[Psychoeducational and cognitive behavioral treatments of bipolar disorder].

UNLABELLED: Bipolar disorder is a severe mood disorder characterized by recurrence of mania and depression. Despite the use of mood stabilizers, a significant proportion of bipolar patients experience relapse, psychosocial impairment and persistent symptoms. A significant part of patients show poor adhesion to the pharmacological treatment. This article aims to provide an overview of research focusing on psychoeducational and cognitive-behavioral treatment (CBT) of bipolar patients. METHOD: Studies were identified through Medline searches between 1971 and 2005. RESULTS: Studies on bipolar pa

The impact of anxiety and mood disorders on physical disease: the worried not-so-well.

Depression and anxiety can adversely affect the course of chronic physical illnesses, increasing morbidity and mortality. The literature during the past year is reviewed for gastrointestinal disorders, cancer, pain, heart disease, diabetes, and pulmonary disease. Causes for this relationship are behavioral and biological. Behavioral factors in patients with depressive and anxiety disorders include unhealthy lifestyle choices, disrupted sleep, and poor adherence to medical regimens. Biological mechanisms include increased inflammatory response and disruption of the hypothalamic-pituitary-adrena

[Anxiety-ridden patients in the doctor's office].

Initially, patients with anxiety syndromes consult the general practitioner complaining of physical symptoms. After an appropriate differential diagnostic work-up, the family doctor can therefore check whether the patient might possibly be suffering from an anxiety disorder. For the treatment of such disorders, psychotherapeutic and psychopharmacological strategies all have a role to play. The general practitioner can initiate supportive medication before referring the patient to a specialist. Early detection and treatment of an anxiety disorder can prevent the condition from becoming chronic

[Case report long-term depressive adjustment disorder. Occupational therapy diagnosis and psychotherapy treatment of an adjustment disorder].

Stressing live events can lead to serious deficits of emotional and social function and decrease in vitality. Depressive mood, anxiety and a considerable reduction in the mastering of daily live activities are symptoms of these disorders. Social medical and economical consequences can be reduced by early diagnosis of left performance and psychotherapeutic interventions. The example of a 41 year old man with adjustment disorder after an embolic brain injury shows how diagnosis of performance, and special psychotherapeutic treatment allowed the reintegration on the labour market.

Measuring adjustment to chronic skin disorders: validation of a self-report measure.

This article describes the development of a questionnaire that assesses problems in adapting to chronic skin disorders, the Adjustment to Chronic Skin Diseases Questionnaire. Patients (N = 442) with different skin disorders completed the original item pool. Principal-components analysis suggested a 6-factor solution that was largely replicated with 2 additional samples of 192 patients with psoriasis or atopic dermatitis and 165 patients with atopic dermatitis. Four of the subscales showed very good internal consistencies, retest reliabilities, and sufficient correlations with expert ratings: S

Multiple endocrine neoplasia type 2a (MEN2a): a call for psycho-social research.

This paper summarizes what is known about the diagnosis, treatment, and follow-care of people with the inherited disorder multiple endocrine neoplasia type 2a (MEN2a). Several databases were searched throughout the year 2000, including MEDLINE, CINAHL, CancerLit, PsychINFO, and Dissertation Abstracts International. MEN2a is a complex disease involving cancer care and control by surgery, health maintenance, and life-long observation of gene mutation carriers. Genetic testing is the standard of care, prophylactic surgery is recommended during directive counseling, and life-long surveillance is e

[Anxiety and physical limitation: a complex relation].

Psychological problems affecting physically handicapped individuals are understudied. However, some studies suggest a higher risk of developing an elevated level of anxiety among these individuals. A previous study reported that people with multiple sclerosis show more anxiety than patients suffering from another type of physical limitations. These results raise questions about the specificity of the link between anxiety and some medical conditions. The aim of the present study is to increase our understanding of manifestations of anxiety associated with degenerative illnesses. Three groups of

Seeking medical consultation: perceptual and behavioral characteristics distinguishing consulters and nonconsulters with functional dyspepsia.

OBJECTIVE: Subjects with functional dyspepsia (FD) in most previous studies have been confined to those who sought medical consultation. The generalizability of results from these studies to individuals with FD who do not seek medical consultation is limited. This study examined 1) differences in perceptual and behavioral characteristics between "nonconsulters" and "consulters" with FD and 2) the influence of these characteristics on dyspeptic and psychological symptoms. METHODS: A matched case-control design was used to compare differences among 43 nonconsulters with FD, 43 consulters with F

Psychiatric disorders in patients with fibromyalgia. A multicenter investigation.

The authors conducted an investigation in four tertiary-care centers to determine if psychiatric comorbidity and psychological variables were predictive of functional impairment in patients with fibromyalgia syndrome (FMS). Seventy-three individuals were administered the Structured Clinical Interview for DSM-III-R, the Rand 36-item Health Survey (SF-36), and multiple self-report measures. The patients with FMS were found to have a high lifetime and current prevalence of major depression and panic disorder. The most common disorders were major depression (lifetime [L] = 68%, current [C] = 22%);

[Pre-eclampsia and its psychosocial sequelae].

In the present work some psychosomatic conditions in the setting of preeclampsia are described. The important psychosocial consequences for women suffering from this disease and the drawback for their partners will be elucidated. Preeclampsia as a disease including hypertension, proteinuria and generalized edema is often associted with generalized seizures occuring most commonly at the end of the second trimenon of pregnancy. The disease bears a heavy risk for the mothers as well as for her unborn child. Until now the exact pathophysiological basis of the disease has not been entirely elucidat

Abnormal illness attitudes in patients with irritable bowel syndrome.

The Illness Attitudes Scales (IAS) and the Beck Depression Inventory (BDI) were administered to 40 patients with irritable bowel syndrome (IBS) and these were compared with 35 patients with organic gastrointestinal (GI) disease, 37 depressed patients, and 40 healthy volunteers. The BDI score was found to be greater in the IBS patients than in either the patients with organic disease or healthy subjects. All the patient groups had abnormal IAS scores compared with the healthy group, but these were most marked among the IBS patients with elevated scores on six out of the eight subscales. Three o

[Follow-up and social consequences of Crohn disease and ulcerative colitis].

PATIENTS: We studied the course and particularly the social consequences of Crohn's disease (n = 107) and ulcerative colitis extending beyond the sigmoid colon (n = 22) during the first 6.1 respectively 6.3 years after onset. RESULTS: Under common treatment, the disease activity decreased and the body mass index increased significantly in the majority of patients, but about 30% of them reached no continuous remission. The professional training was delayed in 23% of patients with Crohn's disease and 9% of those with ulcerative colitis. However, by objective datas and in the patients' own opini

Correlation between depression and vision in aged patients before and after cataract operations.

To investigate the association between depression and vision, 100 cataract operation patients (25 were men and 75 women) aged 71 to 76 years were studied. One day before the operation and 3 mo. after, the patients' depression was tested with the short form of the Beck Depression Inventory and their personalities with Mini-Mult MMPI. The cataract operation restored visual acuity sufficient for reading (minimum E-test value 0.40) to 79% of the subjects. The analysis indicated that their depression was significantly correlated with vision only after the cataract operation. Depression increased wi

Detection of psychopathologic trends in glossodynia patients.

Glossodynia affects primarily middle-aged women. Although many possible etiologies have been proposed for the syndrome, most have not been substantiated. In the present study 56 glossodynia patients were evaluated for their psychopathologic profile as reflected by the SCL-90 questionnaire. The data show that glossodynia patients present a relatively high psychopathologic profile, especially on the scales of somatization and depression. Significant correlations were found between the intensity of pain experienced by the patients and some of the SCL-90 scales (somatization, depression, anxiety,

Psychiatric morbidity among cancer patients and its relationship with awareness of illness and expectations about treatment outcome.

Sixty consecutive patients admitted to an oncology unit in a general hospital were systematically assessed to determine the prevalence of psychiatric disorders. Patients' awareness of the diagnosis of cancer and their perception of treatment intention and outcome were assessed independently by another investigator who was blind to the psychiatric diagnosis. Forty percent of the sample had a diagnosis of psychiatric disorder, based on the Diagnostic and Statistical Manual for Mental Disorders (3rd edition, revised). Adjustment disorders comprised most of the psychiatric diagnoses. Major depress

[Psychological and physical compromises in patients with organic cerebral psychosyndrome--analysis of treatment success].

The extent of psychic and physical disturbances of patients admitted to the Center of Psychiatry in Frankfurt/M. because of organic psychosyndromes in the year 1988 was analysed. Especially the results of the hospital treatment and the proceeding mode of caretaking were examined. Usually an unspecific state of confusion was the main reason for admittance of most of the patients (n = 59). The majority of the patients suffered from a mid-ranged to marked state of dementia. The results of the treatment for these patients was highly significantly worse compared to patients admitted with a psychosi

Elephants dancing in my head: a developmental approach to children's concepts of specific pains.

The present study produced an empirically derived, developmental continuum of children's understanding of specific pains. Subjects of 5 age groups: preschool (ages 3-4), first grade (ages 6-7), third grade (ages 8-10), sixth grade (ages 11-12) and college freshmen (ages 18-23) were interviewed with open-ended questions. The subjects were questioned extensively about 3 specific types of pain: an injury (skinned knee), a medical intervention (injection), and an illness (headache). Subjects were asked to describe each pain, tell why the pain hurt, and state the value of the pain. Their answers we

Psychological side effects of breast cancer screening.

Evaluated the impact of receiving abnormal mammogram results on women's anxiety and breast cancer worries and on their breast self-examination (BSE) frequency and intentions to obtain subsequent mammograms. A telephone survey was conducted with 308 women 50 years old and older approximately 3 months following a screening mammogram. Subjects included women with suspicious abnormal mammograms, nonsuspicious abnormal mammograms, and normal mammograms. Women with suspicious abnormal mammograms exhibited significantly elevated levels of mammography-related anxiety and breast cancer worries that int

Cerebrospinal fluid concentrations of beta-endorphin in chronic low back pain patients. Influence of depression and treatment.

Cerebrospinal fluid (CSF) beta-endorphin concentrations were determined before and after treatment in 28 patients suffering chronic neuralgic low back pain/sciatica. Nine patients carried the additional diagnosis of major depressive disorder. Pain treatment was multimodal and resulted in variable pain reduction. CSF beta-endorphin concentrations spanned a wide range with no association to age, gender, pain ratings, depressive symptomatology, and drug intake. CSF beta-endorphin concentrations were not influenced by the presence of major depressive disorder and did not change with successful tre
