On the importance of scientific rhetoric in stuttering: a reply to Finn, Bothe, and Bramlett (2005).

PURPOSE: To refute the alleged practice of "pseudoscience" by P. Finn, A. K. Bothe, and R. E. Bramlett (2005) and to illustrate their experimental and systematic bias when evaluating the SpeechEasy, an altered auditory feedback device used in the management of stuttering. METHOD: We challenged the experimental design that led to the seemingly predetermined outcome of pseudoscience rather than science: Limited preselected literature was submitted to a purposely sampled panel of judges (i.e., their own students). Each criterion deemed pseudoscientific was contested with published peer-reviewed …

Disfluency data of German preschool children who stutter and comparison children.

UNLABELLED: This study compared the disfluencies of German-speaking preschool children who stutter (CWS, N = 24) with those produced by age- and sex-matched comparison children who do not stutter (CWNS, N = 24). In accordance with Yairi and Ambrose's [Yairi, E., & Ambrose, N. (1992). A longitudinal study of stuttering in children: A preliminary report. Journal of Speech and Hearing Research, 35, 755-760] guidelines the CWS group had a narrow age range (2-5 years) and were seen close to the reported time of their stuttering onset (average of 8 months). Furthermore, over 95% of the CWS group had…

Phonological and language disorders in children who stutter: impact on treatment recommendations.

In the profession of speech-language pathology, it is commonly reported that children who stutter, as a group, are more likely to have phonological and language disorders than their non-stuttering peers. Some support for this belief comes from survey studies that have questioned speech-language pathologists about the children on their caseloads who stutter. Recently, one such study reported that 44% of children who stuttered had at least one additional communication disorder (Arndt & Healey, 2001). In the present investigation, speech-language pathologists (n = 127) who were treating children …

Finding good resources for treating school-age children who stutter.

One of the challenges facing school-based speech-language pathologists is finding appropriate resources for use in treating children who stutter. This article highlights several excellent resources and provides illustrations of how some of these resources may be used. The need to adapt resources for use with specific clients to maximize treatment effectiveness is emphasized.

Following up on treated stutterers: studies of perceptions of fluency and job status.

The present research aimed to investigate perceptions of fluency of persons treated with a fluency shaping technique called smooth speech. F … ared employer perceptions of their employees' speech between a group who had received treatment for stuttering and a nontreatment control. The employers' perceptions of the treatment group were significantly enhanced, whereas no significant change occurred in employers' perceptions for the control group. The last study explored possible determinants of relap …

A preliminary analysis of the ameliorative effects of time-out from speaking on stuttering.

Relatively few attempts have been made to systematically examine the processes responsible for the ameliorative effects of response-contingent stimulation (RCS) on stuttering. It was hypothesized that the reductions in stuttering that frequently accompany RCS are the result of the stutterer being encouraged to access extant fluent speech that may not be fully evident during "contingency-free" (CF) conditions. A preliminary analysis of the hypothesis was conducted by monitoring RCS and CF stuttering frequency and speaking rate in 20 adult stutterers before, during, and after a program of fluenc…

Short- and long-term outcome in an intensive treatment program for adult stutterers.

A three week intensive treatment program for adult stutterers is described. This treatment has evolved from an original program developed by Ingham and Andrews (1973) using speech prolongation techniques, gradual shaping of speech rate to normal, and systematic transfer of skills acquired in the clinic to real life situations. Immediately after intensive treatment, stuttering was virtually eliminated and speech rate and attitudes toward communication were normalized. There was no substantial deterioration in these treatment effects when clients were evaluated in the clinic after two months in …

The Link Between Syntactic Complexity and Stuttering-Like Disfluencies in French Speaking Adults.

BACKGROUND: Stuttering is a neurodevelopmental disorder characterized by speech disfluencies. While studies have shown a link between syntactic complexity and stuttering in children, its role in adults who stutter (AWS) remains unclear. This study investigates whether increased syntactic complexity correlates with stuttering-like disfluencies (SLDs) in AWS and evaluates three syntactic complexity indices. METHODS: Participants included 24 French-speaking adults (12 AWS, 12 controls), matched for age, gender, and education. Participants completed a sentence description task using sentences of …

Each Child Matters: Contributing Factors to Speech-Language Pathologists' Perceptions of Stuttering Therapy.

PURPOSE: Speech-language pathologists (SLPs) often report feeling less confident treating children who stutter. Yet, prior research has focused on SLPs' perceptions of groups, rather than an individual child who stutters. The aim of this study was to assess SLPs' perceptions of the therapy they provide to an individual child who stutters and to identify key factors underlying these perceptions. METHOD: A total of 115 SLPs rated their general experience, knowledge, and confidence related to stuttering treatment and rated how well they perceived therapy to be going for an individual child who s…

Practice for adolescent stuttering: a review of medical records from a Swedish cohort.

Background: Stuttering, during adolescence, may influence adult life. However, little is known about currently applied clinical procedures for assessment and treatment of stuttering in adolescence. Aim: The aim of the present study was to investigate clinicians' assessment and intervention routines and if choice of intervention was related to factors such as age, gender, or stuttering severity. Method: Medical records from 51 adolescents who had visited a Speech and Language Therapy clinic were included. Group differences were analysed through nonparametric Mann-Whitney U test and correlations…

Elevated iron concentration in putamen and cortical speech motor network in developmental stuttering.

Theoretical accounts of developmental stuttering implicate dysfunctional cortico-striatal-thalamo-cortical motor loops through the putamen. However, the analysis of conventional MRI brain scans in individuals who stutter has failed to yield strong support for this theory in terms of reliable differences in the structure or function of the basal ganglia. Here, we performed quantitative mapping of brain tissue, which can be used to measure iron content alongside markers sensitive to myelin and thereby offers particular sensitivity to the measurement of iron-rich structures such as the basal gang…

Conversational speech of school-age children after syllable-timed speech treatment for stuttering.

Purpose: The purpose of this laboratory study was to investigate whether rhythmic speech was primarily responsible for stuttering reductions in four school-aged children after the instatement stage of the Westmead Program of syllable-timed speech (STS) intervention. The study was designed to inform further development of the program. Reduction in variability of vowel duration is a marker of STS, and it was predicted that this would be present in the children's conversational speech after Stage 1 of the program if they were using STS. To strengthen such a finding, it was also predicted that the…

After the RESTART trial: six guidelines for clinical trials of early stuttering intervention.

BACKGROUND: The Rotterdam Evaluation Study of Stuttering Therapy randomized trial (RESTART) was seminal, comparing the Lidcombe Program with RESTART Demands and Capacities Model-based treatment (RESTART-DCM) for pre-school age children who stutter. AIMS: To critique the methods of the RESTART trial to develop guidelines for its systematic replication and extension. Beyond that, to contribute to the refinement of existing methodological guidelines for early stuttering intervention. METHOD: The discussion is organized around methodological issues of primary outcomes, treatment completion, clin…

Newly Recognized Stuttering in Three Young Children Following the Hojedk Earthquake in Iran.

Natural disasters, particularly earthquakes, in addition to physical complications, have always had psychological consequences for those affected by them. Stuttering is one of the psychological consequences of shocking events. After a 6.6 magnitude earthquake in Hojedk, Kerman, Iran, two 5-year-old children and a 4-year-old child with symptoms of discontinuous speech (including repeated sound, syllable, and words) were referred to the Kerman Welfare Organization's rehabilitation center (Kerman, Iran). After history-taking, it became clear that the children had begun to stutter after the earthq…

Visibility graph analysis of speech evoked auditory brainstem response in persistent developmental stuttering.

In this research, the concept of fractality based on nonlinear science and chaos theory is explored to study and evaluate the complexity of speech-evoked auditory brainstem response (s-ABR) time series in order to capture its intrinsic multiscale dynamics. The visibility graph of the s-ABR series is proposed as a quantitative method to differentiate subjects with persistent developmental stuttering (PDS) from the normal group. Differential complexities between normal and PDS subjects is quantified using Graph index complexity (GIC). The model is applied to 14 individuals with PDS and 15 normal…

Subtypes of stuttering determined by latent class analysis in two Swiss epidemiological surveys.

AIMS: Associations between stuttering in childhood and a broad spectrum of risk factors, associated factors and comorbidities were examined in two large epidemiological studies. Subtypes of stuttering were then identified based on latent class analysis (LCA). METHODS: Data were from two representative Swiss population samples: PsyCoLaus (N = 4,874, age 35-82 years) and the ZInEP Epidemiology Survey (N = 1,500, age 20-41 years). Associations between stuttering and sociodemographic characteristics, familial aggregation, comorbidity and psychosocial risk / associated factors were investigated in…

Proton Chemical Shift Imaging of the Brain in Pediatric and Adult Developmental Stuttering.

IMPORTANCE: Developmental stuttering is a neuropsychiatric condition of incompletely understood brain origin. Our recent functional magnetic resonance imaging study indicates a possible partial basis of stuttering in circuits enacting self-regulation of motor activity, attention, and emotion. OBJECTIVE: To further characterize the neurophysiology of stuttering through in vivo assay of neurometabolites in suspect brain regions. DESIGN, SETTING, AND PARTICIPANTS: Proton chemical shift imaging of the brain was performed in a case-control study of children and adults with and without stuttering.…
