Photosensitive and extracellular matrix-mimicking hydrogels promote the tendon-bone healing in rotator cuff injury.

The healing of the tendon-bone interface remains a significant challenge due to the lack of suitable bioactive materials for promoting interface regeneration. To address this, we developed a photosensitive hydrogel system designed to mimic the extracellular matrix. This system is based on light-induced imine crosslinking between type I collagen (Col I) and o-nitrobenzyl alcohol (NB)-modified hyaluronic acid (HANB). Experimental results demonstrate that the hydrogel (Col I-HANB) exhibits excellent controllability, low swelling rate, biodegradability, and tissue-adhesive properties. Using a fluo

Chronic Injuries to the Distal Biceps and Triceps Tendons: Current Evidence, Treatment Strategies, and Clinical Implications.

BACKGROUND: Chronic ruptures to the distal biceps and triceps tendons of the elbow are rare but clinically relevant injuries that may result in persistent functional impairment if left untreated. Tendon retraction, scarring, and degeneration significantly complicate surgical management and require meticulous diagnostic and therapeutic planning. OBJECTIVE: This review summarises current evidence on the epidemiology, diagnosis, and treatment strategies for chronic ruptures to the tendon of the distal biceps and triceps tendon and aims to provide clinically applicable recommendations. Patellar Tendon Reconstruction with Allograft in an Ultra-Low-Velocity Multiligamentous Knee Injury: A Case Report.

CASE: A 33-year-old obese man presented with knee pain and instability after an injury at a trampoline park. He had a patellar tendon rupture, ACL rupture, and MCL rupture. He underwent a 2-stage reconstruction; intraoperative findings required shifting from the initial plan of primary patellar tendon repair to reconstruction using a hemipatella patellar tendon allograft with tibial tubercle bone block. CONCLUSION: The objective of this case report was to describe a segmental patellar tendon defect initially planned for primary repair but found unamenable intraoperatively due to the subacute

Nerve and Tendon Transfers for Upper Extremity Reconstruction in Spinal Cord Injury Patients: A Systematic Review.

Background: Spinal cord injury (SCI) often results in upper extremity paralysis, significantly limiting independence and quality of life. Surgical interventions, including nerve and tendon transfers, offer the potential to restore critical functions such as hand grasp and elbow extension. However, the utilisation of these techniques remains limited despite promising outcomes. As such, this study aimed to systematically review the literature on the clinical outcomes of nerve and tendon transfers for upper extremity reconstruction in patients with cervical SCI. Methods: A systematic search was c

Triceps-to-Biceps Tendon Transfer Technique for Restoration of Elbow Flexion in Adult Brachial Plexus Injury.

Restoration of elbow flexion is a priority in treating adult traumatic brachial plexus injuries. A tendon transfer is an ideal option for patients not candidates for reconstructive nerve surgery or free-functioning muscle transfer. For patients with a partial brachial plexus injury or a pan plexus injury with adequate recovered triceps function and loss of elbow flexion, a triceps-to-biceps tendon transfer is a nonmicrosurgical option to restore elbow flexion. The technique of triceps-to-biceps transfer in which the complete triceps tendon is transferred laterally, secured lateral to the radia

Effectiveness of a novel finger range-of-motion brace for extensor tendon injury: a report of 10 patients.

BACKGROUND: Extensor tendon injuries require surgical repair, followed by rehabilitation to ensure optimal outcomes. Immobilization has been the cornerstone of postoperative management. However, immobilization after surgery frequently makes the finger stiffness, often resulting in reduced functionality and quality of life for patients. Recent studies indicate that early controlled motion can significantly improve outcomes, but safe early range of motion (ROM) exercise is a significant clinical challenge. This article aims to check the efficacy of the novel designed finger ROM brace for prevent

Extensor Tendon Injury After Volar Locking Plating for Distal Radius Fractures: A Systematic Review.

Distal radius fractures are common orthopedic injuries. Treatment has varied historically, but volar locking plating currently predominates. Although flexor tendon injury is a well-studied complication of this operation, extensor tendon injury is less well studied. The purpose of this review is to search the literature and present the epidemiology, presentation, and treatment of this complication. The Cochrane, EMBASE, PubMed, and SCOPUS databases were searched for the terms "volar" + "radius" + ("plate" OR "plating") + "extensor." Ninety final studies were included for analysis in this review

Central band reconstruction for the treatment of Essex-Lopresti injury : A novel technique using the brachioradialis tendon.

Longitudinal stability of the forearm is mainly provided by three structures: the radiocapitellar contact, which acts as the primary stabilizer, the central band of the interosseous ligamentous complex (IOLC) and the intact triangular fibrocartilage complex (TFCC). In an Essex-Lopresti lesion the forearm becomes fully destabilized, since all of these three components are injured. Fixation or replacement of the radial head with a metallic prosthesis along with repair of the TFCC and stabilization of the distal radioulnar joint (DRUJ) are well-established treatment goals. However the reconstruct

Posture-dependent changes in corticomotor excitability of the biceps after spinal cord injury and tendon transfer.

Following tendon transfer of the biceps to triceps after cervical spinal cord injuries (SCI), individuals must learn to activate the transferred biceps muscle to extend the elbow. Corticomotor excitability of the transferred biceps may play a role in post-operative elbow extension strength. In this study, we evaluated whether corticomotor excitability of the transferred biceps is related to an individuals' ability to extend the elbow, and whether posture and muscle length affects corticomotor excitability after SCI and tendon transfer similarly to the nonimpaired biceps. Corticomotor excitabil

Mechanisms of increased passive compliance of hamstring muscle-tendon units after spinal cord injury.

BACKGROUND: People with spinal cord injury sometimes develop abnormally compliant hamstring muscle-tendon units. This study investigated whether the increased muscle-tendon compliance is due to a change in the passive properties of the muscle fascicles or tendons, or to muscle tears. METHODS: Semimembranosus muscle fascicle lengths were measured from ultrasound images obtained from 15 spinal cord injured subjects and 20 control subjects while the hip was passively flexed with the knee extended. Semimembranosus muscles of spinal cord injured subjects were inspected for tears using ultrasound i

Longitudinal effects of anterior cruciate ligament injury and patellar tendon autograft reconstruction on neuromuscular performance.

We examined persons after anterior cruciate ligament injury and for 1.5 years after anterior cruciate ligament reconstruction to analyze changes in anterior knee laxity, lower extremity muscle strength, endurance, and several parameters of neuromuscular function. Sixteen men and nine women (average age, 23.8 years) were evaluated preoperatively, then underwent intraarticular autogenous patellar tendon anterior cruciate ligament reconstruction by the same surgeon and were evaluated at 6, 12, and 18 months postoperatively. Muscle strength was measured isokinetically and neuromuscular function wa

Flexor tendon tears in the hand: use of MR imaging to diagnose degree of injury in a cadaver model.

OBJECTIVE: Treatment of flexor tendon lacerations of the finger partly depends on the degree of injury, which is difficult to determine clinically. We used a cadaver model to investigate the potential of MR imaging in evaluating these injuries. MATERIALS AND METHODS: A scalpel was drawn transversely across the volar surface of four cadaver hands, producing various flexor tendon injuries. MR imaging of each hand was performed using axial two-dimensional spin-echo and three-dimensional gradient-recalled-echo sequences. The three-dimensional data sets were interactively reformatted along the lon

MRI of trauma and sports-related injuries of tendons and ligaments. Part I: Spine and upper extremities.

MRI is valuable in evaluation of tendons and ligaments in sports-related injuries. Tendons and ligaments show dark signal intensity on all MR sequences. Alteration in size, shape, continuity, and signal intensity of these structures can be well visualized with MRI. Impingement syndrome is the leading cause of atrophy and degeneration of the rotator cuff. Shape of the acromion, presence of subacromial spur, and involvement in certain sport activities that lead to shoulder instability may cause impingement of the rotator cuff. This article systematically reviews the injuries of tendons and ligam

[The most common tendon injury in sports--hammer finger. Origin, classification, diagnosis and rational therapy].

The closed rupture of the distal digital extensor tendon at its attachment on the terminal phalanx the so-called mallet finger - is seen very often in ball games ("basketball finger", "baseball finger"). Closed rupture with and without a bony fragment can be distinguished. We demonstrate a rational and anatomy related way of treatment.

Irreducible Lisfranc Injury Caused by Tibialis Anterior Tendon Entrapment: Surgical Strategy and Literature Review.

Lisfranc injuries are difficult to treat, particularly when reduction is blocked by soft-tissue interposition. Tibialis anterior tendon entrapment is a rare cause of irreducible Lisfranc dislocation, with only nine cases reported since 1950. We report the tenth case in a 65-year-old woman who sustained a Lisfranc fracture-dislocation after a trampoline accident. Emergency treatment consisted of closed reduction and temporary percutaneous fixation, followed by staged open reduction and internal fixation after soft-tissue swelling subsided. At definitive surgery, the tibialis anterior tendon was
