Long-term omalizumab efficacy in allergic rhinitis.

BACKGROUND: Omalizumab therapy was found to be safe and effective as an add-on therapy for patients with poorly controlled severe asthma. Although several studies over the last decade have demonstrated its efficacy in other Immunoglobulin E related diseases, its use in such conditions is off-label. OBJECTIVE: This study aimed to assess the effectiveness of long-term therapy with Omalizumab in patients with persistent severe allergic rhinitis and inadequately controlled severe asthma. METHODS: Patients with poorly controlled severe asthma and persistent allergic rhinitis were enrolled and tre…

Correlation between Allergic Rhinitis and Laryngopharyngeal Reflux.

BACKGROUND AND OBJECTIVES: Laryngopharyngeal reflux (LPR) exhibits nonspecific clinical presentations, and these symptoms may be associated with other conditions such as allergies, including allergic rhinitis and laryngitis. However, there is a gap in the literature regarding the correlation of laryngopharyngeal reflux with allergic rhinitis/laryngitis. Hence, the aim of this study is to explore the correlation between these two conditions. PATIENTS AND METHODS: A total of 126 patients with suggestive manifestations of laryngopharyngeal reflux were included in this study. Patients were classi…

Cell-mediated non-allergic rhinitis in children.

OBJECTIVE: Non-allergic rhinitis is a heterogeneous disease whose etiology is largely unknown. Nasal cytology only allows us to recognize different non-allergic rhinitis forms on the basis of the prevalent inflammatory cell infiltrate: non-allergic rhinitis with eosinophils, with neutrophils, with mast-cells and with both eosinophils and mast-cells. The aim of this study is to define the incidence, clinical features and comorbidity of the different types of cell-mediated non-allergic rhinitis in a pediatric age group. METHODS: One hundred and fourteen non-allergic children with chronic nasal …

Nonallergic rhinopathy (formerly known as vasomotor rhinitis).

This review focuses on the poorly understood condition of nonallergic rhinopathy (NAR) at a clinical level, with an eye on current optimal treatment. NAR is the new designation for the conditions formerly referred to as vasomotor rhinitis or nonallergic idiopathic rhinitis. The clinical characteristics and differential diagnosis are provided in detail in this review, and the disease should now be characterized sufficiently for clinical studies.

Allergic rhinitis and chronic suppurative otitis media.

Allergic inflammation in upper airways can act as a predisposing factor for infectious ear diseases. There are some evidences about the role of allergic rhinitis in chronic otitis media with effusion, but its role in establishing chronic suppurative otitis media (CSOM) has not been clearly shown. 68 adult patients with established CSOM, who were candidates for ear surgery, and 184 age- and sex-matched controls were evaluated for the presence of allergic rhinitis. Standard questionnaire was filled out for all participants. All patients and controls underwent skin prick test for 28 common region…

Pediatric allergic rhinitis: physical and mental complications.

Allergic rhinitis (AR) affects up to 40% of children in the United States and its prevalence continues to increase. Most AR develops during the pediatric years and it is the most common chronic allergic disorder seen in children. It is important to note that AR is more than just sneezing and a nuisance for the children. There are numerous complications that can lead to significant problems both physically and mentally in the child who suffers with AR. Under physical complications, otitis media with effusion, recurrent and/or chronic sinusitis, asthma, and snoring impact children with AR. Sleep…

Allergic rhinitis: treatment based on patient profiles.

Allergic rhinitis is a common medical condition characterized by nasal, throat, and ocular itching; rhinorrhea; sneezing; nasal congestion; and, less frequently, cough. The treatment of allergic rhinitis should control these symptoms without adversely affecting daily activities or cognitive performance and should prevent sequelae such as asthma exacerbation or sinusitis. This review describes a stepwise approach to treatment of allergic rhinitis derived from a synthesis of clinical trial results, patient preferences, and real-world tolerability data. Key clinical considerations include frequen…

[Nasal polyposis and allergic rhinitis--our experience].

Nasal polyposis is a multifactor disease characterised by chronic eosinophilic inflammation of the nasal and sinal mucosae. Its aetiology is unknown, but it's often associated with other diseases: allergic rhinitis, asthma, and aspirin sensitivity in adult patients. In children, mucoviscidosis is possible. The aim of this paper is to determine the relationship between nasal polyposis and allergic rhinitis, and their link with idiopathic rhinitis. The study involved 100 patients of both sexes. Patients were divided into three groups: group I--with allergic rhinitis (65 patients), group II--with…

[Oral second generation antihistamines in allergic rhinitis].

BACKGROUND: Histamine is a key mediator of the allergic immediate reaction. Antihistamines belong to the most frequently used treatment modalities in allergic rhinitis. METHODS: The National Libraray of Medicine was searched for current data of the effects of histamine and antihistamines in allergic rhinitis. RESULTS: Histamine acts on 4 different histamine receptors. Activation of H1-receptors on nasal trigeminal nerve fibers transmits nasal itch and sneezing. Nasal hypersecretion is mainly mediated by an trigeminal-parasympathetic reflex. Activation of H1-receptors results in contraction o…

Lipid-based Nose Ointment for Allergic Rhinitis.

OBJECTIVE: To assess the efficacy of a lipid-based nose ointment in treating allergic rhinitis. STUDY DESIGN AND SETTING: Prospective, open, randomized, controlled clinical study in the outpatient departments of 2 tertiary care centers. RESULTS: No specific adverse effects were observed. The nose ointment under investigation led to a significant improvement in sneezing and nasal itching (17 patients). There was no significant change for nasal congestion and rhinorrhea in the intention-to-treat sets. However, there was still a significant drop of the mean relative total symptom score derived …

[Pathologic conditions associated with drug-induced rhinitis].

Rhinitis medicamentosa ("nose-drop-nose") is a term used for pathological condition of the nasal mucous membrane that results from long-term abuse with intranasal vasoconstrictors. The aim of this work was to examine what lead the patients with nosedropnose rhinitis to the initial usage of intranasal vasoactive drugs. In this prospective study, 92 patients with rhinitis medicamentosa were included. The evaluation of all study subjects comprised the history, ORL, microbiological and radiological examination, skin prick tests with a battery of routine respiratory and nutritive allergens and nasa…

Allergic rhinitis update: Epidemiology and natural history.

Allergic rhinitis is the most common chronic condition, with an estimated prevalence in the United States of 5-22%, which increases from infancy, peaks in childhood and adolescence, and decreases in the elderly. As a major cause of morbidity, absenteeism, and restricted activity in both children and adults, allergic rhinitis, similar to asthma, appears to be increasing with time. Allergic rhinitis is commonly defined as seasonal or perennial, depending upon whether symptoms are manifested at defined yearly intervals or throughout the year, respectively. While trees, grasses, weeds, and molds a…

Psyllium laxative-induced anaphylaxis, asthma, and rhinitis.

A 69-year-old nurse was evaluated for a recent episode of anaphylaxis that had occurred after psyllium ingestion. She had experienced recurrent rhinitis and asthma related to psyllium exposure for the past 15 years. The diagnosis of psyllium hypersensitivity was established by a positive psyllium puncture skin test, an elevated psyllium-specific IgE level in serum, and a confirmatory soluble-antigen competitive inhibition test. The patient was symptomatic for several years, and this diagnosis was not considered until she suffered potentially life-threatening anaphylaxis. Psyllium hypersensitiv…

Allergic rhinitis and upper respiratory tract infections.

The existence of a relationship between allergic rhinitis and upper respiratory tract infection (URI) has not yet been verified. In a prospective study, 64 patients with allergic rhinitis and 23 non-allergic individuals were followed for 11.4 months. All participants kept a diary in which they recorded symptoms of URI, nasal discharge, blocked nose, sneezing and sinusitis. The allergic individuals did not have more frequent or more long lasting URI or sinusitis more frequently than non-allergic subjects.

Topical nasal sprays: treatment of allergic rhinitis.

Topical nasal sprays, especially steroids, have regained favor as treatment for allergic rhinitis. Nasal steroids are widely used and are as safe and effective as antihistamines in controlling symptoms of rhinitis. However, if improperly used, steroids can have side effects. It is essential that patients learn correct techniques for administering nasal steroids and understand complications that can result from nasal steroid use. New steroid drugs, such as budesonide, tripedane and fluticasone, are being evaluated and will be available in the near future. Other topical drugs, such as cromolyn a…

Oral vasoconstrictors in perennial non-allergic rhinitis.

Nasal airway resistance and nasal symptoms were evaluated in patients with perennial non-allergic rhinitis before and after vasoconstrictory drugs and placebo taken orally. The patients were partly selected; those suffering mainly from nasal obstruction being tested. Phenylpropanolamine (PPA) in a dose of 100 mg in a sustained-release preparation clearly decreased the nasal airway resistance measured rhinomanometrically. PPA in a dose of 50 mg and combined with an antihistamine had no obvious decongesting effect. Placebo and dihydroergotamine (DHE) in a dose of 5 mg did not decrease the nasal …

Vasomotor rhinitis: neglected cause of nasal congestion.

Vasomotor rhinitis is a condition of chronic nasal congestion which is noninfectious and nonallergic. Its cause is thought to be an imbalance of autonomic control to the nasal mucosa. This disorder is a diagnosis of exclusion, and other causes of chronic nasal obstruction must be considered first. Treatment measures include avoidance of nonspecific stimuli which exacerbate symptoms and, for symptomatic relief, use of oral sympathomimetics. Antihistamine-decongestant combinations may be effective in some patients. Topical vasoconstrictors should not be used.
