Defective route finding is a symptom in which an indivisual demonstrates inhibility to find the way in the familiar places. This symptom arises from lesions at the right parieto-occipital region and the right posterior part of the splenium of the corpus callosum. We present a case of defective route finding resulting from the right anterior thalamic infarction. A 63-year-old right-handed man abruptly presented with difficulty in route finding on his way home. Neither spatial neglect nor agnosia was observed with pictures of his house and surrounding streets. Although there was an attention dis

Shivering is mediated via the predominantly uncrossed reticulospinal tract. We report a case of a patient with left lateral medullary tract infarct involving the reticulospinal tract who displayed ipsilateral absence of shivering during a febrile episode. The physiology and anatomy of shivering and the reticulospinal tract is reviewed.

Essential tremor can be surgically treated by thalamotomy or deep brain stimulation. During the surgical procedure for both of these procedures, a transient microthalamotomy effect has been observed. This benefit is usually short-lived and appears to be predictive of patient outcome. This report describes a patient who underwent a temporary placement of a stimulating electrode within the ventral intermediate nucleus of the motor thalamus. The procedure was aborted intraoperatively and the patient demonstrated a microthalamotomy effect. This microthalamotomy effect has remained persistent for 2

Traditionally, the dorsal column-medial lemniscus system has been viewed as a pathway not involved in pain perception. However, recent clinical and experimental studies have provided compelling evidence that implicates an important role of the dorsal column pathway in relaying visceral nociceptive information. Several clinical studies have shown that a small lesion that interrupts fibers of the dorsal columns (DC) that ascend close to the midline of the spinal cord significantly relieves pain and decreases analgesic requirements in patients suffering from cancer originating in visceral organs.

Dopamine deficiency causes disinhibition and overactivity of the subthalamic nucleus (STN). Output neurons from the STN are excitatory and use glutamate as a neurotransmitter. They project to the external and internal segments of the globus pallidum (GPe and GPi), the substantia nigra pars reticulata (SNr), and the pedunculopontine nucleus (PPN). In addition, STN neurons provide excitatory innervation to dopaminergic (DA) neurons in the substantia nigra pars compacta (SNc) that contain glutamate receptors. Stimulation of the STN induces bursting activity in SNc dopaminergic neurons. This raise

The relative numbers and locations of neurons projecting to the "forelimb" region of the precentral motor cortex were studied in three monkeys by using the retrograde transport of horseradish peroxidase. Within the forelimb area of the motor cortex itself, there are extensive and profuse interconnections. However, regions within this area receive afferents from very few neurons in other parts of the motor cortex representing hindlimb or head movements. Most of the motor cortical representation of the forelimb in the anterior bank of the central sulcus is devoid of callosal connections. In both

The authors report the results of a long-term follow-up study of the effects of the physiologically defined selective VIM (nucleus ventralis intermedius)-thalamotomy on tremor of Parkinson's disease in 27 patients and essential tremor in 16 patients. The follow-up period ranged from 3.25 to 10 years (mean 6.58 years). In 43 patients a total of 50 operations (including four bilateral operations and three reoperations) were carried out. The early (2 to 4 weeks after surgery) and late effects on the tremors were determined clinically and electromyographically. Fourteen parkinsonian cases were tre

Corticocortical afferents to both cortical walls of the cat middle suprasylvian sulcus (MSs area) were investigated by means of retrograde axonal transport of horseradish peroxidase (HRP). the visual cortex (V-I and V-II) projects to the medial wall of the MSs, the projection from V-II being heavier. The auditory cortex (A-I, A-II,. and Ep), including cortical walls of the dorsal part of the anterior and posterior ectosylvian sulci, sends fibers to the lateral wall of the MSs. Connections from the first auditory area (A-I) are heavier than from the second (A-II). In the rostral part of the MSs

The retinal afferents to the medial interlaminar nucleus and to its rostro-dorsal extension at the edge of the pulvinar have been studied in cats by fiber degeneration and autoradiographic methods. Fiber degeneration following section of one optic nerve shows two distinct retinal inputs: One is coarse-fibered and goes to the medial interlaminar nucleus itself; the second, which is fine-fibered, goes to the rostral and medial borders of the medial interlaminar nucleus and continues into the pulvinar. The regions in receipt of these fine fibers have been called the "geniculate wing". The topogra

The present study has been undertaken to elucidate the thalamic projections to the posterior suprasylvian gyrus (PSG) and the ventrally adjacent areas in the cat using retrograde axonal transport of horseradish peroxidase. The posterior part of the PSG, area 19 b, receives fibers mainly from the dorsolateral part of the medial pulvinar nucleus, and a lesser number from the lateral pulvinar nucleus. It receives a few fibers from the inferior pulvinar nucleus and the medial interlaminar nucleus (NIM) of the lateral geniculate. The anterior part of the PSG, area 21 b, receives a large number of f

Thalamocortical dysrhythmia is a key pathology of chronic neuropathic pain, but few studies have investigated thalamocortical networks in chronic low back pain (cLBP) given its non-specific etiology and complexity. Using fMRI, we propose an analytical pipeline to identify abnormal thalamocortical network dynamics in cLBP patients and validate the findings in two independent cohorts. We first identify two reoccurring dynamic connectivity states and their associations with chronic and temporary pain. Further analyses show that cLBP patients have abnormal connectivity between the ventral lateral/

Quantitative analyses of cross-sectional areas of the thalami, caudate nuclei, and lentiform nuclei were performed in 29 preterm infants (16 males, 13 females; mean age 29.6 weeks, age range 27 to 24 weeks,) with periventricular leukomalacia (PVL). MRI was carried out in the infants between 9 and 18 months of corrected age and in 16 control infants. Bilateral thalami, caudate nuclei, lentiform nuclei, cerebral hemispheres, and cerebellum were measured by computer. Ratios of the areas of the thalami (Th), caudate nuclei (Ca), lentiform nuclei (Le), and cerebral hemispheres (CH) to that of the c

As the symptom of rapid eye movement sleep behavior disorder (RBD) is paroxysmal during sleeping, the disruptions of dynamic functional connectivity (FC) between thalamic subnuclei and the cortex may play a critical role in Parkinson's disease (PD). A total of 35 PD patients with probable RBD (PD-pRBD) and 40 PD patients without probable RBD (PD-npRBD) and 41 healthy controls were enrolled. All participants underwent functional magnetic resonance imaging scan and clinical assessment. Altered dynamic FC between bilateral 14 thalamic nuclei and cortex was calculated. Although the RBDSQ demonstra…

Movement disorders have been reported as rare complications of stroke. The basal ganglia have been implicated in the pathophysiology of most post-stroke dyskinesias. We outline different types of post-stroke myoclonus and their possible pathophysiology. A middle-aged man developed generalized myoclonus after an ischemic stroke in the superior midbrain and subthalamic nuclei. Spontaneous resolution was seen by 72 hours. A lesion to the subthalamic nuclei disrupted the normal thalamic inhibition, which likely led to the involuntary movements seen in our patient. In this case, myoclonus was gener…
