Physicians and psychologists almost always treat premature emission or ejaculation as a psychological problem. We report on 6 men who had sudden onset of severe premature emission after spinal cord injury. All men suffered spinal cord injury related to trauma at the T12-L1 level, and had areflexic bladders after the injury. Two men had normal penile sensation. None of the men had premature ejaculation before injury with average time of intercourse of 10 to 20 minutes before organsm. After the injury they noticed emission before or at vaginal entry. Two patients reported emission with any sexua

A review of 1300 patients with spinal cord injury (SCI), over a period of 14 years, revealed 12 patients with an 'acute abdomen'. Seven events occurred during the initial admission, ranging from 10 days to 9 months from injury, and five during readmission of 'chronic' SCI patients. Four were in the acute stage 10-30 days from injury, all with peptic ulcer perforations. The remainder had either an intestinal obstruction, appendicitis or peritonitis. All of the neurological levels were above T6 except for one patient who had a low level paraplegia. The classical signs of an 'acute abdomen' may b

This article describes an outbreak of scabies on a 30-bed acute spinal cord injury unit and the measures taken to eradicate the infestation. Because spinal cord injury patients have sensorimotor deficits, the sensation of itching often is absent, making an infestation of scabies more difficult to identify. A total of 30 patients and 38 employees were exposed and treated for scabies. Educational programs about scabies were instituted immediately for all employees. Routine daily activities and therapy schedules were severely hampered by the outbreak. Despite the efforts of staff to solve the pro

UNLABELLED: Lipid peroxidation has been identified as a deleterious consequence of contusive spinal cord trauma and of thermal injury. The L3-L6 spinal cord segment was thermally injured using a radiofrequency heating chamber mounted on the vertebral column of anesthetized rats. Hind limb function was assessed 2 hours later. A bolus of methylprednisolone (MP, 30 mg/kg) was then given intravenously, followed by 5.4 mg/kg/hr MP for 6 or 24 hours. Cord water content and regional spinal cord blood flow (RSCBF, 14C-butanol distribution) were measured at seven cord levels after function had been rea

This overview of the management of children with spinal cord injury has focused on a program based on bladder dynamics. The bladder is modulated pharmacologically and surgically, if necessary, to create a reservoir that stores urine at low pressure, and emptying is achieved by clean intermittent catheterization. The parent assumes the responsibility for bladder emptying when the child is unable to perform catheterization. Reflex voiding in a balanced fashion is not advocated. The program described not only avoids the deleterious effects of high voiding pressures, it also obviates the wearing o

The relative efficacy of intermittent catheterisation, self or assisted, over indwelling catheterisation was studied on 44 patients of different extent of spinal cord lesions. All the surviving cases (9) of incomplete cord lesion became catheter free irrespective of the method of catheterisation. In these patients urine was also found to be infection free by the 5th week of admission. Twenty-seven cases of complete cord lesion (out of a total of 35) became catheter free, of whom 18 patients were on intermittent catheterisation (out of a total of 20) and 9 patients were on indwelling catheter (

Oesophageal perforations associated with cervical fractures occur from a variety of injuries. Fractures of the cervical spine, blunt trauma and penetrating injuries such as gunshot wounds, knives and missiles, perforate the cervical oesophagus. This retrospective study consists of 24 patients with an oesophageal perforation and cervical fracture. Motor vehicle accidents were responsible for 54% of the oesophageal perforations. The other oesophageal injuries were related to anterior spine surgery, gunshot wounds and sports-related activities. The clinical features related to these injuries incl

Between January 1, 1970, and December 31, 1988, 179 children (birth to age 16) were treated for spinal cord and/or vertebral column injury b … ative management had no bearing on neurological outcome. Twelve percent of the children with severe spinal cord injuries developed posttraumatic spinal deformity. We conclude that spinal injury patterns differ between preadolescent and older children. Most injuries can be successfully managed with nonoperative therapy. Prognosis is primarily correlated with the severi …

Loss of normal bladder function results in increased risk of developing urinary tract infection. Recurrent bacteriuria continues to be a problem in most spinal cord injured persons regardless of the bladder emptying method used. Lower urinary tract complications have decreased with intermittent catheterization, but the effect of increased intravesical pressure and presence of bacteriuria on renal function are still undetermined. Current methods of neurogenic bladder management are often based on professional or institutional presuppositions rather than scientific data. Although there are many

A case of cancer of urinary bladder in a 39-year-old incomplete C7 paraplegic male is reported. He was injured in 1962, and was admitted to our department in August, 1982 because of macrohematuria. Intravenous pyelography showed dilation of right ureter and pelvis. Cystography revealed filling defect on the right wall of bladder, but vesicoureteral reflux was not seen. Endoscopically, we found the tumor on the right wall, which seemed to invade to the trigone and right ureteral orifice. CT scan and pelvic angiography showed that the tumor extended extramurally of the bladder and metastasized t

We evaluated 23 male spinal cord injury patients who underwent percutaneous nephrolithotomy for the success rate of stone removal and the incidence of operative complications. There were 18 quadriplegic and 5 paraplegic patients, and 5 had bilateral procedures. Of the kidneys 7 had staghorn calculi, 8 had pelvic and caliceal combinations, 6 had large multiple caliceal stones, 4 had large (more than 2.5 cm.) pelvic stones and 3 had pelvic stones less than 2.5 cm. Placement of a nephrostomy tube and stone extraction were performed as a single procedure with the use of general anesthesia in all b

After return of the reflex arc below the level of anaesthesia, the spinal cord injury (SCI) patient will manifest an intra-abdominal emergency by the clinical signs of dysreflexia depending upon the level and completeness of the cord lesion. Thirty-six SCI patients are presented to correlate the autonomic response to visceral disease with the level of their cord lesion demonstrating that early recognition and diagnosis is possible in these patients.

Six spinal cord injury patients with chronic prostatitis were reviewed, all of whom had been treated with an indwelling Foley catheter during the phase of spinal shock. The 3 glass urine specimen test, the bladder wash-out test, a study of antibody coated bacteria and urethrography had limited diagnostic value. A specific diagnostic 5 glass specimen test proved to be useful and reliable. Longterm antibiotic treatment was successful in only one patient. Injection of antibiotics into the prostate gland was ineffective in the five patients in whom it was carried out. During a follow up from 1 to

Mechanical trauma to the cervical spine still occurs at birth. In 2 of 48 perinatal postmortems traumatic damage to the cervical spinal cord was found. Also in this series at least 12% of cases from one hospital showed some degree of trauma to the cervical spine but this was of a lesser degree in individual cases than 20 years ago.

During the course of hospitalization for comprehensive rehabilitation, a 14-year-old boy with C4 spinal cord injury and hypercalcemia was treated with 5 different treatment regimens which were instituted to reduce the hypercalcemia and associated complications. These regimens included low calcium diet, steroids, oral phosphates, intravenous saline, diuretics, hyperalimentation, calcitonin and spironolactone in various combinations, and mobilization. Careful metabolic monitoring carried out throughout hospitalization permitted the evaluation in retrospect of the impact of each treatment regimen

We studied the effect of the opiate antagonist naloxone on the recovery of cats injured with a 400-g-cm impact injury to T-9. The animals were evaluated by recording somatosensory evoked potentials and performing weekly neurological examinations. Several dose schedules were followed. Six of eight cats that received an intravenous or intraperitoneal bolus of naloxone (10 mg/kg) 45 minutes after injury regained the ability to walk. Recovery occurred in only one of five animals that were treated with an infusion of naloxone, 10 mg/kg/hour, and in none of five animals given 1 mg/kg as a bolus. Bec
