient. In our second, ongoing trial we introduced a robotic-assisted laparoscopic surgery of the donor to develop minimal invasive surgery for this procedure. Here, we report the surgery and pregnancy behind the first live birth from that trial. MATERIAL AND METHODS: In the present study, within a prospect

Indirect Doppler ultrasound abnormalities of significant portal vein stenosis after liver transplantation.

PURPOSE: To determine indirect Doppler ultrasound (DUS) abnormalities associated with significant portal vein (PV) stenosis (PVS) in recipients of liver transplantation (LT). METHODS: This retrospective study was approved by our institutional review board. Between February 2006 and May 2017, 41 LT recipients were diagnosed with significant PVS, defined as having more than 50% narrowing of PV diameter for any reason, including thrombosis or flow disturbance associated with prominent collateral vessels on portal venography. We reviewed the DUS findings of hepatic arteries (HAs) as well as PVs o

Endothelial activation with prothrombotic response in irradiated microvascular recipient veins.

BACKGROUND: Surgical wounds within previously irradiated tissues are common in reconstructive surgery and subject to an increased incidence of postoperative complications due to vascular dysfunction, including thrombosis in both microvascular anastomosis and the microcirculatory bed. However, there is no study that has described gene expression patterns in radiated human blood vessels. This study aims to determine if radiation can induce changes in gene expression that can promote thrombus formation in human microvascular recipient veins. METHODS: Paired biopsies from radiated recipient veins

A cadaver study in preparation for facial allograft transplantation in humans: part II. Mock facial transplantation.

BACKGROUND: The authors have performed mock facial transplantation by harvesting total facial-scalp flaps from donors and transferring them to recipient cadavers. METHODS: A total of 10 fresh human cadavers were dissected. In the donor, the authors have measured the time of facial-scalp flap harvesting, and the length of the arterial and venous pedicles and sensory nerves that were included in the facial flaps. In the recipient, the authors have evaluated the time of facial skin harvest as a monobloc full-thickness graft, the anchoring regions for the inset of the donor facial flaps, and the

Subclavian to middle cerebral artery saphenous vein bypass graft.

A variation of an extracranial-intracranial arterial bypass is presented. The subclavian artery is used as the donor vessel and the saphenous vein as the graft; thus, a bypass to a cortical branch of the middle cerebral artery can be accomplished. The advantage of this modification is that the saphenous vein, when tunneled subcutaneously behind the ear, is positioned in a straight line from the donor to the recipient vessel. Since the vein lies in the axis of head rotation, turning of the head causes little displacement of the graft, as opposed to a graft from the common carotid artery to the

[Renal allograft transplant vascular complications. Diagnostic and treatment.].

Vascular complications remain common after renal transplantation, occurring in 3% to 15% of patients. These complications can compromise graft function, with graft loss rates ranging from 12.6 to 66.7%.Vascular abnormalities of the graft, specifically the presence of multiple vessels, represent the most frequently studied risk factor for the development of vascular complications. Other risk factors identified for the development of vascular complications are linked to the characteristics of the recipient, or thromboembolic diseasesharing atherosclerosis and/or hypercoagulant state aspathogenic

Microsurgical correction of bilateral facial contour deformities.

Bilateral facial contour deformities are difficult reconstructive problems to correct. This paper summarizes our experience with five adult … s of a limited preauricular incision, and the superficial temporal artery and vein were utilized as recipient vessels. The two remaining patients underwent free-tissue transfer to branches of the external carotid system. There were no flap losses in this series. Follow-up ranged from 1 to 4 years. A Modified Surgical Technique for Kidney Transplantation in Mice.

Kidney transplantation in mice is a complicated and challenging surgery procedure. There are very few publications demonstrating the key steps of this operation. Therefore, this article introduces the technique and points out the surgical caveats associated with this operation. In addition, important modifications in comparison to the conventional procedure are demonstrated. Firstly, a patch of the abdominal aorta is cut and prepared so that the proximal bifurcations of the renal artery, including the ureteral artery are transected together with the donor kidney en bloc. This reduces the risk

Parvovirus B19 microepidemic in renal transplant recipients with thrombotic microangiopathy and allograft vasculitis.

Parvovirus B-19 (B-19) can lead to various clinical scenarios in renal transplant recipients. Here, we report a B-19 microepidemic that occurred between January and March 2007, involving renal transplant recipients from a single center in Tabriz, Iran. We observed 6 patients in whom there was a temporal association between active B-19 infection and thrombotic microangiopathy and intrarenal small and medium-sized vessel vasculitis. Patients typically presented with deteriorating renal allograft function and anemia, and laboratory findings revealed thrombotic microangiopathy. Ultimately, extensi

The extended latissimus dorsi-serratus anterior chimeric local free flap for salvage of the complicated posterolateral thoractomy incision.

BACKGROUND: Composite chest wall reconstruction, following the assurance of chest wall stability, often requires well-vascularized soft tissue coverage with flaps to insure adequate wound healing. Unfortunately, prior surgical approaches such as the posterolateral thoractomy incision or extensive wound breakdown may impede the availability of local or regional choices. A free flap would then be a reasonable option, but in the unstable patient a new donor site is unreasonable. Instead, the otherwise inadequate muscle remnants often transected by the usual thoracotomy incision can be extended by

Activated Clotting Time-Guided Systemic Heparinization as One Component of a Stepwise Salvage Algorithm for Inadequate Internal Mammary Artery Flow in Microsurgical Breast Reconstruction.

BACKGROUND: The internal mammary vessels are commonly used as recipient vessels for free flap breast reconstruction. However, inadequate arterial flow may be encountered intraoperatively. This hypothesis-generating study evaluated the safety and feasibility of activated clotting time (ACT)-guided systemic heparinization as one component of a salvage strategy in such situations. METHODS: We retrospectively reviewed consecutive autologous breast reconstructions using the internal mammary artery (IMA) between 2019 and 2025. Based on intraoperative jet bleeding status and intimal findings, IMA ve…

CT evaluation of the renal donor and recipient.

Proper pre- and post-transplant diagnostic imaging work-up is fundamental in ensuring a successful outcome for renal transplantation. Despite exposure to ionizing radiation, CT has high spatial resolution and is a widely available and fast imaging technique. CT is performed routinely to delineate the anatomy of the kidney, relevant vasculature, and urinary collecting system in the living donor, to assess the iliac vessels in potential recipients prior to surgery, and to assess early and late-term post-transplant complications. The purpose of this article is to outline the optimal CT protocol a
