Rapid reduction in tenosynovitis of the wrist and fingers evaluated by MRI in patients with rheumatoid arthritis after treatment with etanercept.

OBJECTIVE: To assess the efficacy of etanercept in reducing tenosynovitis evaluated by MRI of the hand (h-MRI) in patients with active rheumatoid arthritis (RA) refractory to disease-modifying antirheumatic drug (DMARD) after 6 weeks of treatment. METHODS: 31 patients with active RA defined by a disease activity score (DAS28) >3.2 and synovitis in the hands were randomised into two groups: 19 patients received 50 mg weekly subcutaneous etanercept added to previous DMARD treatment and 12 patients continued with previous DMARD therapy. Clinical evaluation, blood tests, functional capacity evalu…

Prevalence of De Quervain's Tenosynovitis among Mobile Phone Using Students in Pakistan: A Systematic Review and Meta-Analysis.

BACKGROUND: De Quervain's tenosynovitis (DQT) is a growing ergonomic health issue among mobile phone users. It affects students' thumbs and wrists, limiting their thumb movement and reducing work productivity. However, there is limited epidemiological data among students. Therefore, this systematic review and meta-analysis aimed to find the pooled prevalence of DQT among students in Pakistan. METHODS: A careful search of cross-sectional studies that showed the prevalence of DQT among students in Pakistan was conducted. Studies found in PubMed, Semantic Scholar, HINARI, ScienceDirect, and Goog…

MRI-based extensor carpi ulnaris tendon cross-sectional area as a quantitative biomarker for extensor carpi ulnaris tenosynovitis.

BACKGROUND: Extensor carpi ulnaris tenosynovitis (ECUT) is a common cause of ulnar-sided wrist pain and is often associated with tendon hypertrophy. Although extensor carpi ulnaris tendon thickness (ECUTT) has been used to assess tendon enlargement, a one-dimensional measurement may not fully reflect morphological changes of the tendon. We therefore investigated the diagnostic value of extensor carpi ulnaris tendon cross-sectional area (ECUTCSA) measured on magnetic resonance imaging (MRI) as a quantitative imaging biomarker for ECUT. METHODS: In this retrospective case-control study, MRI exa…

OMERACT International Consensus for Ultrasound Definitions of Tenosynovitis in Juvenile Idiopathic Arthritis: Systematic Literature Review and Delphi Process.

OBJECTIVE: Synovitis and tenosynovitis are present in juvenile idiopathic arthritis (JIA), both as joint pain and/or inflammation, making them difficult to detect on physical examination. Although ultrasonography (US) allows for discrimination of the 2 entities, only definitions and scoring of synovitis in children have been established. This study was undertaken to produce consensus-based US definitions of tenosynovitis in JIA. METHODS: A systematic literature search was performed. Selection criteria included studies focused on US definition and scoring systems for tenosynovitis in children,…

Bridge over Troubled Tendon? A Novel Case of Heterotopic Ossification of Lister's Tubercle Causing Extensor Pollicis Longus Tenosynovitis.

INTRODUCTION: Distal radius fractures are a common injury in adults, and surgical treatment frequently consists of open reduction internal f … d bridging bone creating a tunnel encasing the extensor pollicis longus (EPL) tendon with extensive tenosynovitis but without fraying. Surgery consisted of HO excision, tenosynovium debridement, screw removal, and transposition of the EPL tendon superficial to the extensor retinaculum. At follow-up, the patient had decreased pain and restoration of range of mot …

Bilateral Supernumerary extensor carpi radialis brevis tendons mimicking de quervain's tenosynovitis: A rare cause of radial wrist pain.

We present a rare case of bilateral supernumerary extensor carpi radialis brevis (ECRB) tendons in a 35-year-old male with chronic radial wrist pain. MRI demonstrated 3 tendons in the second extensor compartment of both wrists, mimicking De Quervain's tenosynovitis. Anatomical variations such as this may contribute to symptoms and must be considered in differential diagnosis. Awareness of these findings is essential for accurate diagnosis and appropriate treatment.

Transverse versus longitudinal skin incision in first extensor tendon release for radial styloid tenosynovitis: a randomized controlled trial.

BACKGROUND: De Quervain's tenosynovitis, characterized by the entrapment of the extensor pollicis brevis and abductor pollicis longus tendons in the first extensor compartment, leads to pain and swelling near the radial styloid. When conservative treatments including rest and antiinflammatory medications fail, surgery is often recommended. This study aimed to compare transverse and longitudinal skin incisions with respect to surgical scars, postoperative pain, and functional outcomes in the open release of the first extensor compartment in radial styloid tenosynovitis. METHODS: Seventy patien…

Significance of early diagnosis and surgical management in treating Mycobacterium immunogenum-related pyogenic extensor tenosynovitis: a case report.

BACKGROUND: Non-tuberculous mycobacteria (NTM) are environmental organisms that are increasingly contributing to human infections. Mycobacterium immunogenum, a variant of NTM discovered in 2001, is a rapidly growing mycobacterium that exhibits multidrug resistance. Reports of infections caused by this organism, particularly tenosynovitis in the musculoskeletal system, are limited. CASE PRESENTATION: A 71-year-old female with vesicular pemphigus, undergoing immunosuppressive therapy, presented with a progressively enlarging tumour on the dorsum of her right hand, along with erythematous papule…

Incidence and Risk Factors for Pregnancy-Related de Quervain's Tenosynovitis in South Korea: A Population-Based Epidemiologic Study.

BACKGROUND: Although pregnant or lactating women have been recognized to be predisposed to de Quervain's tenosynovitis (DQT), there is a lack of epidemiologic evidence. The purpose of this study was to estimate the nationwide incidence of pregnancy-related DQT (PRDQT) and to analyze risk factors using the Korean National Health Insurance (NHI) database. METHODS: A retrospective epidemiologic study of pregnant women in South Korea from 2013 to 2017 was conducted using the NHI claims database. Using corresponding diagnostic codes, we identified women diagnosed with DQT during pregnancy or the p…

Are Patient Expectations and Illness Perception Associated with Patient-reported Outcomes from Surgical Decompression in de Quervain's Tenosynovitis?

BACKGROUND: Psychological factors such as depression, pain catastrophizing, kinesiophobia, pain anxiety, and more negative illness perceptions are associated with worse pain and function in patients at the start of treatment for de Quervain's tenosynovitis. Longitudinal studies have found symptoms of depression and pain catastrophizing at baseline were associated with worse pain after treatment. It is important to study patients opting for surgery for their condition because patients should choose surgical treatment based on their values rather than misconceptions. Psychological factors associ…

Proximal Linear Extension of Skin Hypopigmentation After Ultrasound-Guided Corticosteroid Injection for de Quervain Tenosynovitis: A Case Presentation.

UNLABELLED: de Quervain tenosynovitis is a stenosing tenosynovitis of the first dorsal compartment of the wrist that can lead to painful functional impairment of the upper limb. This case presentation describes a rare adverse effect of corticosteroid injection (CSI) involving local skin atrophy and hypopigmentation with proximal linear extension. In this case, hypopigmentation developed from the wrist to beyond the elbow after CSI with ultrasound guidance and targeted placement of the injectate in the extensor tendon sheath of the first dorsal compartment. Dermal complications of CSI are rare …

Treatment of digital pyogenic flexor tenosynovitis: single open debridement, irrigation, and primary wound closure followed by antibiotic therapy.

INTRODUCTION: Digital pyogenic flexor tenosynovitis requires fast, aggressive treatment. Although this infection occurs frequently, treatment consensus is lacking. MATERIALS AND METHODS: Between 2011 and 2015, 22 patients with acute pyogenic flexor tenosynovitis were treated with a single open debridement followed by irrigation; the incision was closed and a 10-day antibiotic course was administered. The average incision-to-suture time was 25 min, and the average hospital stay was 4 days. Recovery was uncomplicated for 20 patients, while two were reoperated, one due to germ resistance and the…

Emerging Forms of Avian Orthoreovirus Infection in Turkeys in Québec, Canada, Associated with Tenosynovitis, Hepatitis, and Encephalitis (2020-2022).

New avian orthoreovirus strains have emerged in commercial turkey in North America since 2011, mostly characterized by tenosynovitis and rarely with a necrotizing hepatitis in younger birds. In 2020, orthoreovirus infections were diagnosed in turkey flocks for the first time in Québec, leading to significant economic losses in young poults and in older turkeys. At necropsy, older birds had a lymphoplasmacytic tenosynovitis occasionally associated with aortic rupture. In younger poults, the lesions were mostly characterized by hepatocellular necrotic foci and degenerated syncytial hepatocytes. …

Bilateral Wrist Tenosynovitis owing to Acute Conversion of Adult T-Cell Leukemia-Lymphoma in a Patient with Rheumatoid Arthritis.

Human T-cell leukemia virus type 1- (HTLV-1-) associated arthritis is a relatively common disease. However, tenosynovitis owing to adult T-cell leukemia-lymphoma (ATL) is a rare condition. To the best of our knowledge, there have been no reports of tenosynovitis caused by conversion to acute ATL from one of the other ATL types. We present the case of a 60-year-old woman with rheumatoid arthritis (RA) with bilateral wrist tenosynovitis owing to the conversion to acute ATL from one of the other ATL types. She had swelling around the bilateral wrist joint under well controlled RA inflammation. Sh…

Which Psychological Variables Are Associated With Pain and Function Before Surgery for de Quervain's Tenosynovitis? A Cross-sectional Study.

BACKGROUND: Depression, anxiety, and pain catastrophizing have been associated with worse pain and function in studies of patients with de Quervain's tenosynovitis. Illness perceptions are the patient's thoughts and feelings about their illness. More negative perceptions of the illness such as the illness having a long duration or serious consequences are associated with worse physical function in patients with hand osteoarthritis. It is currently unknown whether these psychological factors play a similar role in de Quervain's. We chose to study patients who have tried nonoperative management …
