The authors present a case study of synovitis after an arthroplasty of the first metatarsophalangeal joint and placement of a hemisilicone joint implant. This was an unusual presentation because excision of the prosthesis was necessary 1 month after placement. Also, a review of the literature is presented on silicone synovitis and other complications that arise after silicone joint implantation.

Silicone oil has established itself as an internal tamponade in the treatment of complicated retinal detachments. Patients frequently complain of visual changes after intraocular silicone oil injection. The degree of these changes as well as refractive changes were analyzed in 11 aphacic eyes that had a postoperative visual acuity of at least 0.1. Visual acuity and refraction were obtained with the patients in a supine position. In children aged 3 months or older lacrimal probing was performed to cure nasolacrimal duct obstructions. In 56 cases where this procedure failed a silicone tube was inserted. The results were favourable in 47 cases. Children with facial malformations and those with a narrow nasolacrimal duct however proved to be poor candidates for silicone tube insertion.

We have described a patient in whom an erosive arthropathy of the right wrist developed three years after insertion of a silicone lunate prosthesis. The clinical picture of silicone synovitis is being recognized more frequently after bone and joint implants, but the diagnosis may be delayed because initial suspicions focus on infection or crystal-induced arthropathy rather than foreign body reaction.

Intraocular silicone oil tamponade is used as an adjunct to vitrectomy in the treatment of proliferative vitreoretinopathy. The authors analyzed the effect of silicone oil on the corneal endothelium in 28 patients (10 phakic and 18 aphakic). In the phakic group, the endothelial cellular loss is small, no significant. In the aphakic group, the endothelial cellular loss is 20%.

A 58-year-old white male patient developed corneal complications following cataract surgery and implantation of a silicone posterior chamber intraocular lens. At the time of penetrating keratoplasty, the implant was noted to be partially dislocated into the anterior chamber, surrounded by a dense fibrovascular membrane. Histopathological features were consistent with pseudophakic bullous keratopathy.

Silicone oil is now used with increasing frequency to treat cases of complicated retinal detachment (RD). The authors report their results using silicone oil in eyes with RD resulting from proliferative diabetic retinopathy (PDR) or proliferative vitreoretinopathy (PVR). The retina was completely reattached at last examination in 16 (70%) of 23 eyes with PDR and 20 (67%) of 30 eyes with PVR. A retrospective study was carried out in search of silicone deposits in 11 periodic hemodialysis patients (1 liver biopsy, 4 splenectomies and 6 autopsies) treated between 12 and 60 months with equipment containing a silicone pump segment. Fifty-two patients with rectal prolapse have been treated by the silicone rubber band perianal suture technique and satisfactory results have been obtained in 46 (89%). Eleven patients required reoperation to achieve this result. The procedure is a minor one, with little morbidity and no mortality. Provided that faecal impaction can be avoided in patients having this operation a successful outcome, can be expected. It is recommended especially for the frail and elderly with rectal prolapse.
