Post-operative sleep disturbance: causes, factors and effects on outcome.

Post-operative sleep disturbance, with suppression of rapid eye movement sleep and slow wave sleep followed by a subsequent rebound, seems to be related to the magnitude of trauma and thereby to the surgical stress response. In this context, cortisol, autonomic stimulation, and certain cytokines may lead to abnormal sleep. Furthermore, the environment, pain and the administration of analgesics seem to be important factors in the precipitation of sleep abnormalities. Post-operative sleep disturbance may contribute to the development of episodic hypoxaemia, haemodynamic instability and altered m

Growth hormone responses to sleep, insulin hypoglycaemia and arginine infusion.

This study compares the peak serum growth hormone (GH) concentration during slow wave sleep with the serum GH responses to insulin-induced hypoglycaemia and intravenous arginine infusion in 23 children referred because of short stature (20) or precocious puberty (3). Peak serum GH concentration during sleep correlated significantly with peak GH response to insulin hypoglycaemia (r = 0.64, p less than 0.01) and arginine infusion (r = 0.57, p less than 0.01). 3 children had subnormal (less than 15 mU/l) peak serum GH concentrations during sleep but normal responses to either insulin-induced hypo

The sleep phenotype of Borderline Personality Disorder: A systematic review and meta-analysis.

AIM: To delineate the sleep profile of Borderline Personality Disorder (BPD). METHOD: A meta-analysis to synthesise findings on the objective and subjective sleep characteristics of BPD. RESULTS: We identified 32 studies published between 1980 and December 2015. Meta-analysis indicated significant differences between BPD and healthy control groups across objective sleep continuity (sleep onset latency, total sleep time, sleep efficiency) and architecture (rapid eye movement latency/density, slow wave sleep) measures, and self-reported sleep problems (nightmares, sleep quality). Findings were

Night-time sleep and daytime sleepiness in narcolepsy.

This report describes night-time sleep and daytime sleepiness in a large (N=530) sample of patients meeting the International Classification of Sleep Disorders criteria for diagnosis of narcolepsy. Sleep data were obtained from polysomnographic recordings on two consecutive nights. Sleepiness was assessed using the Multiple Sleep Latency Test, the Maintenance of Wakefulness Test and the Epworth Sleepiness Scale. Analysis revealed that sleep was mild to moderately disturbed on both recording nights. A first-night effect was suggested by decreased REM latency and increased percentage REM and slo

Independent associations between fatty acids and sleep quality among obese patients with obstructive sleep apnoea syndrome.

The aim of this study was to examine the relationships between gluteal adipose tissue fatty acids and sleep quality in obese patients with obstructive sleep apnoea syndrome after controlling for possible confounders. Sixty-three patients with obstructive sleep apnoea syndrome based on overnight attended polysomnography were included. Gluteal adipose tissue fatty acids were analysed by gas chromatography. Anthropometric measurements were carried out. Depressive symptoms were assessed by the Zung Self-rating Depression Scale. Saturated fatty acids were positively related to total sleep time, sle

[Studies with the fully automated EEG sleep analysis system QUISI].

To judge sleep reliably would be interesting not only for outpatient sleep diagnostics, but also in the inpatient sphere. A step in this direction is the easy-to-apply fully automated sleep-EEG-analysis system QUISI. Nine males patients with obstructive sleep apnoea syndrome were examined simultaneously by polysomnography and the QUISI system. A comparison of the sleep profiles evaluated by polysomnographic recordings using Rechtschaffen and Kales' rules with the automated classification using QUISI recording shows that the difference of the sleep parameters sleep period time (SPT), total slee

Neurophysiology of CSWS-associated cognitive dysfunction.

The phenomenon of continuous spikes and waves during slow-wave sleep (CSWS) is associated with a number of epileptic syndromes, which share a behavioral phenotype characterized by deterioration of cognitive, behavioral, or sensorimotor functions. Available evidence seems to suggest that spike-wave activity is a result of a complex interaction between cortical and subcortical inhibitory networks and can "per se" produce a transient loss of underlying cortical functions. Syndromes like Landau-Kleffner syndrome, CSWS, and phenomena such as negative myoclonus could share in common--at least at the

Pre-sleep acute stress modulates sleep-dependent emotional memory.

OBJECTIVES: Acute stress prior to bedtime significantly alters the subsequent sleep. Although sleep is crucial for consolidating newly acquired memories, the impact of pre-sleep stress remains unclear. This experimental study examined how acute stress induction before bedtime influences overnight consolidation of emotional and neutral memories. METHODS: 34 healthy adults (65% females, Mage = 25.76 years, SD = 3.35) were randomly assigned to stress (SC) or control (CC) conditions, using a virtual reality stress or control paradigm. Prior to manipulation, participants encoded emotional and neut

[Effect of anticholinesterase agents on the protein and RNA content in the cells of the supraoptic nucleus during sleep and its disorder].

In rats, administration of anticholinesterase drugs (both the penetrating blood-brain barrier armin and the nonpenetrating preparation Gd-42) prevented the decrease of protein content in the neurons and glia of the supraoptic nucleus, as well as of RNA content in the neurons which usually occurred in intact animals after 24-hr deprivation of the REM sleep by the Jouvet method (also with partial disturbance of the slow wave phase).

Sleep increases firing rate modulation during interictal epileptiform discharges in mesial temporal structures.

Epileptic seizures and interictal epileptiform discharges are strongly influenced by sleep and circadian rhythms. However, human data on the effect of sleep on neuronal behaviour during interictal activity have been lacking. We analysed EEG from eight epileptic patients implanted with macro and micro electrodes in mesial temporal structures. Sleep staging was performed on polysomnography and video-EEG. Automated detection identified thousands of interictal epileptiform discharges per patient. Both their rate and amplitude increased with deeper stages of non-rapid eye movement sleep. Single- an

Polysomnography and maintenance of wakefulness test as predictors of CPAP effectiveness in obstructive sleep apnea.

Nasal continuous positive airway pressure (CPAP) therapy is usually effective in the treatment of obstructive sleep apnea (OSA), reversing the excessive daytime sleepiness associated with OSA. We investigated how polysomnography and maintenance of wakefulness test (MWT) predicted response to CPAP therapy in 10 patients with obstructive sleep apnea and 10 healthy controls. After a full polysomnography and MWT the patients were started on CPAP. MWT was repeated to quantify the treatment effect. Oxygen desaturation index and the percentage of slow wave sleep were the best predictors for baseline

Stress and sleep: Novel insights into stress-related sleep disorders from EEG features to neural circuitry.

Sleep disorders and stress-related psychiatric conditions exhibit a high degree of comorbidity. Epidemiological patterns suggest that sleep disturbances frequently precede the onset of affective symptoms. This temporal relationship indicates that stress-induced disruption of sleep physiology may act as an early driver in the pathogenesis of psychiatric disorders. Current clinical assessments relying on subjective measures or standard polysomnography staging may overlook subtle neurophysiological alterations. This review examines the impact of stress on sleep dynamics through the lens of specif

Sleep spindles: an overview.

Sleep spindles are a distinctive EEG phasic feature of NREM sleep and are prevalent during stage 2 as compared to slow wave sleep. While the neurophysiological mechanisms of spindle generation, that involves thalamic and corticothalamic networks, have been recently delineated and are briefly reviewed, their definitive functional meaning still remains to be elucidated. This review summarizes the present knowledge on visually scored and automatically detected spindles, as well as the literature on EEG power in the sigma band. Among the factors known to affect sleep spindles and sigma activity, t

Effects of light therapy on sleep and anxiety in Parkinson's disease: A pilot trial.

BACKGROUND: Light therapy (LT) may improve sleep and affective disorders in Parkinson's disease (PD), but effects of different intensities and underlying mechanisms remain unclear. METHODS: Thirty-three patients with PD were randomized to receive either 1 month of bright LT (BLT) at 10000 lx or dim LT (DLT) at 200 lx, separated by a 1-month washout period, in a crossover fashion. All patients completed clinical assessments, polysomnography (PSG) and resting-state electroencephalogram (EEG) at baseline and after each intervention period. Functional connectivity was assessed using weighted Phas

[Miller-Dieker syndrome (type I lissencephaly) with specific EEG changes].

We present the case report of a girl with a subtype of Lissencephaly syndrome, type I, "Miller-Dieker syndrome", pointing out the specific EEG features in infancy and early childhood. The following pathognomonic EEG manifestations may confirm the diagnosis of an lissencephalic syndrome: abnormally fast background activity of an extraordinary high voltage increasing with age, missing topographic structuring, no reactivity to sleep or medication, unusually high-voltaged sharp-slow-wave complexes.

Sleep deprivation in narcoleptic subjects: effect on sleep stages and EEG power density.

Sleep of 8 narcoleptic and 8 control subjects was recorded under baseline (i.e., prior wakefulness 16 h) and after 24 h without sleep. During both baseline and recovery total sleep time and stage 2 non-REM sleep were significantly decreased in narcoleptic subjects. Slow wave activity (i.e., EEG power density in the range of 0.75-4.5 Hz) decayed exponentially during baseline and after sleep deprivation in both narcoleptic and control subjects. During both baseline and recovery EEG power density in delta and sigma frequencies in non-REM sleep was enhanced in narcoleptic subjects relative to cont

Sleep deprivation leads to non-adaptive alterations in sleep microarchitecture and amyloid-β accumulation in a murine Alzheimer model.

Impaired sleep is a common aspect of aging and often precedes the onset of Alzheimer's disease. Here, we compare the effects of sleep deprivation in young wild-type mice and their APP/PS1 littermates, a murine model of Alzheimer's disease. After 7 h of sleep deprivation, both genotypes exhibit an increase in EEG slow-wave activity. However, only the wild-type mice demonstrate an increase in the power of infraslow norepinephrine oscillations, which are characteristic of healthy non-rapid eye movement sleep. Notably, the APP/PS1 mice fail to enhance norepinephrine oscillations 24 h after sleep d

Sleep, health, and aging. Bridging the gap between science and clinical practice.

Problems with sleep are common with advancing years and occur in over half of adults age 65 and older. It has been estimated that insomnia affects about a third of the older population in this country. This inability to have restful sleep at night results in excessive daytime sleepiness, attention and memory problems, depressed mood, falls, and lowered quality of life. Other factors associated with aging, such as disease, changes in environment, or concurrent age-related processes also may contribute to problems of sleep. Data indicate that age by itself does not predict incident complaints of
