"Two-Step Shish Kebab": A Useful Technique for Reverse Shoulder Arthroplasty in Humeral Surgical Neck Non-unions.

The treatment of surgical neck non-unions of the proximal humerus presents significant challenges, with reverse shoulder arthroplasty emerging as a treatment option. During this procedure, avoiding tuberosity osteotomies is crucial to optimizing functional outcomes and minimizing complications. We introduce the "two-step shish kebab" method, a useful technique where the metaphyseal and tuberosity segments of the proximal humerus are percutaneously reduced and transfixed with a modified Steinmann pin before performing the deltopectoral approach. This pin is subsequently replaced by the humeral

Does Postoperative Glenoid Retroversion Affect the 2-Year Clinical and Radiographic Outcomes for Total Shoulder Arthroplasty?

BACKGROUND: While glenoid retroversion and posterior humeral head decentering are common preoperative features of severely arthritic glenohumeral joints, the relationship of postoperative glenoid component retroversion to the clinical results of total shoulder arthroplasty (TSA) is unclear. Studies have indicated concern for inferior outcomes when glenoid components are inserted in 15° or more retroversion. QUESTIONS/PURPOSES: In a population of patients undergoing TSA in whom no specific efforts were made to change the version of the glenoid, we asked whether at 2 years after surgery patient

Radiographic changes and clinical outcomes associated with an adjustable diaphyseal press-fit humeral stem in primary reverse shoulder arthroplasty.

BACKGROUND: Press-fit humeral fixation in reverse shoulder arthroplasty (RSA) has become increasingly popular; however, radiographic analysis of these stems is limited. We aimed to evaluate the radiographic and clinical outcomes of an adjustable diaphyseal press-fit humeral stem in primary RSA. METHODS: We conducted a retrospective review of 232 primary RSAs in 219 patients performed by a single surgeon using this system. Radiographic outcomes were evaluated in patients with at least 2 years of radiographic follow-up. Standardized postoperative digital radiographs were analyzed for loosening,

Proximal humerus osteolysis after revision rotator cuff repair with bioabsorbable suture anchors.

Biodegradable anchors were designed to provide secure fixation while allowing for later resorption and replacement by host tissue. First-generation implants degraded relatively rapidly and caused foreign-body reactions, synovitis, fragmentation, and osteolysis. Newer implants have similar complications. It is not known if the primary cause of the osteolysis is biological (precipitated by breakdown products of the polymer) or mechanical (caused by initial loss of implant stability). Case reports have described glenoid osteolysis around biodegradable suture anchor placement for shoulder stabiliz

Stemless shoulder arthroplasty: current status.

BACKGROUND: Since the original Neer humeral replacement in the 1950s, the standard primary anatomic total shoulder arthroplasty design has slowly evolved. Most recently, the humeral stem has become progressively shorter to help combat stem-related complications. Currently, there are several companies who have developed and marketed a stemless humeral arthroplasty component. MATERIALS AND METHODS: Manufacturers' data for 5 stemless shoulder arthroplasty components currently on the market were analyzed and reviewed. A literature review of short-term results for stemless shoulder arthroplasty wa

Wear-induced loss of mass in reversed total shoulder arthroplasty with conventional and inverted bearing materials.

The notching phenomenon is one of the major concerns with reversed total shoulder arthroplasty. Repetitive contact between the humeral implant and the scapula (mechanical notching) produces progressive abrasion of the implant if the moving part is made of polyethylene. Its debris may then lead to active osteolysis (biological notching). Inversion of bearing materials, i.e. Glenosphere made of polyethylene and humeral Inlay made of metal, aims at the reduction of this phenomenon. However, the question arises if the tribological behavior would then be different. On an experimental setup, the gra

Does stem length affect proximal humeral stress shielding? A systematic review.

INTRODUCTION: Total shoulder arthroplasty (TSA) has increased in use, with revision surgeries also increasing. One cause of revision is aseptic loosening driven by stress shielding-a condition where altered load distribution leads to proximal humeral osteolysis surrounding the humeral prosthesis. This systematic review compares stress shielding between traditional-length (TL) and short-stem (SS) humeral prostheses, hypothesizing that SS stems reduce stress shielding. METHODS: A systematic search of Cochrane, Embase, and Medline databases was conducted. Inclusion criteria targeted studies from

A retrospective comparative cohort study of radial head arthroplasty versus resection in complex elbow dislocations.

INTRODUCTION: The management of radial head fractures for the treatment of complex elbow dislocations remains controversial. Until the publication of recent therapeutic algorithms, different actions have been carried out, including resection or arthroplasty of the radial head. This study aims to compare the clinical and radiological results of these two techniques. MATERIALS AND METHODS: A retrospective study was conducted between the years 2001 and 2016 in complex elbow dislocations associating comminuted fractures of the radial head (Mason type IV) for two cohorts: 20 patients who received

Short- to mid-term results of metallic press-fit radial head arthroplasty in unstable injuries of the elbow.

We assessed the short- to mid-term survival of metallic press-fit radial head prostheses in patients with radial head fractures and acute traumatic instability of the elbow. The medical records of 42 patients (16 males, 26 females) with a mean age of 56 years (23 to 85) with acute unstable elbow injuries, including a fracture of the radial head requiring metallic replacement of the radial head, were reviewed retrospectively. Survival of the prosthesis was assessed from the radiographs of 37 patients after a mean follow-up of 50 months (12 to 107). The functional results of 31 patients were ass

Press-fit vs. cemented humeral stem fixation for reverse shoulder arthroplasty: functional outcomes at a mean follow-up of 9.5 years.

BACKGROUND: Reverse shoulder arthroplasty (RSA) is a reliable, long-term treatment option for degenerative shoulder pathology. However, the functional outcomes degrade around the fifth year postoperation, particularly for internal and external rotation. Long-term radiographic changes have been reported around the humeral stem, depending on the fixation method. OBJECTIVES: The primary objective was to compare the clinical outcomes of RSAs by fixation method-cemented vs. press-fit-after a minimum follow-up of 5 years. We hypothesized that proximal humeral osteolysis impacts the functional outco

Evaluation of stress distribution of different marginal designs on PEEK and PEKK substructure materials, cortical and cancellous Bone:A finite element analysis.

BACKGROUND: High-performance polymers are used in different fixed prosthesis treatments due to their many advantages such as biocompatibility, shock absorption ability, high fracture resistance. The effects of marginal design on the forces on high-performance polymers are unknown. This study aimed was to investigate the stress distribution of different marginal designs on Polyetheretherketone (PEEK) and Polyetherketoneketone (PEKK) substructure materials, cortical bone and cancellous bone by finite element analysis. METHODS: A first maxillary molar tooth was modeled in 3D using the "3D Comple

Bilateral Reverse Total Shoulder Arthroplasty for Bilateral Proximal Humeral Fractures Complicated by Periprosthetic Fractures.

Reverse shoulder arthroplasty (RSA) is being increasingly employed for complex proximal humeral fractures (PHFs) in older patients, particularly in cases with severe osteoporosis. However, bilateral PHFs and subsequent periprosthetic fractures remain rare and challenging. We report a case of a 60-year-old woman with alcoholic liver disease who sustained bilateral PHFs after a fall. Due to her compromised condition, staged bilateral RSA was performed. The initial postoperative course was uneventful. Eight months after the first surgery, she sustained a displaced periprosthetic fracture on the r

Validation of a new classification for periprosthetic shoulder fractures.

INTRODUCTION: Successful treatment of periprosthetic shoulder fractures depends on the right strategy, starting with a well-structured classification of the fracture. Unfortunately, clinically relevant factors for treatment planning are missing in the pre-existing classifications. Therefore, the aim of the present study was to describe a new specific classification system for periprosthetic shoulder fractures including a structured treatment algorithm for this important fragility fracture issue. METHODS: The classification was established, focussing on five relevant items, naming the prosthes
