BACKGROUND: Reverse total shoulder arthroplasty (rTSA) is widely recognized as an effective treatment for complex proximal humerus fractures (PHFs), providing reliable and predictable outcomes. However, whether glenoid lateralization confers tangible advantages in this setting remains unclear. This study aimed to evaluate the impact of different amounts of glenoid lateralization on clinical and functional outcomes following rTSA for PHFs. To our knowledge, this is the first study to specifically address this question.
METHODS: A total of 132 patients with 3 or 4-part PHFs were treated with rT

INTRODUCTION: The purpose of this study was to utilize a large national database to evaluate short- and long-term outcomes of acute management of proximal humerus fractures (PHF) with reverse total shoulder arthroplasty (RSA) compared to open reduction internal fixation (ORIF).
METHODS: The TriNetX platform was utilized to perform a retrospective review of patients who sustained PHFs treated within three weeks of injury. Patients were identified using Current Procedural Terminology codes and the International Classification of Disease diagnosis codes. The cohorts were 1:1 propensity matched.

BACKGROUND: Proximal humerus fracture is one of the most common fractures in the elderly population. However, in patients with complex fracture patterns, there is still no general consensus in the best treatment method. This study aims to evaluate the outcomes between those treated with reverse total shoulder arthroplasty (rTSA) and open reduction internal fixation (ORIF).
METHODS: All geriatric patients (> 60 years of age) with proximal humerus fractures undergoing surgical treatment were analysed. There were 25 patients treated with rTSA and 75 with ORIF. Propensity score matching was used

UNLABELLED: This article was updated on TK because of a previous error, which was discovered after the preliminary version of the article was posted online. In Table VII, the fracture rate in the study by Walch et al. that had read "4.6% (21 of 457)" now reads "0.9% (4 of 457)."
BACKGROUND: Acromial and scapular fractures after reverse total shoulder arthroplasty (rTSA) are rare and challenging complications, and little information is available in the literature to identify patients who are at risk. This study analyzes risk factors for, and compares the outcomes of patients with and without,

BACKGROUND: Recent literature has shown that acute reverse total shoulder arthroplasty (RTSA) yields good outcomes in the treatment of displaced proximal humeral fractures, and there have also been recent studies showing that delayed RTSA can be successfully used for sequelae of proximal humeral fractures such as nonunion and malunion. The use of meta-analysis affords the opportunity to formally compare the outcomes of acute RTSA for fracture and delayed RTSA for fracture sequelae.
METHODS: We searched the MEDLINE, Embase, and Cochrane Library databases. We included all studies reporting on R

PURPOSE: The main goal of this study was to retrospectively evaluate the clinical, functional and radiographic outcomes of displaced medial epicondyle fractures with or without associated elbow dislocation in children undergoing surgical treatment and to evaluate upper-extremity function with the Quick Disbilities of the Arm, Shoulder and Hand (Quick DASH) questionnaire. The hypothesis was that patients with associated elbow dislocation would have similar functional outcome to patients with displaced fractures with or without intra-articular entrapment of the medial epicondyle.

We report an unusual case of clinical improvement after 2 years from one stage bilateral open reduction of chronic anterior shoulder dislocations with internal fixation of greater tuberosity fractures. The chronic bilateral anterior shoulder dislocations with greater tuberosity fractures were caused by electric shock. After the injury the patient was hospitalized on a medical ward and the dislocations were not diagnosed. Four weeks later the diagnosis of both anterior shoulders dislocations with greater tuberosity fractures was confirmed by X-ray. The patient was treated with one stage bilater

Recurrent anterior shoulder instability with subcritical bone loss is a challenging clinical scenario. Many treatment options have been described, including arthroscopic Bankart repair, arthroscopic Bankart repair plus remplissage, open Bankart repair, and the Latarjet procedure. Arthroscopic Bankart repair alone has higher rates of failure in high-risk patient populations, especially at long-term follow-up. These high-risk populations include patients with significant glenoid bone loss (>20%), bipolar bone loss (including subcritical bone loss and off-track or "near-track" [8-10 mm] Hill-Sach

BACKGROUND: Almost one-third of patients with proximal humeral fractures are treated surgically, and the number is increasing. When surgical treatment is chosen, there is sparse evidence on the optimum method. The DelPhi (Delta prosthesis-PHILOS plate) trial is a clinical trial comparing 2 surgical treatments. Our hypothesis was that reverse total shoulder arthroplasty (TSA) yields better clinical results compared with open reduction and internal fixation (ORIF) using an angular stable plate.
METHODS: The DelPhi trial is a randomized controlled trial comparing reverse TSA with ORIF for displa

BACKGROUND: Reverse shoulder arthroplasty (RSA) is being increasingly used for complex, displaced fractures of the proximal humerus in older patients. Anatomic tuberosity healing in RSA has been recognized to restore better shoulder function. We compared the reported clinical and functional outcomes of RSA in proximal humeral fractures with and without tuberosity healing.
METHODS: We performed a systematic review of literature based on Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines and searched MEDLINE, CINAHL, Embase, and Cochrane Central Register of Controlled

BACKGROUND: Minimally invasive plate osteosynthesis for humeral shaft fractures has been described recently, but there are no randomized studies comparing the clinical results for shoulder function between this technique and locking intramedullary nailing.
METHODS: A prospective randomized study was performed. Forty-one humeral shaft fractures (40 patients) were randomized to be treated with a minimally invasive plate (n = 21) or a locking intramedullary nail (n = 19). Clinical and radiographic outcome assessments were conducted at 1 year postoperatively. Shoulder function was the primary out

BACKGROUND: Conservative management for uncomplicated displaced clavicle fractures is common practice. Delay of surgical fixation may result in less favorable outcomes.
METHODS: A retrospective cohort study was conducted of 60 patients with a closed mid-third clavicle fracture that did not meet current operative or nonoperative guidelines; 20 primary (plate fixation <6 weeks), 20 delayed (plate fixation >6 weeks), and 20 matched conservative patients were included. Each patient completed 2 questionnaires, the Disabilities of the Arm, Shoulder, and Hand and the American Shoulder and Elbow Surg

BACKGROUND: Acute unreconstructible 3- or 4-part proximal humerus fractures can be treated with hemiarthroplasty or reverse polarity shoulder arthroplasty. Randomized trials using implants from multiple different companies or uncemented implants have found superior results with reverse polarity arthroplasty.
HYPOTHESIS: We tested the hypothesis that cemented reverse polarity arthroplasty produces a superior outcome compared to cemented hemiarthroplasty using one implant system in patients aged 65 years and over at 12 months follow-up as measured with the Constant Score.

BACKGROUND: Due to the high complication rates associated with open reduction and internal fixation using locking plates for complex proximal humerus fractures (PHFs) in elderly patients, many authors consider reverse shoulder arthroplasty (rTSA) the treatment of choice. However, the use of cemented screws in locked plating (open reduction and internal fixation using locking plates with cemented screws [ORIF-LPc]) has been suggested to reduce implant-related complications. The aim of this matched-pair study was to compare clinical and radiographic outcomes, complication, and revision surgery r

INTRODUCTION: Humeral shaft fractures are a common injury of the upper limb, with the primary surgical treatment modality being the locking compression plate (LCP) technique. The advent of Anterior medial minimally invasive plate osteosynthesis (MIPO) technology has led to its gradual implementation in clinical practice. However, the efficacy and long-term outcomes of MIPO require further investigation. The objective of this study is to compare the therapeutic effects of LCP and MIPO in the management of humeral shaft fractures.

BACKGROUND: Reverse total shoulder arthroplasty (RTSA) has continued to increase in clinical utility and popularity as an effective treatment for cuff tear arthropathy (CTA), irreparable rotator cuff tears (RCTs), osteoarthritis, and acute 3- and 4-part proximal humeral fractures. Performing RTSA for acute proximal humeral fractures presents the unique challenges of tuberosity management, bone loss, and instability compared with elective indications such as CTA or irreparable RCTs. The purpose of this study was to compare the clinical outcomes, active range of motion (ROM), radiographic outcom

A minimally invasive technique is presented for the treatment of acute traumatic posterior shoulder dislocation with a reverse Hill-Sachs lesion. Arthroscopic reduction was combined with internal fixation using 1 absorbable anchor. Under high-definition arthroscopic visualization, precise anatomical reduction of the impacted anteromedial humeral head compression fracture was achieved. Absorbable anchors were inserted into the subchondral bone defect beneath the reduced fragment. Initial stability was provided, and congruity of the articular surface is maintained. Compared with conventional app

BACKGROUND: Total shoulder arthroplasty (TSA) has seen a dramatic rise in utilization, particularly in the elderly population. While indications have expanded beyond glenohumeral osteoarthritis (OA) to include proximal humerus fractures (PHFs), robust evidence comparing outcomes between these groups remains limited. We sought to compare the rate of surgical and medical complications of TSA (reverse or anatomic) performed for PHFs versus OA in elderly patients at 1 and 3 years post-operatively.
