Obstructive sleep apnea associated with cerebral hypoxemia and death.

An increase in the arousal threshold may predispose critically ill patients with obstructive sleep apnea (OSA) to prolonged apneas and death during sleep. We report two cases in whom polysomnographically documented OSA resulted in EEG changes compatible with cerebral hypoxemia with subsequent respective transient encephalopathy in one instance and death in the other.

Obstructive sleep apnea, nocturia and polyuria in older adults.

STUDY OBJECTIVE: The purpose of this study was to examine the relationship between nocturia and obstructive sleep apnea (OSA) in community dwelling older men and women. DESIGN: A repeated measures design was employed over a 24-hour period. SETTING: The study was conducted in a clinical research center. PARTICIPANTS: Thirty community-dwelling elders (mean age=65.5, SD=8.4 years) with symptoms of nocturia and sleep disordered breathing, volunteered to participate. Both men (n = 13) and women (n = 17) and minority subjects (African-Americans, n = 19; Caucasian, n = 11) were included in the stu

[Treatment of the obstructive sleep-apnea syndrome in adults].

When treating the obstructive sleep-apnoea syndrome (OSAS), conservative management and the correction of treatable stenoses in the upper airway should be considered first. If these measures are neither effective nor applicable, then continuous positive airway pressure (CPAP) is the preferred treatment. Surgical interventions should only be considered after failure of non-surgical treatment modalities. Pharmacological management of OSAS is usually only indicated as a form of supplementary treatment in specific patients. Oral-appliance therapy appears to be of value in the management of OSAS an

[Cephalometric assessment in obstructive sleep apnea and hypopnea syndrome].

Several cephalometric studies have been published to study sleep apnea and hypopnea. The purpose of this review is to provide a clear analysis of the different and apparently contradictory cephalometric results and to describe certain pitfalls of the method. In general, an excessive development of the tongue and soft palate are observed in these subjects, limiting the pharyngeal airway. Bony anomalies include mandibular retrognathism and an excessive anterior ventral development of the skull associated with insufficient anterior growth of the base of the skull. The head is held in extension. T

[Surgery and obstructive sleep apnea syndrome: indications and precautions].

Surgery has long been a therapeutic option for obstructive sleep apnea syndrome (OSAS). The first procedures targeted the soft palate but results of isolated uvulo-palatopharyngoplasty (UPPP) were inconstant. A better understanding of the mechanisms involved in pharyngeal collapse incited interest in the role of the tongue and the retropharyngeal space. Surgical procedures were then developed for this level of the upper airways. There are a large number of possibilities. Excepting tracheostomy which court-circuits the collapsed zone, all the other procedures are designed to widen more or less

Obstructive apnea during sleep is associated with peripheral vasoconstriction.

Obstructive apnea during sleep is associated with a substantial transient blood pressure elevation. The mechanism of this pressor response is unclear. In this study we measured muscle sympathetic nerve activity (MSNA), mean arterial pressure (Psa), and mean limb blood velocity as an index of blood flow (MBV, Doppler) and calculated changes in limb vascular resistance during and after apneas during both wakefulness and sleep in patients with the obstructive sleep apnea syndrome. Immediately postapnea during sleep Psa increased significantly compared with the earlier stages of apnea and this was

Epidemiology of obstructive sleep apnea: a population health perspective.

Population-based epidemiologic studies have uncovered the high prevalence and wide severity spectrum of undiagnosed obstructive sleep apnea, and have consistently found that even mild obstructive sleep apnea is associated with significant morbidity. Evidence from methodologically strong cohort studies indicates that undiagnosed obstructive sleep apnea, with or without symptoms, is independently associated with increased likelihood of hypertension, cardiovascular disease, stroke, daytime sleepiness, motor vehicle accidents, and diminished quality of life. Strategies to decrease the high prevale

Chemical control stability in patients with obstructive sleep apnea.

The role of chemical control instability in the pathogenesis of obstructive sleep apnea (OSA) is not clear. We studied 32 patients with OSA during sleep while their upper airway was stabilized with continuous positive airway pressure. Twelve patients had repetitive OSA whenever they were asleep, regardless of body position or sleep stage, and were classified as having severe OSA (apnea-hypopnea index [AHI] = 88 +/- 19). The remaining 20 patients had sporadic OSA or repetitive OSA for only part of the time (mild/moderate OSA; AHI = 27 +/- 16). Susceptibility to periodic breathing (PB) was asses

Diagnosing and comanaging patients with obstructive sleep apnea syndrome.

BACKGROUND: Obstructive sleep apnea syndrome, or OSAS, is a common, but underdiagnosed, disorder that potentially is fatal. It is characterized by repetitive episodes of complete or partial upper airway obstruction leading to absent or diminished airflow into the lungs. These episodes usually last 10 to 30 seconds and result in loud snoring, a decrease in oxygen saturation, and chronic daytime sleepiness and fatigue. The obstruction is caused by the soft palate, base of the tongue or both collapsing against the pharyngeal walls because of decreased muscle tone during sleep. Potentially fatal s

Obstructive sleep apnea syndrome: differences between children and adults.

The obstructive sleep apnea syndrome is a common cause of morbidity in childhood. The clinical presentation, pathophysiology, polysomnographic characteristics and treatment differ between children and adults. Measurements of the upper airway pressure-flow characteristics can be useful in evaluating upper airway function in patients with OSAS as well as the normal population.

Diagnostic Approaches to Childhood Obstructive Sleep Apnea Hypopnea Syndrome.

Obstructive sleep apnea hypopnea syndrome (OSAHS) is a common sleep disorder in adults that is increasingly recognized in children, affecting 1 to 3% of children. Children experience a spectrum of severity related to the degree of upper airway obstruction, the duration of the disease, and the presence or absence of hypoxemic episodes. Failure to diagnose and treat OSAHS can result in serious, but generally reversible consequences for the child including impaired growth, neurocognitive and behavioral dysfunction, and cardiorespiratory failure. Even mild OSAHS appears linked to reversible health

Accuracy of clinical evaluation in pediatric obstructive sleep apnea.

Eighty-two children underwent polysomnography (PSG) for symptoms suggestive of obstructive sleep apnea (OSA). Symptoms reported included snoring, witnessed apneic episodes, daytime somnolence, mouth breathing, and enuresis. Tonsillar size, nasal airway patency, and percentile weight were recorded. OSA was diagnosed on PSG when obstructive events were noted and apnea + hypopnea index was five or more per hour. The overall predictive accuracy of clinical suspicion of OSA was 25 (30%) of 82. Predictive accuracies (as a percentage of those with symptoms/signs who have OSA) and prevalences (as a pe

Preparing a child with obstructive sleep apnea for transplantation.

All of the expected outcomes have been met for A.W. Intense collaboration among the pulmonary, nutrition, transplant, nephrology teams, and A.W.'s parents have accomplished what at first seemed to be quite difficult. After months of hard work and close monitoring of A.W., he was now at the 90th percentile on the growth curve. The diligence of the dietician and A.W.'s parents provided the intended outcome. A.W. had achieved a manageable weight and could be safely transplanted. The transplant team has decided to quickly taper prednisone until A.W. is completely off of this immunosuppressive afte

Midline glossectomy and epiglottidectomy for obstructive sleep apnea syndrome.

Obstructive sleep apnea syndrome is caused by narrowing of the pharyngeal airway and loss of dilator tone during sleep. In patients with severe apnea surgical correction often requires attention to both the oropharynx and hypopharynx. Tongue reduction surgery has been described for persistent apnea after failure of palatal surgery. We describe our experience with midline glossectomy with epiglottidectomy in 12 patients with a mean age of 48.8 +/- 14.2 years and body mass index of 36.0 +/- 8.8 kg/m2. Response to treatment was defined as a postoperative respiratory disturbance index (RDI) below

[Medical treatment of snoring and obstructive sleep apnea syndrome].

Although snoring may have deleterious effects by itself, its only clearly identified hazard is that of a social nuisance; therefore, treatment of snoring should before all make no harm. Conversely, the potential hazards of obstructive sleep apnea are well established, and it is clearly important to eliminiate sleep apneas. The medical treatments which may improve snoring and obstructive sleep apneas are basically the same. They include avoidance of risk factors such as obesity, alcohol and hypnotics as well as active treatments such as positional treatment, dental appliances and by nasal conti

[Coronary heart disease in patients with obstructive sleep apnea].

OBJECTIVE: As patients with severe obstructive sleep apnoea (OSA) have increased cardiovascular morbidity and mortality rates, the prevalence of coronary heart disease (CHD) was investigated in patients with OSA proven by polysomnography (apnoea/hypopnea index > or = 10/h). PATIENTS AND METHODS: In 440 patients (400 men, 40 women; mean age 55.3 [16-80] years) with OSA proven by polysomnography, the possible diagnosis of CHD was assessed by history, physical examination, ECG at rest and on exercise, one- and two-dimensional echocardiography and, where indicated, myocardial scintigraphy and cor

Simulated driving performance in patients with obstructive sleep apnea.

To assist in determining ability to drive in patients with obstructive sleep apnea (OSA), we developed a divided attention driving test (DADT) based on the work of Moskowitz and Burns (6). We first examined its ability to detect impaired performance by testing normal subjects both sober and impaired by alcohol (mean blood alcohol level, 95 +/- 25 mg/dl). Subsequently, 21 male patients with OSA (age 49.3 +/- 12.7 [SD] yr; apnea hypopnea index [AHI] 73 +/- 29) and 21 age- and sex-matched control subjects underwent polysomnography followed by daytime sleep latency testing (MSLT). Before each day-

Obstructive sleep apnea syndrome (OSAS) in children: diagnostic challenges.

Obstructive sleep apnea syndrome (OSAS) is increasingly recognized in the pediatric population. It is characterized by a combination of partial upper airway obstruction and intermittent obstructive apnea that disrupts normal ventilation and sleep. It is estimated to occur in 1-3% of children with a peak age of 2 to 5 years. Common symptoms include habitual snoring, difficulty breathing during sleep, restlessness, and witnessed apnea. Adenotonsillar hypertrophy is the most common associated condition in otherwise normal children, but cranialfacial abnormalities, neuromuscular diseases, and obes
