PURPOSE: To report the results of treatment of Brown syndrome using Achilles tendon allograft as a superior oblique tendon expander.
METHODS: Achilles tendon allografts were prepared from the tissue bank. Five eyes of 3 patients with congenital Brown syndrome (mean age, 6 years) were enrolled in the study. Surgery was performed with a superotemporal conjunctival fornix approach. The superior oblique tendon was exposed and severed; then a 2- to 3-mm-wide strip of Achilles tendon (with a thickness of 1-2 mm) was trimmed and sutured between the cut ends using double-armed 5-0 Mersilene sutures.

A simultaneous traumatic complete rupture of the patellar tendon and the contralateral quadriceps tendon is reported to occur in patients with renal failure and other inflammatory diseases, but is extremely rare in a healthy individual because of the different contributory factors and mechanisms of injury. We present a rare case report of such a combination of injuries in a 48-year-old healthy man. To our knowledge only three such cases have been reported in the English literature. This is an unusual combination and hence there is potential for missed diagnosis leading to suboptimal treatment.

Extensor hallucis longus tendon injuries are uncommon, representing < 2% of tendon injuries. Lacerations are more common than spontaneous ruptures, and if neglected are often difficult to primarily repair because of tendon retraction and scarring. Few reports address the operative treatment of chronic extensor hallucis longus tendon injuries. To our knowledge the use of a gracilis tendon autograft has not been reported. We describe the use of this free tendon autograft with a hallux interphalangeal joint arthrodesis in one patient.

To restore function of the ruptured extensor pollicis longus (EPL) tendon, two operative principles have been described: reconstruction of the original tendon using a tendon interposition graft or a tendon transposition operation, most commonly transposition of the extensor indicis. The patellar tendon donor site of 20 patients who underwent anterior cruciate ligament (ACL) reconstruction using the patellar tendon tissue as autograft was examined with high resolution 7.5 MHz ultrasound.
