[Disseminated strongyloidiasis associated with pulmonary fibrosis and gastrointestinal bleeding].

Strongyloidiasis is a parasitic disease that can be lethal in immunosuppressed patients especially if lung fibrosis is associated, which has been rarely reported. We report the case of a patient treated with corticosteroids for pulmonary fibrosis who developed disseminated strongyloidiasis revealed by massive gastrointestinal bleeding. Diagnosis was based on histological analysis of gastrointestinal biopsies and broncho-alveolar lavage. Treatment improved digestive features as well as respiratory function.

[Disseminated strongyloidiasis, a rare cause of multiple organ failure].

INTRODUCTION: Strongyloidiasis is an parasitic intestinal disease, frequent in tropical countries and fairly rare in temperate areas under certain conditions. Disseminated forms can provoke multivisceral damage. CASE REPORT: A 75 year-old woman, hospitalized for occlusion, developed multivisceral (cerebral, renal, cardiac pulmonary and hepatic) damage over 48 hours, attributed to strongyloidiasis. The diagnosis was confirmed by isolation of larvae in the tracheal and gastric aspiration fluids. Treatment combined ivermectin and albendazole. COMMENTS: Disseminated stronglyloidiasis is essentia…

Strongyloidiasis in a Cope's grey tree frog (Hyla chrysoscelis).

A male Cope's grey tree frog (Hyla chrysoscelis) died spontaneously with ventral subcutaneous edema and was necropsied. Thickening of the intestinal mucosa was observed histopathologically, with villous atrophy and intraepithelial nematodes present. Adult female Strongyloides sp. nematodes were isolated from the fixed intestinal tract, the first time this nematode genus has been recovered from this frog genus. Intestinal strongyloidiasis should be considered as a cause for protein-losing enteropathy and death in frogs.

Parasitic diseases in immunocompromised hosts. Cryptosporidiosis, isosporiasis, and strongyloidiasis.

Cryptosporidiosis and isosporiasis are intestinal infections caused by the protozoan parasites Cryptosporidium parvum and Isospora belli, respectively. HIV infection and other immunodeficiency diseases predispose human subjects to severe and prolonged cryptosporidiosis. There is also evidence that HIV infection predisposes to chronic isosporiasis. Strongyloidiasis is caused by a nematode worm, Strongyloides stercoralis. Administration of corticosteroids to patients with chronic low-grade S. stercoralis infection can trigger a fulminant, life-threatening form of strongyloidiasis.

[Fatal strongyloidiasis in an immunodepressed patient following renal transplantation].

Occurrence of fatal hyperinfection with Strongyloides stercoralis in an immunodepressed patient after kidney transplantation is reported. Physiopathology of the illness is discussed and the seriousness of the hyperinfection syndrome is stressed. Endoscopy with duodenal biopsy is useful for the establishment of diagnosis.

Strongyloidiasis in cotton rats (Sigmodon hispidus) from central Oklahoma.

Thirty-one of 40 cotton rats (Sigmodon hispidus) collected from central Oklahoma were infected with Strongyloides sp. (78% prevalence). Larvae of Strongyloides sp. (rhabditiform or filariform) were not demonstrable in intestinal contents and scrapings. Female nematodes recovered from intestinal contents and scrapings had morphological similarities with Strongyloides sigmodontis. Cotton rats infected with Strongyloides sp. were indistinguishable clinically from non-infected hosts. Infected animals had no significant gross lesions, but the presence of Strongyloides sp. in the intestinal mucosa w

Endemic strongyloidiasis in northern Italy: clinical and immunological aspects.

One hundred and eighteen cases of S. stercoralis infection were detected by stool examination among 4203 patients seen over a period of three years at the Division of Infectious Diseases (IRCCS, San Matteo, University of Pavia) of a University Hospital in Northern Italy. Because chronic strongyloidiasis is often asymptomatic, a 3 per cent prevalence in this selected population indicates that the parasitosis may be much more frequent in the general population of the area. Parasite-specific humoral responses were studied in 48 patients. Strongyloides-specific IgG and IgA antibodies were found by…

Severe strongyloidiasis presenting as occult gastro-intestinal tract malignancy.

A 38-year-old man presented with severe loss of weight, diarrhoea alternating with constipation and blood in the stool. Clinically, colonic malignancy was suspected, but this was not confirmed. Fresh stool contained numerous larvae of Strongyloides stercoralis. The patient responded promptly to levamisole, with cessation of symptoms and rapid weight gain.

Specific allergic sensitization to Strongyloides antigens in human strongyloidiasis.

Reaginic antibodies to antigens from the human parasite Strongyloides stercoralis were studied by histamine release from basophils in 15 patients with chronic uncomplicated strongyloidiasis and in 12 controls (six North Americans with no evidence of parasitic diseases, two patients with Schistosoma mansoni, two with hookworm, one with Ascaris lumbricoides, and one with Onchocerca volvulus). All of the patients had antibodies to the somatic larval antigens, and 14 of 15 patients also had antibodies to the excretory/secretory products of S. stercoralis larvae. None of the controls had a positive…

Humoral and cell-mediated immune responses in murine strongyloidiasis.

The immunological changes occurring after primary and challenge infections with Strongyloides ratti in C57B1/6 mice are described. Serum IgM and IgG antibodies against Strongyloides antigen appeared one week after primary infection. The levels of antibody in both immunoglobulin classes increased markedly after secondary infection and persisted for at least 6 weeks. Immediate hypersensitivity (15 min footpad) reactions were transient after a primary infection, but were marked and persistent after a secondary infection. Arthus (5 h footpad) reactions were mild and very transient after a primary

Case report: Antemortem diagnosis of central nervous system strongyloidiasis.

Strongyloidiasis is usually a benign illness confined to the gastrointestinal tract. However, dissemination (hyperinfection syndrome) may occur, particularly in patients with impaired cell-mediated immunity. The diagnosis of hyperinfection syndrome is often made postmortem, and mortality is high, even when the disease is recognized during life. Central nervous system involvement with Strongyloides stercoralis has previously been recognized in only a few cases at postmortem examination, and in one case antemortem. We describe a patient with disseminated strongyloidiasis in whom central nervous …

[Diagnosis of strongyloidiasis using the E.L.I.S.A. test (author's transl)].

An immunoenzymatic test, the E.L.I.S.A. test is used to diagnose human Strongyloidiasis. First results show that this reaction can complete other investigational methods already in use to diagnose this affection, i.e. coproanalysis and intradermal reactions, both in epidemiological studies and individual cases. In addition, the test is a good means of assessing the effectiveness of treatment.

[Electromyographic analysis of motor perturbations linked with gastrointestinal strongyloidiasis].

The effects of experimental infection of lambs with third stage Trichostrongylus larvae are described. Preceding diarrhea, disturbances in the motility of the reticulum, abomasum and jejunum were observed by electromyography. Disappearance of the migrating myoelectric complexes occurred about 24 h before diarrhea and was accompanied by a decrease in both the frequency of reticular contractions and of contractions propagated through the antro-duodenal junction. The significance of these findings is discussed and compared with natural infestation in the field.
