Mucocele of neurosurgical interest: clinical considerations on five cases.

The authors present five patients with mucocele, submitted to neurosurgery. Two had mucoceles spreading from the frontal and/or the anterior ethmoidal sinuses and had only compressive mass symptoms, either on the ocular globe or on the frontal lobe or on both. Three patients had mucoceles growing from the sphenoid and/or posterior ethmoidal sinuses. In these latter, the mass symptoms were less evident. All the patients suffered excruciating retro-ocular pain and two presented cranial nerve damage. The correct diagnosis in these cases is crucial to avoid a too aggressive treatment since these p

Endoscopic management of paranasal sinus mucocoeles.

Mucocoeles of the paranasal sinuses are relatively uncommon and in the past have been generally treated by an external surgical approach. Forty-eight mucocoeles in the frontal, fronto-ethmoidal and sphenoidal sinuses have been treated during the last five years, 20 by an entirely endonasal endoscopic approach and 28 by a combination of an external procedure and an endoscopic approach. There were no recurrences in the endoscopic group, with a mean follow-up of 34 months whilst three recurrences occurred in the combined external and endoscopic group which had a mean follow-up of 44 months. This

Quantitative histology of the normal sphenoidal sinus.

From 32 normal sphenoidal sinuses in 16 patients the mucosa was removal at autopsy, stained by the PAS-alcian blue whole mount method, and the density of goblet cells as well as of mucous glands was determined. The median density of goblet cells was 6200 cells per mm2, without statistically significant differences between the walls. The median density of glands was very low, 0.06 gland/mm2, highest in the anterior wall which has 0.1 gland/mm2 and lowest in the posterior wall which has 0.05 gland/mm2. The total gland count was less than 50 in 94% of the sinuses, 51--80 in 4%. The glands were sm

Sensitivity of Sinus Radiography Compared to Computed Tomogram: A Descriptive Cross-sectional Study from Western Region of Nepal.

INTRODUCTION: Radiography of the paranasal sinuses is commonly used diagnostic modality. However, the trustworthiness of plain radiographic findings of paranasal sinuses is debatable. The intention of this study was to weigh the diagnostic soundness of plain radiograph of the paranasal sinuses to that of computed tomogram scan. METHODS: This is a descriptive cross sectional study carried out in 110 participants in Department of Radiology of Gandaki Medical College from November 2017 to April 2018. Ethical approval is obtained from Institution review board (Ref. No.39/074/075). Sample size was

Incidental findings in paranasal sinuses and mastoid cells: a cross-sectional magnetic resonance imaging (MRI) study in a pediatric radiology department.

PURPOSE: Misdiagnosis of "sinusitis" is still frequent in children, although mucosal swelling in the paranasal sinuses of children has been reported as a common incidental finding. Recent radiological publications on the problem are rare. We prospectively evaluated the mucosal thickening in the paranasal sinuses and mastoid cells of children who underwent MRI of the head for reasons other than sinusitis or mastoiditis. MATERIALS AND METHODS: 147 patients, 0.2 - 22.7 years, median 8.9. Axial and sagittal T2 TSE images were evaluated by two experienced pediatric radiologists. Categories for the

Sphenoid sinus in the Holstein cow: A sectional and 3D reconstructive anatomical study.

Cattle disorders affecting the sphenoid sinus are underreported, likely due to difficulties in imaging and lacking topographic knowledge. This study aimed to provide a detailed description of the normal anatomical and morphometric features of the cattle sphenoid sinus. Twelve healthy adult Holstein cow heads were used, and the sinus was examined using CT, anatomical sectioning and 3D modelling. The sinus was bilaterally detected in all animals but exhibited structural asymmetry and significant interindividual differences in morphological characteristics. Three parts of the sinus were defined,

The sphenoiditis spectrum.

This retrospective, comparative study involves 13 patients with sphenoiditis from 2 specialist centres: the Royal National Throat, Nose and Ear Hospital, Gray's Inn Road (7 patients) and the National Hospital for Nervous Diseases, Queen's Square (6 patients). It was found that the ENT patients had a very much longer history, had more nasal symptoms, and suffered none of the neurological complications attributable to the disease. All the neurological patients presented with a complication after a comparatively short history and had few nasal symptoms. One ENT patient was found to have a mucopyo

Undifferentiated sarcoma of the sphenoid sinus.

Paranasal sinuses sarcomas are rare and no treatments have been established. We report a young-adult case of sphenoid sinus sarcoma treated by carbon-ion radiotherapy. The patient presented with progressive left-sided visual impairment. A tumor was then identified and partial resection by transnasal approach was performed. The resected mass showed typical morphology of mesenchymal tumor, and morphological and molecular analyses ruled out a predominant-differentiation phenotype. The pathological diagnosis was undifferentiated sarcoma. The residual lesion was treated with carbon-ion radiotherapy

Visual loss secondary to eosinophilic mucin rhinosinusitis in a woman: a case report.

INTRODUCTION: Eosinophilic mucin rhinosinusitis is an inflammatory pathological condition of the nose and paranasal sinuses. It is rare, occurs in immunocompetent patients and is characterised by peripheral eosinophilia and extensive bilateral sinus disease. To the best of our knowledge, visual loss with this condition has not been previously reported. CASE PRESENTATION: We present the case of a 26-year-old Asian woman with a background history of chronic sinusitis who presented with acute left-sided visual loss. Imaging showed significant opacification in the frontal, ethmoidal and sphenoida

The significance of evaluating sphenoid sinus fluid by postmortem computed tomography in cases of drowning.

PURPOSE: In cases of drowning, the presence of sphenoid sinus fluid is a non-specific autopsy finding. However, studies have reported that fluid accumulation in the paranasal sinuses is more commonly observed in drowning victims. Furthermore, some laboratory tests, such as diatom and electrolyte analysis, can serve as supplementary diagnostic tools for diagnosing drowning. Therefore, accurate sphenoid sinus fluid sampling is an important aspect of an autopsy in suspected drowning cases. The aim of this study was to identify the significance of evaluating sphenoid sinus fluid by PMCT images in

The drainage system of the paranasal sinuses: a review with possible implications for balloon catheter dilation.

BACKGROUND: Intersinus connections and accessory ostia of the maxillary sinus are well known to rhinologic surgeons but are less known for the remaining paranasal sinuses. Probing and dilatation of the natural ostia of diseased sinuses is being popularized currently by the clinical value of balloon catheter dilation (BCD). Although short-term high success rates with treatment of the dilated ostia/duct have been reported with this technique, the potential impact of fractures of adjacent bony septae on accessory sinus ostia, on mucosally lined surfaces, and on intersinus drainage pathways still

Invasive Klebsiella syndrome with coexisting fungal endophthalmitis.

Endogenous endophthalmitis accounts for approximately 5 - 10% of all endophthalmitis cases. We report a case of a middle-aged gentleman with underlying uncontrolled diabetes mellitus who presented with fever and generalised body weakness for one week. He was diagnosed with invasive Klebsiella syndrome based on blood culture with presence of bilateral pleural effusion, liver abscess, renal impairment and sphenoidal sinusitis. The patient developed sudden bilateral painless reduced vision on day two of admission. Ocular examination revealed bilateral severe anterior chamber reaction and severe v

Management of multiple spontaneous nasal meningoencephaloceles.

OBJECTIVES/HYPOTHESIS: Multiple spontaneous nasal meningoencephaloceles in the same patient are rare lesions. Although many skull base defects occur after prior trauma or surgery, otolaryngologists must be aware of the potential for spontaneous encephaloceles. We present our experience with this unusual condition and discuss its pathophysiology and unique management issues. STUDY DESIGN: Retrospective. METHODS: Review of medical records, radiographic images, and cerebrospinal fluid pressures. RESULTS: We identified 5 patients with multiple, simultaneous, spontaneous encephaloceles: 4 patien

Sphenoethmoidal mucocele masquerading as trochlear palsy.

Acquired vertical strabismus is commonly caused by superior oblique muscle palsy, often resulting from blunt head trauma or vascular problems, and less often from brain tumors, meningitis, and aneurysms. To date, mucoceles in the ethmoid sinus have rarely been reported as a cause for superior oblique muscle palsy. We report a case of trochlear nerve palsy and subsequent optic neuropathy caused by a mucocele in the ethmoid and sphenoid sinuses.
