Impact of skull base cisterns invasion and extent of resection on the management of skull base intracranial epidermoid tumors.

BACKGROUND: This study reports the authors' experience with microsurgical resection of skull base epidermoid tumors, examining the relationship between the number of invaded intracranial subarachnoid cisterns (SC) and the extent of resection, recurrence and outcomes. METHODS: Data from 84 patients who underwent skull base epidermoid tumor resection between 2000 and 2022 were prospectively collected and retrospectively reviewed. Clinical presentations, imaging findings (including the number of invaded cisterns), and extent of resection were analyzed for their influence on postoperative complic

Transoral versus transfacial surgical approach to maxillary tumors: evaluation of outcomes and perspectives.

Neoplasms of the maxilla have multiple different origins and histology, and often extend towards the infratemporal fossa, orbit, or skull base. Extensive resection may be required, often leading to poor esthetic and functional results. Usually, these lesions are removed via a transfacial approach. The aim of this study was to compare the outcomes of the transoral versus transfacial approach for maxillary tumors. A single-institution retrospective study was conducted on patients with maxillary-midface tumors, treated between January 2009 and December 2019. The patients were divided into two gro

Repeat Imaging in Idiopathic Unilateral Vocal Fold Paralysis: Is It Necessary?

OBJECTIVE: Imaging plays a critical role in the evaluation of patients presenting with unilateral vocal fold paresis or paralysis of unknown etiology. In those with idiopathic unilateral vocal fold paralysis (iUVFP), there is no consensus regarding the need or timing of repeat imaging. This study seeks to establish the rate of delayed detection of alternate etiologies for these patients to determine if and when imaging should be repeated. METHODS: Retrospective chart review was conducted identifying patients at our institution with vocal fold movement impairment between 1998 and 2014. Idiopat

The role of gadolinium-DTPA in the evaluation of extracranial head and neck mass lesions.

Based on our experience with 28 patients, there is good evidence that gadolinium-enhanced MR will be useful in evaluation of skull base involvement, sinus involvement, or intracranial extension by neoplasms that involve the neck above the hard palate. This region has historically been a difficult area to image with any modality, because of the detailed anatomy and numerous important structures that traverse it. Gadolinium-enhanced MR should provide a valuable tool in the evaluation and management of these patients. The evidence that gadolinium is useful in staging of primary and nodal squamous

Endoscopic paramaxillary ligation of the internal maxillary artery.

Endoscopic skull base surgery is increasingly utilized for managing skull base tumors. Complex techniques for resecting multicompartmental lesions can lead to higher intraoperative bleeding rates. While embolization can reduce blood flow, it may not always be feasible and can introduce additional risks. Endoscopic techniques allow for direct visualization and coagulation of parent arteries. This study discusses a subperiosteal transoral medial dissection technique, endoscopic paramaxillary corridor, to enable early vascular control prior to resection.

Complete excision of a giant chondrosarcoma within the cavernous sinus: a case report and literature review.

BACKGROUND: Primary skull base chondrosarcoma (SBC) is a rare malignant central nervous system tumor, often involving the cavernous sinus. Complete excision of tumors invading this region is exceptionally challenging due to the presence of the internal carotid artery and numerous nerves within the cavernous sinus, particularly in cases with substantial tumor volume. CASE PRESENTATION: This report describes a rare case of a massive primary SBC pushing the lateral wall of the cavernous sinus, measuring approximately 6.6 cm × 4.5 cm × 4.4 cm. Utilizing neurophysiological monitoring and intraoper

Extradural disconnection of the cavernous sinus with preservation of the internal carotid artery: indication and technique.

BACKGROUND: Extradural disconnection of the cavernous sinus (CS) with preservation of the internal carotid artery (ICA) is indicated for aggressive and recurrent tumors, in patients presenting loss of oculomotor function and non-functional circle of Willis. METHOD: Extradural resection of the anterior clinoid process disconnects the CS anteriorly. The ICA is dissected in the foramen lacerum via extradural subtemporal approach. The intracavernous tumor is split and removed following the ICA. Bleeding control of the inferior and superior petrosal and intercavernous sinuses completes posterior C

Surgical Strategy in Modification of the Transpetrosal Approach to Avoid Postoperative Venous Complications: A Report of 74 Consecutive Cases.

BACKGROUND: The transpetrosal approach is a complex skull base procedure with a high risk of complications, particularly caused by injury of the venous system. It is in part related to variability of blood outflow pathways and their distinctive patterns in each individual patient. OBJECTIVE: To evaluate outcomes and complications after skull base surgery with use of the petrosal approach modifications, which selection was based on the detailed preoperative assessment of venous drainage patterns. METHODS: Overall, 74 patients, who underwent surgery via the transpetrosal approach at our instit

Primary chondrosarcomas of the larynx treated with proton radiotherapy: A single institutional experience.

BACKGROUND: Primary laryngeal chondrosarcomas are rare entities whose excellent survival rates following resection promote conservative surgical approaches to maintain quality of life without compromising outcomes. There are excellent outcomes in skull base chondrosarcomas treated with maximal safe resection and post-operative proton therapy. Extrapolating from these findings, we report our institutional experience treating symptomatic or growing laryngeal chondrosarcomas using proton beam therapy. CASES: Demographic information, clinical characteristics, treatment details, and follow-up data

Perioperative nasal and paranasal sinus considerations in transsphenoidal surgery for pituitary disease.

Endoscopic endonasal skull base surgery has emerged as the treatment modality of choice for a range of skull base lesions, particularly pituitary adenomas. However, navigation and manipulation of the nasal corridor and paranasal sinuses requires that surgeons are aware of effective techniques to maximize patient outcomes and avoid sinonasal morbidity postoperatively. This paper is a narrative review aimed to provide an updated and consolidated report on the perioperative management of the nasal corridor and paranasal sinuses in the setting of endoscopic skull base surgery for pituitary disease

Ophthalmic and orbital considerations in the evaluation of skull base malignancies.

INTRODUCTION: The orbital contents, afferent and efferent visual pathways, and the cranial nerves involved in eye movement, corneal sensation and eyelid closure traverse the skull base, a region bounded by the intracranial cavity, the paranasal sinuses, and the deep spaces of the face and head. As such, tumors from above or below have potential to affect some aspect of the visual system. METHODS: We discuss here the clinical ophthalmologic and orbital considerations in the evaluation of patients with these tumors, as well as the ophthalmic sequelae of treatment with radiation or surgery (or b

Tumors of the anterior skull base.

A variety of histologic tumor types are present in the anterior skull base. Primary tumors of this area may be derived from the bone, paranasal sinuses, nasopharynx, dura, cranial nerves, pituitary gland and brain. Symptoms are caused mostly through mass effect but, if the tumor becomes aggressive, also through invasion. Selection of surgical approaches to the anterior skull base is based upon balancing risk reduction with maximizing extent of resection. Here we review a spectrum of neoplastic entities found in the anterior skull base in adults and discuss clinical and radiographic presentatio

High-Fidelity Simulation for Skull Base Surgery Enabled by Virtual Dura Mater Construction.

OBJECTIVE: Advances in imaging and artificial intelligence have enabled high-resolution three-dimensional (3D) constructions for neurosurgical simulation. However, accurate reproduction of the dura mater-particularly the tentorium, a thin yet critical structure in skull base surgery-remains challenging, limiting the fidelity of preoperative simulations. We describe a novel method for constructing the dura mater in 3D surgical simulation and demonstrate its utility in planning skull base tumor surgery. METHODS: Magnetic resonance imaging and contrast-enhanced computed tomography were imported

Surgical treatment of selected tumors via the navigated minimally invasive presigmoidal suprabulbar infralabyrinthine approach without rerouting of the facial nerve.

The feasibility of a novel skull base approach - the navigated minimally invasive presigmoidal suprabulbar infralabyrinthine approach (NaMIPSI-A) without rerouting of the fallopian canal for selected jugular foramen tumors (JFTs) - has been demonstrated in a neuroanatomical laboratory study. Here, we present our clinical experience with the NaMIPSI-A for selected JFTs, with a particular focus on its efficacy and safety. All patients with JFTs who were treated via the NaMIPSI-A were included in this study. The JFTs were classified according to a modified Fisch classification. The neurological a
