Delayed hemorrhage following deep brain stimulation device placement in a patient with Parkinson's disease and lupus anticoagulant syndrome: illustrative case.

BACKGROUND: Treatment options for Parkinson's disease (PD) include both medical and surgical approaches. Deep brain stimulation (DBS) is a surgical procedure that aims to improve motor symptomatology. OBSERVATIONS: A 66-year-old White male with a 9-year history of PD presented to the neurosurgery clinic for DBS consideration. On the morning of scheduled surgery, preoperative laboratory test results revealed a prolonged prothrombin time of 50 seconds. Surgery was postponed, and further work-up revealed that the patient had a positive test result for lupus anticoagulant (LA). DBS implantation w

Sleep patterns and obesity in childhood.

PURPOSE OF REVIEW: To highlight the recent findings on sleep-obesity associations in children. We focus on sleep duration, sleep timing and chronotype, and describe the potential mechanisms underlying sleep-obesity associations. RECENT FINDINGS: Poor sleep is increasingly common in children and associations between short sleep duration in early childhood and obesity are consistently found. Less is known about the infancy period, and the findings in adolescents are inconsistent. Sleep timing patterns may also contribute to obesity risk. Variable and shifted sleep schedules and evening chronoty

Environmental circadian disruption alters rhythmicity of clock and epigenetic gene expression in SKH-1 female mouse livers.

Endogenous circadian rhythms are primarily entrained by environmental cues that are used to regulate the function of cells and peripheral tissues throughout the body. We found that disrupted light schedules - reflective of shift work conditions - demonstrated impaired gene expression throughout the 24-h day in core clock and epigenetically regulated genes. Dysregulated expression in these genes has been independently detected in the etiology of sleep-related diseases, suggesting epigenetic regulation as a potential mechanistic link between circadian disruption and health outcomes. Because of t

Cardiovascular responses to orthostasis during a simulated 3-day heatwave.

Global warming has caused an increase in the frequency, duration, and intensity of summer heatwaves (HWs). Prolonged exposure to hot environments and orthostasis may cause conflicting demands of thermoregulation and blood pressure regulation on the vasomotor system, potentially contributing to cardiovascular complications and occupational heat strain. This study assessed cardiovascular and skin blood flow (SkBF) responses to orthostasis before, during and after a 3-day simulated HW. Seven male participants maintained a standard work/rest schedule for nine consecutive days split into three 3-da

Durability of COVID-19 vaccine-induced immunity in Saudi Arabia: evidence, gaps, and implications for Hajj and mass-gathering preparedness.

The policy question for COVID-19 vaccination has shifted from whether vaccines induce protection to how long clinically meaningful protection persists and how immune information should guide preparedness. This question is especially relevant to Saudi Arabia, where Hajj and Umrah create predictable periods of dense population mixing among pilgrims with diverse vaccination histories, prior infection exposures, ages, and comorbidity profiles. This structured narrative review synthesizes evidence on durability of COVID-19 vaccine-induced immunity, Saudi epidemiological and serological data, hetero

Melatonin Alleviates Circadian Rhythm Disruption-Induced Enhanced Luteinizing Hormone Pulse Frequency and Ovarian Dysfunction.

Circadian rhythm disruption (CRD), stemming from sleep disorders and/or shift work, is a risk factor for reproductive dysfunction. CRD has been reported to disturb nocturnal melatonin signaling, which plays a crucial role in female reproduction as a circadian regulator and an antioxidant. The hypothalamic-pituitary-ovarian (HPO) axis regulates female reproduction, with luteinizing hormone (LH) pulse pattern playing a pivotal role in folliculogenesis and steroidogenesis. However, the effect of CRD on the HPO axis and the involvement of melatonin remains unclear. Female CBA/CaJ mice underwent CR

Childcare provision for on-call workers in the NHS: Is the 24/7 service ideal matched by reality?

BACKGROUND: The National Health Service (NHS) in the UK aims to deliver healthcare services around the clock. Major Trauma Centres (MTCs) are crucial in this operation, requiring continual 24/7 operations. Despite efforts to provide on-site nurseries catering to children aged 3 months to 5 years, there remains a significant gap in childcare provision for on-call workers. This shortfall particularly affects surgeons, nurses and military medical personnel whose shifts extend beyond standard nursery operating hours. This discrepancy raises concerns about the sufficiency of support for healthcare

Circadian rhythms in health and disease.

Circadian rhythms, serving as a crucial temporal bridge linking neural activity and pathological biology, are regulated by central pacemaker neurons to maintain cellular homeostasis and coordinate diverse biological processes. Circadian rhythm disruption is closely associated with various diseases, including neurodegenerative disorders, metabolic disorders, and cancer. Circadian rhythms are controlled by core clock genes, such as BMAL1 and CLOCK, modulating numerous physiological processes, encompassing metabolism, immune responses, neural activity, and hormone regulation. Circadian rhythms di

The Role of Melatonin and Chronotherapy in the Treatment of Circadian Rhythm Disruption and Depression.

The circadian rhythm is the body's natural timekeeper, determining when to sleep and wake over a 24-hour cycle. This biological clock can be reset by light and temperature, enabling body functions to align with the day-night cycle. Disruptions to the body's circadian rhythm, such as nontraditional shift work schedules, frequent flying, and people who spend too much time looking at screens, are believed to be behind an increase in sleep problems linked to mental health problems such as depression. Research suggests that a 'misalignment' of the body clock may paradoxically lead to either the ons…

Hepatitis A resurgence: time to reconsider routine pediatric immunization.

The shifting epidemiological landscape of hepatitis A virus (HAV) infection represents a growing concern across Europe, driven by a significant 2025 resurgence and widespread pediatric outbreaks. Although perceived as a mild childhood infection, HAV can cause severe clinical complications, including acute liver failure requiring emergency liver transplantation, and death. Furthermore, the clinical and financial burden is heavily compounded by atypical biphasic or relapsing courses occurring in up to 20% of cases, which inflate healthcare costs through prolonged hospitalizations. Circadian clocks and adaptive immune function: from mechanisms to therapeutic applications.

The circadian clock exerts profound regulatory control over adaptive immunity, particularly affecting T and B lymphocyte development, trafficking, activation, and effector function. These immune cells possess autonomous molecular clocks governed by core genes such as Bmal1, Clock, Per1/2, and Cry1/2, which influence transcriptional programs related to receptor expression, cytokine signaling, and metabolism. Circadian regulation shapes the daily oscillation of lymphocyte recirculation through lymphoid organs, modulates antigen responsiveness, and fine-tunes subset differentiation-most notably s

[Varicella vaccination in Germany. A provisional appraisal in the context of MMR vaccination].

In 2004, a general varicella immunization was introduced in Germany for infants from the age of 11 months, followed by the subsequent recommendation in 2009 of a second vaccine dose. The vaccination is carried out at the same time as the immunization against measles, mumps, and rubella (MMR). Results of the nationwide sentinel surveillance of varicella and herpes zoster implemented by the Varicella Working Group (Arbeitsgemeinschaft Varizellen, AGV) show that the defined goals for varicella immunization (reduction of varicella-related morbidity, complications and hospitalizations) have been re
