[Non-tuberculous infections of the spine].

Non tuberculous spondylodiscitis of the rachis is an uncommon entity that affects boys and male adults with greater frequency. The zone with higher incidence of affection is the lumbar region. Usually the causal germ travels through the hematogenous via from an extrarachidial infectious area. Clinical manifestations begin with an acute segmentary rachialgia and paravertebral muscular contracture while presenting fever. In special cases of little boys or when having radiated pain, the presenting form could be confusing. There is always a vertebral rigidity when performing the physical exam and

Anterolisthesis in the cervical spine--spondylolysis.

Two cases of cervical spondylolisthesis are reported, and the relevant English language literature is reviewed. Analysis of data derived from all of the cases reported revealed no statistically significant etiological or prognostic factors. The roentgenological evaluation, including associated anomalous findings, is presented. Etiological and clinical aspects of cervical spondylolisthesis are discussed. It is concluded that cervical spondylolysis is a relatively rare congenital defect. The role of manipulative therapy in the treatment of associated symptoms is discussed.

[Pedicle agenesis in the cervical spine].

Congenital absence of a cervical pedicle is rare, and because of the associated enlargement of the intervertebral foramen, may be confused with a "dumbbell" tumour. A recent observation of this anomaly has prompted this report. Based on the findings of 29 reported cases in the literature, the characteristic radiological features of the abnormality are described and the differential diagnosis as well as the clinical implications of this disorder are discussed.

[Injuries of the cervical spine in Switzerland].

The post traumatic evaluation of patients after soft tissue injuries of cervical spine is not clear. As objective criteria of the injury are often absent, the judgement in the medical expertise is difficult. 1100 medical records as a representative sample of 11,000 patients with cervical spine injury recorded at the Swiss National Insurance Company during the years 1978-81 were analysed. 320 records were evaluated in detail. Those injured received a questionnaire about their present psychosocial situation, complaints, necessity of treatment and financial compensation. 207 or 65% returned the f…

Hydatid disease of the cervical spine.

Hydatid disease affecting the cervical spine occurred in a 31-year-old man. Because bone grafting is associated with a high failure rate, treatment included filling the excised defect with a high-density polyethylene implant and fusing the cervical spine to stabilize it.

Classification of injuries to the thoracolumbar spine.

Significant advances have been made over the past 40 years in the understanding of the pathoanatomy, biomechanics, and neurophysiology of thoracolumbar injuries. These improvements are reflected in the sophisticated classifications currently in use. Yet, we remain in a transitional phase of our classification of these fractures. Future schemes will probably offer two precise classifications--one neurologic and one structural--for every injury.

Fractures of the thoraco-lumbar spine.

A personal prospective study of 98 consecutive patients presenting with neurological impairment and fractures or dislocations between the 9th thoracic and 2nd lumbar vertebrae bodies. Fifty-three patients underwent Harrington instrumentation, and 45 patients were treated recumbently. Neurological improvement was much better following Harrington rods in the complete paraplegia group but there was no difference in neurological recovery between the two groups in those with incomplete paraplegia. Forty-two patients who had been stabilised with Harrington rods underwent post-operative myelography o

Late stability of the cervical spine.

Radiological criteria, laid down to detect instability of the cervical spine after cadaveric biomechanical studies, were applied to 52 cases of cervical spine injury. Only one case of instability was detected, and late neurological signs resolved after posterior cervical fusion. Three other cases which fulfilled the radiological criteria of instability clinically were symptom free. It is concluded that these criteria alone cannot establish the indications for surgical intervention.

[Treatment of secondary cancer of the spine].

Sixty spinal metastases have been treated surgically. In 22 cases a posterior approach was made with decompression laminectomy and internal fixation. In 38 others an anterior approach was made to resect the vertebral body and replace it by cement. In the thoracic and lumbar spines, internal fixation after a posterior approach led to secondary displacement in five cases out of nine. In contrast, spines approached anteriorly remain stable. The results were analysed after a follow-up period ranging from six months to four years. In most of the cases there was a dramatic improvement in spinal and …

Aneurysmal bone cyst of the spine.

The cases of twenty-two patients with an aneurysmal bone cyst of the spine above the sacrum were analyzed with regard to sex, age, site, symptoms, and radiographic findings. Four patients had extension of the lesion to the adjacent vertebra or rib, and twelve patients had neurological deficits. The primary treatment was either radiotherapy or surgery alone, or surgery and radiotherapy combined. No recurrences were found in patients who were treated with surgery alone or with surgery and radiotherapy, while three of the six patients who were treated with radiotherapy had a local recurrence, two…

Stabilisation of the spine affected by malignancy.

A technique for securely stabilising the spine affected by malignancy has been developed. It provides excellent pain relief and some improvement of neurological deficit, and allows of laminectomy and radiotherapy. Although the technique is relatively simple, and the materials used are commonplace and inexpensive, it needs to be learnt which may mean that surgery for spinal malignancy will need the service of a trained surgeon, rather than that of an initiate.

Primary osteogenic sarcoma of the spine.

Primary osteogenic sarcoma of the spine is a rare tumor making up 0.85% to 2.0% of all osteogenic sarcomas. The majority (29/41) of previously reported cases have occurred in vertebrae affected by Paget disease or radiation exposure. Two cases of osteogenic sarcoma of the spine, one lumbar, the other thoracic, with no known predisposing factors are presented.

Tuberculosis of the upper cervical spine.

Six patients, aged between 3 and 51 years, with tuberculosis of the upper cervical spine were studied. Prominent features of the disease included pain and stiffness, paralysis, swelling of the retropharyngeal soft tissue, osteolytic erosions, and atlanto-axial subluxation. Cure was obtained with antibiotics, transoral decompression and C1-2 fusion.

[Hemothorax and fractures of the dorsal spine].

The frequency of pleural complications was examined in 72 patients with recent fractures of the thoracic spine. 17 patients (24%) were found to have pleural hemorrhage. In 8 cases this was the result of isolated spinal fracture, without fracture of the ribs. In these cases, posterior mediastinal effusion, which was readily visible on chest films, always occurred prior to or concomitantly with the pleural effusion. Most of the spinal fractures were located between the fourth and eighth vertebrae. Bone lesions were always serious unstable fractures with vertebral collapse and kyphosis. Acute res…

CT evaluation of the lumbosacral spine.

The biologic cost-effectiveness of computed tomography (CT) versus myelography is so favorable to CT that it is now the method of choice for evaluating patients with less clear-cut clinical findings. CT is now used to detect lesions formerly difficult to diagnose, such as subluxation, arthrosis, facet osteophytes, and stenosis of the vertebral canal, as well as herniated disks and lateral disks. The findings in over 1,000 patients examined by CT for lumbosacral spinal pathology are documented. Associated or multiple abnormalities were present in about 60% of cases, with bulging or herniated di
