Accuracy of Three Commercial Wearable Devices for Sleep Tracking in Healthy Adults.

Sleep tracking by consumers is becoming increasingly prevalent; yet, few studies have evaluated the accuracy of such devices. We sought to evaluate the accuracy of three devices (Oura Ring Gen3, Fitbit Sense 2, and Apple Watch Series 8) compared to the gold standard sleep assessment (polysomnography (PSG)). Thirty-five participants (aged 20-50 years) without a sleep disorder were enrolled in a single-night inpatient study, during which they wore the Oura Ring, Fitbit, and Apple Watch, and were monitored with PSG. For detecting sleep vs. wake, the sensitivity was ≥95% for all devices. For discr

Hormonal and pharmacological manipulation of the circadian clock: recent developments and future strategies.

The mammalian circadian oscillator, located in the suprachiasmatic nuclei of the anterior hypothalamus, serves as the principal source of rhythmic temporal information for virtually all physiologic processes in the organism, including the alternating expression of sleep and wakefulness. Recent studies, in both animal models and human subjects, have demonstrated the important modulation of sleep and wakefulness mediated by the circadian clock. Independent of other factors, notably prior sleep-wake history, the circadian clock potentiates wakefulness (and alertness) at one phase of the diurnal c

Control of non-REM sleep by ventrolateral medulla glutamatergic neurons projecting to the preoptic area.

Understanding the neural mechanisms underlying sleep state transitions is a fundamental goal of neurobiology and important for the development of new treatments for insomnia and other sleep disorders. Yet, brain circuits controlling this process remain poorly understood. Here we identify a population of sleep-active glutamatergic neurons in the ventrolateral medulla (VLM) that project to the preoptic area (POA), a prominent sleep-promoting region, in mice. Microendoscopic calcium imaging demonstrate that these VLM glutamatergic neurons display increased activity during the transitions from wak

Hypocretins (orexins): The ultimate translational neuropeptides.

The hypocretins (Hcrts), also known as orexins, are two neuropeptides produced exclusively in the lateral hypothalamus. They act on two specific receptors that are widely distributed across the brain and involved in a myriad of neurophysiological functions that include sleep, arousal, feeding, reward, fear, anxiety and cognition. Hcrt cell loss in humans leads to narcolepsy with cataplexy (narcolepsy type 1), a disorder characterized by intrusions of sleep into wakefulness, demonstrating that the Hcrt system is nonredundant and essential for sleep/wake stability. The causal link between Hcrts

Comparison of upper-airway evaluations during wakefulness and sleep.

The location of upper-airway obstruction during sleep has been predicted by using waking measures that include physical examination and endoscopic Müller's maneuver. However, this prediction remains speculative. To objectively evaluate these clinical measures, 22 patients with severe obstructive sleep apnea had upper-airway studies during nonsedated sleep with solid-state manometry and videoendoscopy. Objective tongue-base obstruction was measured with four methods, each identifying a different aspect of airway collapse during sleep. Manometry identified tongue obstruction during (1) early and

Late feeding in the active period decreases slow-wave activity.

AIMS: Sleep and feeding behaviors closely interact to maintain energy homeostasis. While it is known that sleep disorders can lead to various metabolic issues such as insulin resistance, the mechanism for this effect is poorly understood. We thus investigated whether different feeding rhythms during the active period affect sleep-wake regulation. MAIN METHODS: For 2weeks, mice were randomly assigned to 1 of 3 feeding schedules as follows: free access to lab chow during the active period (ZT12-24, Ad-lib group), free access to lab chow during the first half of the active period (ZT12-18; Morni

Towards improving the prognosis of stroke through targeting the circadian clock system.

Rhythmicity of the circadian system is a 24-hour period, driven by transcription-translation feedback loops of circadian clock genes. The central circadian pacemaker in mammals is located in the hypothalamic suprachiasmatic nucleus (SCN), which controls peripheral circadian clocks. In general, most physiological processes are regulated by the circadian system, which is modulated by environmental cues such as exposure to light and/or dark, temperature, and the timing of sleep/wake and food intake. The chronic circadian disruption caused by shift work, jetlag, and/or irregular sleep-wake cycles

Exploring the electrocardiogram as a potential tool to screen for premotor Parkinson's disease.

The aim of this study was to test the hypothesis that patients with REM sleep behavior disorder, many of whom will develop Parkinson's disease (PD) or a related synucleinopathy, will demonstrate decreased heart rate variability (HRV) compared with a group of age-matched controls as measured by an electrocardiogram during wakefulness. We compared HRV in 11 untreated idiopathic REM sleep behavior disorder patients (9 men and 2 women; mean age, 63.3 years; SD, 7.5 years) and 11 control subjects with idiopathic insomnia without REM sleep behavior disorder (7 men and 4 women; mean age, 59.5 years;

The gut vagal sensory pathway drives postprandial sleep via activation of PVH-projecting GABAergic neurons in the NTS.

Feeding status bidirectionally modulates sleep; however, the neural circuitry that integrates the sensing of gastrointestinal (GI) state and sleep remains poorly understood. The afferent fibers of the vagus nerve extensively innervate the GI tract, transmitting postprandial satiety signals to the brain. This study investigates the key role of the upper gut-innervating vagal sensory neurons in modulating sleep-wake states and promoting postprandial sleep, uncovering the underlying circuit mechanisms. Both feeding and activation of stomach/duodenum-innervating vagal sensory neurons reduce wakefu

Relational hypnotherapy, lucid dreaming, and sleep paralysis: A case study.

Sleep paralysis (SP) is understood as a form of temporary muscle paralysis that occurs during the transition between rapid eye movement (REM) sleep and wakefulness. SP is normally associated with vivid, fear-provoking hallucinations such as being threatened or attacked by home invaders and paranormal encounters like alien abductions. SP affects about 20% of the population, and it is a common symptom of narcolepsy, a disorder that disrupts regular sleep cycles and causes sleep deprivation. This article is intended to illuminate the use of relational hypnotherapy in conjunction with lucid dreami

Pathophysiology of central sleep apneas.

The transition from wake to sleep is accompanied by a host of physiologic changes, which result in major alterations in respiratory control and may result in sleep-related breathing disorders. The central sleep apneas are a group of sleep-related breathing disorders that are characterized by recurrent episodes of airflow reduction or cessation due to a temporary reduction or absence of central respiratory drive. The fundamental hallmark of central sleep apnea (CSA) disorders is the presence of ventilatory control instability; however, additional mechanisms play a role in one or more specific m

Purkinje cell dysfunction causes disrupted sleep in ataxic mice.

Purkinje cell dysfunction disrupts movement and causes disorders such as ataxia. Recent evidence suggests that Purkinje cell dysfunction may also alter sleep regulation. Here, we used an ataxic mouse model generated by silencing Purkinje cell neurotransmission (L7Cre;Vgatfx/fx) to better understand how cerebellar dysfunction impacts sleep physiology. We focused our analysis on sleep architecture and electrocorticography (ECoG) patterns based on their relevance to extracting physiological measurements during sleep. We found that circadian activity was unaltered in the mutant mice, although thei

[Daytime sleepiness and driving behaviour].

Daytime sleepiness is reported by 10-15 % within the general population of industrialised countries. According to federal statistics in Switzerland, only ~1.5 % of motor vehicle crashes are caused by excessive daytime sleepiness, which is in sharp contrast to the scientific literature, indicating a figure of 10 to 30 %. This is most likely related to the difficulty to detect the underlying sleepiness by police officers and their low awareness of this cause in case of car accidents. As a consequence of this massively reduced figure, the real problem is underestimated and countermeasures as well

Sleep-Related Eating Disorder and Sexsomnia; Two Rare Parasomnias? A Mini-Review with Illustrative Case-Reports.

OBJECTIVES: Sleep-related eating disorder and sexsomnia are non-rapid eye movement (NREM) parasomnias, characterized by reward-seeking behaviors during NREM sleep. These conditions are likely underpinned by specific patterns of sleep-wake dissociation and appear to be more common than expected, leading to significant health risks, psychological distress and even forensic consequences. METHODS: We briefly review recent literature on sleep-related eating disorder and sexsomnia, and present three cases of each; for illustrating their clinical features and complexity. RESULTS: Genetic predisposi

Afraid and Awake: The Interaction Between Trauma and Sleep in Children and Adolescents.

Traumatic experiences and sleep disturbances are both common in children and adolescents. Because of the reciprocal relationship between sleep complaints and trauma, a mental health evaluation should include not only an assessment of posttraumatic stress disorder and other trauma symptoms but also a specific evaluation of sleep-related complaints. Similarly, if a history of both trauma and sleep complaints is identified, an effective trauma-informed intervention, whether psychological, psychopharmacologic, or a combination of the two, should directly address sleep issues.
