[A case of postpneumonectomy empyema cured by open drainage thoracotomy and thoracoplasty with pectoral myoplasty].

The patient was a 57-year-old male with lung adenocarcinoma arising in right upper lobe. Because the tumor invaded to the trunchus intermedius and B6, right upper lobe, middle lobe and S6 was resected with bronchial plasty which was performed between the main bronchus and basal bronchus. Because the bronchial fistula appeared after 12 days of operation, completion pneumonectomy was performed, and the stump of the main bronchus was covered with intercostal muscle. Although fistula of the bronchial stump did not appear, open drainage thoracotomy was performed because of deterioration of empyema …

Comparison of continuous paravertebral and extradural infusions of bupivacaine for pain relief after thoracotomy.

Pain was controlled in 20 post-thoracotomy patients using a continuous infusion of 0.25% bupivacaine through an extradural or para-vertebral catheter. Both techniques provided good analgesia. Hypotension and urine retention occurred significantly less frequently in the paravertebral than in the extradural group.

Anatomical basis of modern thoracotomies: the latissimus dorsi and the "serratus anterior-rhomboid" complex.

The latissimus dorsi and the "serrato-rhomboid" complex are the muscles most often involved in present-day thoracotomies for lung surgery. The present anatomic study emphasizes: the continuity between the serratus anterior and the rhomboid levator scapulae mass as a wide muscular sheet with a deep common fascia, extending the serrato-thoracic space (of Gillis) to the vertebral column as the rhomboserrato-thoracic space (the levator scapulae is situated higher up, above the ordinary thoracotomy); the presence of a "composite fascia" in the posterior angle between the seratus anterior and the rh…

Esophagectomy without thoracotomy: is there a risk of intramediastinal bleeding? A study on blood supply of the esophagus.

In transhiatal blunt esophagectomy there is surprisingly little bleeding if no adjacent great vessels are torn. This prompted an investigation by new injection techniques and corrosion on the human esophageal vasculature three-dimensionally. The three main arterial sources were confirmed: the superior thyroid artery, bronchial arteries at the level of the carina, and the left gastric and splenic artery. Two facts became obvious that were not appreciated hitherto. All major vascular trees divide into minute branches at some distance from the esophagus. Those branches go on to form a dense submu

Etidocaine in intercostal nerve block for pain relief after thoracotomy; a comparison with bupivacaine.

For pain relief after thoracotomy, intercostal nerve block with etidocaine 1% and bupivacaine 0.5%, both containing adrenaline 5 mug/ml, was used. Duration of skin analgesia for sharp pain was around 11 hours for both solutions. Post-operative pain was noted 6 and 5 hours after injection for etidocaine and bupivacaine respectively. No pathological changes in acid-base balance or ventilation were observed. Peak expiratory flow decreased to 35-40% of the pre-operative values and remained at this level for about 12 hours. Arterial and venous blood levels of the local anaesthetics were low and no …

Optimizing Recovery After Thoracotomy: Evidence-Based Non-Opioid and Multimodal Analgesia - A Narrative Review.

PURPOSE OF REVIEW: After open thoracic surgery, post-thoracotomy pain remains an important postoperative problem affecting nearly half of patients. It can give rise to respiratory problems, delayed rehabilitation, prolonged hospitalizations, and chronic post-thoracotomy pain syndrome. Despite the reality that opioids are currently the primary source of anesthesia after surgery, their consumption is restricted by complications, including prolonged dependency, tolerance, and respiratory impairment. RECENT FINDINGS: To maximize recovery after the thoracotomy, this narrative review incorporates s…

Partial Upper Sternotomy versus Right Thoracotomy with Central Cannulation for Surgical Aortic Valve Replacement.

OBJECTIVES: This study compared surgical aortic valve replacement (SAVR) via partial upper sternotomy (PUS) and right anterolateral thoracotomy (RALT) with central aortic cannulation regarding intraoperative perfusion parameters, transfusion requirements, and early postoperative outcomes. METHODS: We retrospectively analyzed 380 patients who underwent isolated SAVR via PUS or RALT. Propensity score matching yielded 107 matched pairs. All patients underwent central aortic and peripheral venous cannulation. RESULTS: Cardiopulmonary bypass, aortic cross-clamp, and total operative times were lon…

Effects of Mirror Therapy-Based Upper Extremity Exercises After Thoracotomy: A Randomized Controlled Trial.

PURPOSE: To investigate the efficacy of mirror therapy-based upper extremity exercises on shoulder pain, shoulder functions, quality of life, and fear of re-injury. DESIGN: Single-blind randomized controlled prospective study. METHODS: This prospective randomized controlled study included patients who underwent surgery using the posterolateral-thoracotomy method. Patients were randomly divided into two groups as control (n = 40) and study group (n = 40). Patients were included in the pulmonary rehabilitation program 5 days a week for 4 weeks. In addition, the study group patients performed t…

Atrial functional mitral regurgitation: cardiac remodeling and outcome after mini-thoracotomy mitral valve surgery.

BACKGROUND: The atrial functional mitral regurgitation (AFMR) refers to a newly recognized disease entity in which mitral regurgitation occurs secondary to left atrial (LA) disease, without left ventricular (LV) dilatation and dysfunction (at least initially) and intrinsic mitral valve (MV) disease. METHODS: We conducted an observational analysis on 28 subjects, mean age and standard deviation 72.7±8.4 years, with AFMR who underwent mini-thoracotomy MV surgery (mitral annuloplasty and tricuspid ring annuloplasty when needed). No surgical treatment of atrial fibrillation (AF) has been performe…

Analysis of Risk Factors for Postoperative Dysuria in Patients Undergoing Thoracotomy under General Anaesthesia.

BACKGROUND: Thoracotomy under general anaesthesia is one of the most difficult surgeries and is prone to result in postoperative complications. This study explored risk factors for postoperative dysuria in patients undergoing thoracotomy under general anaesthesia to provide a reference for the formulation and selection of subsequent clinical management programs. METHODS: Patients undergoing thoracotomy under general anaesthesia (n = 179) admitted to our hospital from June 2019 to June 2021 were selected. They were divided into dysuria group (n = 79) and normal urination group (n = 100) accord…

Right mini-thoracotomy for concomitant aortic valve replacement and right coronary artery bypass graft.

BACKGROUND: Full sternotomy is the standard approach for combined surgery. Evidence of the minimally invasive approach's advantages for aortic and mitral valve surgery has been reported. Our aim was to report our experience with minithoracotomy for elective patients presenting with aortic valve stenosis associated with right coronary artery disease. MATERIAL AND METHODS: Between January 2016 and August 2021, 17 patients underwent concomitant aortic valve replacement and right coronary artery bypass grafting by right anterior thoracotomy. RESULTS: The mean age was 73.3 years and the mean Euro…

Outcomes of minimally invasive isolated tricuspid valve repair and replacement through right mini-thoracotomy.

OBJECTIVE: Isolated tricuspid valve surgery is uncommon and associated with high perioperative morbidity and mortality. We aimed to study the overall outcomes of patients who underwent minimally invasive right thoracotomy tricuspid valve surgery (Mini-TVS), consisting of either tricuspid valve repair (TVre) or replacement (TVR). METHODS: We performed a retrospective analysis of all Mini-TVS procedures (2017-2022), through which we identified isolated tricuspid valve surgeries. We examined in-hospital outcomes, survival analysis over a 4-year period, and competing risk analysis for reoperative…

Thoracotomy Wound Dehiscence After Lung Transplant Is an Effective Surgical Solution: A Case Report.

Post-thoracotomy wound dehiscence after a lung transplant carries with it morbidity in the postoperative period. While this complication has been widely reported in the literature after a clamshell incision, the thoracotomy dehiscence's surgical solution has not received much emphasis. We present an original technical solution to deal with this complication, performed successfully in a 62-year-old woman diagnosed as having idiopathic pulmonary hemosiderosis after a single lung transplant. This surgical treatment consists of necrotic rib tissue removal, pleural effusion drainage, pulmonary adhe…
