SCO-spondin knockout mice exhibit small brain ventricles and mild spine deformation.

Reissner's fiber (RF) is an extracellular polymer comprising the large monomeric protein SCO-spondin (SSPO) secreted by the subcommissural organ (SCO) that extends through cerebrospinal fluid (CSF)-filled ventricles into the central canal of the spinal cord. In zebrafish, RF and CSF-contacting neurons (CSF-cNs) form an axial sensory system that detects spinal curvature, instructs morphogenesis of the body axis, and enables proper alignment of the spine. In mammalian models, RF has been implicated in CSF circulation. However, challenges in manipulating Sspo, an exceptionally large gene of 15,71

Less-Invasive Approach to Early-Onset Scoliosis-Surgical Technique for Magnetically Controlled Growing Rod (MCGR) Based on Treatment of 2-Year-Old Child with Severe Scoliosis.

BACKGROUND: Spinal deformities in children can be caused by various etiologies, such as congenital, syndromic, neuromuscular, or idiopathic. Early-onset scoliosis (EOS) is diagnosed before the age of ten years, and when the curvature continues to progress and exceeds a Cobb angle of 60-65 degrees, surgical treatment should be considered. Initial minimally invasive surgery and the implantation of magnetically controlled growing rods (MCGRs) allows for the noninvasive distraction of the spine, growing, and avoids multiple operations associated with the classic distractions of standard growing ro

Influence of body mass index on the reliability and validity of ultrasound assessments in adolescent idiopathic scoliosis: An original research.

OBJECTIVES: Clinical ultrasound provides a non-invasive method to assess spinal curvature in adolescent idiopathic scoliosis (AIS). However, the reliability and validity of ultrasound assessment may be affected by body mass index (BMI). This study investigated the impact of BMI on the reliability and validity of ultrasound assessments in AIS. DESIGN: 165 participants with suspected AIS were recruited for both ultrasound and radiographic assessments. Lateral spinal curvature was measured on ultrasound imaging using the spinous process method and on X-ray using the Cobb method. The same operato

Automating Scoliosis Measurements in Radiographic Studies with Machine Learning: Comparing Artificial Intelligence and Clinical Reports.

Scoliosis is a condition of abnormal lateral spinal curvature affecting an estimated 2 to 3% of the US population, or seven million people. The Cobb angle is the standard measurement of spinal curvature in scoliosis but is known to have high interobserver and intraobserver variability. Thus, the objective of this study was to build and validate a system for automatic quantitative evaluation of the Cobb angle and to compare AI generated and human reports in the clinical setting. After IRB was obtained, we retrospectively collected 2150 frontal view scoliosis radiographs at a tertiary referral c

How different the scoliotic curves are?

The pathomechanism of spinal deformity development in adolescent idiopathic scoliosis (AIS) has been related to the sagittal curvature of the spine. It is not known how the variations in the sagittal profile relates to the coronal deformity patterns in AIS. A total of 70 Lenke 1 and 50 Lenke 5 AIS patients were included retrospectively. A finite element (FE) model was developed for each spine where the sagittal spinal curvatures were modeled as 2D S shaped elastic rods. Transverse plane deformation patterns of these rods under physiological loading were determined and clustered based on their …

Vertebral pelvic angles accurately predict risk of proximal junctional kyphosis following long-segment fusion for adult spinal deformity.

PURPOSE: VPAs quantify the spatial relationship of each vertebra to the pelvis and are independent of patient positioning, making them a valuable tool for preoperative planning and intraoperative assessment of sagittal alignment. Given the biomechanical relevance of vertebral pelvic angles (VPAs), their role in predicting proximal junctional kyphosis (PJK) warrants further investigation. We hypothesized that malalignment of VPAs is associated with an increased risk of developing PJK following long-segment fusion for adult spinal deformity (ASD). METHODS: ASD patients ≥ 18 years undergoing ≥ 5

Return to Golf after corrective surgery in adult spinal deformity patients with long fusion constructs: A retrospective case series.

PURPOSE: Golf is a popular sport that requires truncal strength and spinal flexibility. Spinal fusion surgery limits range of motion which may impact a golfer's ability to return to golf (RTG). The purpose of this study is to report clinical outcomes and return to golf following long-construct spinal fusion surgery. METHODS: Patients fused from the pelvis to T10 or higher were screened for golfing participation between November 2023 - April 2024. Inclusion criteria included: ≥18 years old, >2 year minimum follow-up, active golfers. Visual analogue scale (VAS) back pain level, self-perceived g

Long-term mechanical failure in well aligned adult spinal deformity patients.

BACKGROUND CONTEXT: Mechanical complications (MC) are frequently linked to suboptimal postoperative alignment and represent a primary driver of revision surgery in the context of adult spinal deformity (ASD). However, it's worth noting that even among those deemed "well aligned," the risk of experiencing MCs persists, hinting at the potential influence of factors beyond alignment. PURPOSE: The aim was to assess the incidence of MCs among well-aligned patients and delving into the relevant risk factors and surgical outcomes that come into play within this specific subgroup. The CAPN3 p.Lys 254del variant is not always associated with dominant CAPN3-related muscular dystrophy.

INTRODUCTION/AIMS: Limb-girdle muscular dystrophy R1 (LGMDR1) calpain 3-related usually presents as a recessively transmitted weakness of proximal limb-girdle muscles due to pathogenic variants in the CAPN3 gene. Pathogenic variants in this gene have also been found in patients with an autosomal dominantly inherited transmission pattern (LGMDD4). The mechanism underlying this difference in transmission patterns has not yet been elucidated. Camptocormia, progressive limb weakness, myalgia, back pain, and increased CK levels are common clinical features associated with dominant forms. The p.Lys2

Upper camptocormia in Parkinson's disease: Neurophysiological and imaging findings of both central and peripheral pathophysiological mechanisms.

BACKGROUND: Camptocormia is a disabling complication of Parkinson's disease (PD), but its pathophysiology is poorly elucidated. Depending on the fulcrum of forward trunk flexion, two subtypes have been defined, upper (UCC) and lower camptocormia, the former being much more frequent. The aim of the study was to explore possible pathophysiological mechanisms of PD-related UCC. METHODS: Ten PD patients with UCC (UCC-PD) and ten PD patients without camptocormia (NoUCC-PD) underwent simultaneous electromyography (EMG) of thoracic paraspinal (TPS), obliquus externus abdominis (OEA), rectus abdomini

Effect of Cage Type on Short-Term Radiographic Outcomes in Transforaminal Lumbar Interbody Fusion.

BACKGROUND: The effect of expandable versus static cage type on radiographic outcomes in transforaminal lumbar interbody fusion (TLIF) has not been well studied in the literature. We studied the effect of 3 cage types on change in foraminal height (FH), disk height (DH), subsidence, and segmental lordosis (SL). METHODS: We conducted a retrospective review of patients who underwent TLIF in a single institution from 2014 to 2019. The following 3 cage types were identified: banana cage, bullet, and expandable cage. Computed tomography (CT) scans of the lumbar spine or lumbar radiographs (when CT

Sacropelvic Fixation: When, Why, How?

The indications for sacropelvic fixation continue to evolve with emerging instrumentation technologies and advancing techniques. Common indications include long construct fusions, high-grade spondylolisthesis, sacral fractures, sacral tumors, and global sagittal and/or coronal imbalance among others. The authors' preferred technique is through use of a freehand S2-alar-iliac screw placement.

The Free-Hand Technique for S2-Alar-Iliac Screw Placement: A Safe and Effective Method for Sacropelvic Fixation in Adult Spinal Deformity.

BACKGROUND: Spinopelvic fixation is an integral part of achieving solid fusion across the lumbosacral junction, especially in deformity procedures requiring substantial correction or long-segment constructs. Traditional S2-alar-iliac (S2AI) screw-placement techniques utilize fluoroscopy, increasing operative time and radiation exposure to the patient and surgeon. We describe a novel free-hand technique for S2AI screw placement in patients with adult spinal deformity. METHODS: We reviewed the records of 45 consecutive patients who underwent spinopelvic fixation performed with use of S2AI screw

Intraoperative motor evoked potential monitoring to patients with preoperative spinal deficits: judging its feasibility and analyzing the significance of rapid signal loss.

BACKGROUND CONTEXT: Transcranial motor evoked potential (MEP) monitoring has been widely adopted in spine surgery, but so far the useful monitoring data for patients with preoperative spinal deficits (PPSDs) are limited. Originally we thought that they seemed technically more difficult and less reliable in performing the MEP monitoring to PPSDs. PURPOSE: Our objective was to study (1) the feasibility of MEP monitoring in PPSDs and the (2) the significance of rapid MEP loss. STUDY DESIGN/SETTING: A retrospective case notes study from a prospective patient register was used as the study design
