Biological barriers and mucus interactions in nanoparticle dry powder inhalation: overcoming the lung defense.

Nanoparticle-based dry powder inhaler (DPI) systems offer a promising platform for the treatment of chronic respiratory diseases by enabling localized delivery, controlled release, and reduced systemic side effects. However, their clinical translation is significantly limited by the lung's complex biological defense mechanisms. Following deposition, inhaled nanoparticles encounter sequential barriers, including the airway mucus layer, pulmonary surfactant interface, epithelial tight junctions, and macrophage-mediated immune clearance. These barriers collectively determine nanoparticle diffusio

Air detection of monkeypox virus in a dedicated outpatient clinic room for monkeypox infection diagnosis.

BACKGROUND: The World Health Organization (WHO) reported an outbreak of monkeypox virus (MPXV) in Western countries on May 12th, 2022. In early October, WHO counted 68 900 cases in the world outside Africa. MPXV spreads all around the environment of infected patients through direct contact with lesions, body secretion, or liquids. Interrogations about MPXV spreading through respiratory secretions have been reported but appear bewildering. Thus, we investigated for virus identification in the air around infected patients to move forward with unresolved questions. METHODS: We collected air samp

Self-contained system for mitigation of contaminated aerosol sources of SARS-CoV-2.

Contaminated aerosols and micro droplets are easily generated by infected hosts through sneezing, coughing, speaking and breathing1-3 and harm humans' health and the global economy. While most of the efforts are usually targeted towards protecting individuals from getting infected,4 eliminating transmissions from infection sources is also important to prevent disease transmission. Supportive therapies for Severe Acute Respiratory Syndrome Coronavirus 2 (SARS CoV-2) pneumonia such as oxygen supplementation, nebulizers and non-invasive mechanical ventilation all carry an increased risk for viral

Comparing high-flow nasal cannula oxygen and non-invasive ventilation to standard oxygenation in non-selected intensive care unit patients admitted for acute hypoxaemic respiratory failure: protocol for the KISS (Key oxygenation Interventions in Surgical and non-Surgical patients) adaptive randomised controlled trial.

INTRODUCTION: Hypoxaemic acute respiratory failure (ARF) in intensive care unit (ICU) patients is associated with high mortality. Three main devices are used to provide oxygen to hypoxaemic ARF patients: non-invasive ventilation (NIV), high-flow nasal cannula oxygen (HFNO) and standard oxygen (first-attempt device in usual care). To date, no multicentre randomised controlled study has compared NIV and HFNO to standard oxygen with day 28 mortality rate as primary outcome in hypoxaemic ARF in non-selected patients. Our hypothesis is that NIV and/or HFNO is superior to standard oxygen to reduce d

Clinical presentation and management of severe Ebola virus disease.

Clinicians caring for patients infected with Ebola virus must be familiar not only with screening and infection control measures but also with management of severe disease. By integrating experience from several Ebola epidemics with best practices for managing critical illness, this report focuses on the clinical presentation and management of severely ill infants, children, and adults with Ebola virus disease. Fever, fatigue, vomiting, diarrhea, and anorexia are the most common symptoms of the 2014 West African outbreak. Profound fluid losses from the gastrointestinal tract result in volume d

The Oral Cavity Potentially Serving as a Reservoir for SARS-CoV-2 but Not Necessarily Facilitating the Spread of COVID-19 in Dental Practice.

Intraoral tissues, secretions, and microenvironments may provide severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) with the conditions necessary for viral cellular entry and inhabitation. The aim of the present study is to overview the oral cavity that potentially serves as a reservoir for SARS-CoV-2, and then discuss the possibility that such oral cavity facilitates the spread of coronavirus disease 2019 (COVID-19) in dental practice. Articles were retrieved from PubMed/Medline, LitCovid, ProQuest, Google Scholar, and preprint medRxiv databases. Results of the literature search ind

Evaluation of exposure to nano-sized particles among transport and vehicle service workers.

BACKGROUND: Exposure to fine and ultrafine particles from transport processes is a main consequence of emissions from engines, especially those with self-ignition. The particles released in these processes are a source of occupational and environmental particles exposure. The aim of this study was to assess the fine and nano-sized particles emission degree during work connected with transport and vehicle servicing. MATERIAL AND METHODS: The tests were carried out at 3 workplaces of vehicles service and maintenance (a car repair workshop, a truck service hall, and a bus depot) during 1 work da

Photobiomodulation preserves mitochondrial redox state and is retinoprotective in a rodent model of retinitis pigmentosa.

Photobiomodulation (PBM) by far-red (FR) to near-infrared (NIR) light has been demonstrated to restore the function of damaged mitochondria, increase the production of cytoprotective factors and prevent cell death. Our laboratory has shown that FR PBM improves functional and structural outcomes in animal models of retinal injury and retinal degenerative disease. The current study tested the hypothesis that a brief course of NIR (830 nm) PBM would preserve mitochondrial metabolic state and attenuate photoreceptor loss in a model of retinitis pigmentosa, the P23H transgenic rat. P23H rat pups we

Opportunities for biomaterials to address the challenges of COVID-19.

The coronavirus disease 2019 (COVID-19) pandemic has revealed major shortcomings in our ability to mitigate transmission of infectious viral disease and provide treatment to patients, resulting in a public health crisis. Within months of the first reported case in China, the virus has spread worldwide at an unprecedented rate. COVID-19 illustrates that the biomaterials community was engaged in significant research efforts against bacteria and fungi with relatively little effort devoted to viruses. Accordingly, biomaterials scientists and engineers will have to participate in multidisciplinary

Partial liquid ventilation: Effect of initial dose and redosing strategy in acute lung injury.

OBJECTIVE: Partial liquid ventilation (PLV) with perfluorochemicals has been shown to be effective in treating acute respiratory failure in animal studies and human trials. To determine the influences of perfluorochemicals on initial dose and redosing strategy, we studied their effects on gas exchange, pulmonary mechanics, and lung architecture. DESIGN: After lung injury was induced by repeated warm saline lavages, the animals were instilled endotracheally with different doses of perflubron during 5-10 mins in PLV-treated groups. The animals were randomized to five groups: PLV-12S (12 mL/kg pe

Prospective Study Evaluating a Pain Assessment Tool in a Postoperative Environment: Protocol for Algorithm Testing and Enhancement.

BACKGROUND: Assessment of pain is critical to its optimal treatment. There is a high demand for accurate objective pain assessment for effectively optimizing pain management interventions. However, pain is a multivalent, dynamic, and ambiguous phenomenon that is difficult to quantify, particularly when the patient's ability to communicate is limited. The criterion standard of pain intensity assessment is self-reporting. However, this unidimensional model is disparaged for its oversimplification and limited applicability in several vulnerable patient populations. Researchers have attempted to d
